PEACE

A system protecting a manned space must be fitted with an approved time delay and alarm arranged to require the alarm to sound for at least 20 seconds or the time necessary to escape from the space, whichever is greater, before the agent is released into the space. Alarms must be conspicuously and centrally located. The alarm must be powered by the extinguishing agent. Vessel's fixed fire shutdown for all four engines was inconsistent during testing. Provide fire service technician report attesting to system service and testing.

Each ring life buoy on a vessel must:(1) Be approved in accordance with § 160.050 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.150. Ring Buoy's grab lines are UV damaged. Replace ring life buoy and life line.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Safetyradiotelephony cert was expired. Get new insepction and certificate.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. First Aid kit does not meet ISO 18813:2006 (4.12) or equivalent. Provide first aid kit as per ISO Standard 18813:2006 or equivalent.

The master or person in charge of a ship to which this section applies shall ensure that the ship is not operated unless a garbage management plan meeting paragraph (b) of this section is on the ship and that each person handling garbage follows the plan.(b) Each garbage management plan under paragraph (a) of this section must be in writing and—(1) Provide for the discharge of garbage by means that meet Annex V of MARPOL, the Act, and §§ 151.51 through 151.77;(2) Describe procedures for minimizing, collecting, processing, storing, and discharging garbage; and(3) Designate the person who is in charge of carrying out the plan. Provide garbage management plan to inspector.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of electrical equipment and and systems, including the following:(a) Inspection of of all cable as far as practicable without undue disturbance of the cable or electrical apparatus; Accommodation lighting junction boxes are missing cable glands and over junction box limit. Provide line diagram of vessel's electrical distribution for the accommodation bunk room to verify circuit design and overcurrent protection.

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10. Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designate member of the crew. Vessel added the monthly fire inspections into the underway checklist.

Electrical equipment on a vessel must be installed and maintained to: Minimize accidental personnel contact with energized parts. Vessel's operating station crew bunks had undocuemented 12 vdc terminals supplying fans and usb outlets. Vessel removed power supply and disconnected all wiring from service.

A durable placard must be posted next to all radiotelephone installations with the emergency broadcast instructions and information, specific to the individual vessel. Vessel is missing emergency radio broadcast placard. Create and post placard IAW 46 CG 184.510 Recommended Emergency Broadcast Instruction to the satisfaction of the attending marine inspector.

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. Interconnected smoke detectors in the galley and lazarette did not activate while testing the machinery space detector. Repair smoke detectors and provide test to the satisfaction of the attending marine inspector.

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10 (incorporated by reference, see § 175.600 of this chapter) as amended here:(i) Certification or licensing by a state or local jurisdiction as a fire extinguisher servicing agency will be accepted by the Coast Guard as meeting the personnel certification requirements of NFPA 10 for annual maintenance and recharging of extinguishers. Lazarette fire extinguisher service date was Feb 2023 and beyond annual NFPA servicing requirement. Provide valid servicing record to the attending marine inspector.

Each life jacket carried on a vessel on oceans, coastwise, or Great Lakes route, must have a life jacket light approved in accordance with § 161.012 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.112. Each life jacket light must be securely attached to the front shoulder area of the life jacket. Four life jacket lights did not activate upon testing. Provide valid test of life jacket lights to attending marine inspector.

Each nozzle must be of corrosion-resistant material and be capable of being changed between a solid stream and a spray pattern. Corrosion-resistant nozzle onboard vessel was leaking and not functioning as designed. Provide replacement corrosion-resistant nozzle for required fire hose to attending marine inspector.

Drug testing programs required by this part must be conducted in accordance with 49 CFR part 40, Procedures for Transportation Workplace Testing Programs. This subpart summarizes the responsibilities of credentialed mariners, marine employers, MRO, SAP and other chemical testing service providers in 49 CFR part 40. Vessel master could not provide proof of chemical testing consortium enrollment. Provide valid chemical testing enrollment to the attending marine inspector.

Waste receptacles. Unless other means are provided to ensure that a potential waste receptacle fire would be limited to the receptacle, waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Vessel had several combustible waste receptacles onboard. Remove all combustible waste receptacles and replace with non-combustible type.

A Certificate of Documentation is required for the operation of a vessel in certain trades, serves as evidence of vessel nationality, and permits a vessel to be subject to preferred mortgages. COD was expired onboard the vessel. Provide valid COD to the attending marine inspector.

The Rules concerning shapes shall be complied with by day. Vessel is required to carry shapes for signaling purposes for restricted in ability to maneuver (ball, diamond, ball) and vessel not under command (ball, ball). Provide day shapes to the satisfaction of the attending marine inspector.

The master of the vessel shall ensure that:(a) Each EPIRB, other than an EPIRB in an inflatable liferaft, must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative;(b) The EPIRB's battery is replaced after it is used, or before the date required by FCC regulations in 47 CFR part 80, whichever comes sooner; and(c) The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. Vessel could not provide any previous test records for the required EPRIB monthly test. Vessel master provided new inspection log and performed monthly inspection on site. Deficiency rectified.

The owner, managing operator, or a qualified servicing facility as applicable must conduct the following inspections and tests:(1) Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10 (incorporated by reference, see § 175.600 of this chapter) as amended here: Monthly inspections required by NFPA 10 may be conducted by the by the owner, operator, person-in-charge, or a designate member of the crew. Vessel could not provide any record of required NFPA monthly portable fire extinguisher inspections. Vessel master did provide new monthly log and performed and recorded the inspection on site. Deficiency rectified on site.

The master shall conduct sufficient fire drills to make sure that each crew member is familiar with his or her duties in case of a fire. Fire fighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information:(1) Date of the drill and training; and(2) General description of the drills. Vessel could not provide any logs or proof of past completion of fire drills. Fire drill was preformed for machinery space during annual inspection. Master accurately logged the event. Deficiency rectified on site.

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons who have fallen overboard. Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information:(1) Date of the drill and training; and(2) General description of the drill scenario and training topics. Vessel could not provide logs or proof of past completion of MOB or abandon ship drills. MOB and abandon ship drills were conducted during annual inspection. Master accurately logged the event. Deficiency rectified on site.

A device must be provided to automatically shut down power ventilation serving the protected space and engines that draw intake air from the protected space prior to release of the extinguishing agent into the space. The fixed fire suppression system did not shut down the engines or generator in the engine space upon activation of the pressure sensor. Engine space power ventilation is designed to be shut down upon generator shut down. Prior to vessel departure, demonstrate proper operation of engine and ventialtion shut downs upon activation of the fixed fire suppression system.

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. EPIRB hydrostatic release was found to be two years over expiration date. Provide current hydrostatic release prior to departure.

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Vessel's anchor light was not functioning. Provide functioning anchor light prior to 16MAY23.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. First aid kit analgesic medications were found expired. Provide first aid kit meeting ISO 18813:2006(E).

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets and immersion suits. Identification of the items may be in words, or the appropriate symbols in IMO Resolution A.760(18). Letters and numbers must be at least 50 millimeters (2 inches) high. Symbols must be at least 100 mm (4 inches) square. Provide accurate life jacket location markings of required size.

Electrical equipment on a vessel must be installed and maintained to: (c) Minimize accidental personnel contact with energized parts. Two dead end cables found under pilothouse console. Remove cables or terminate ends in accordance with 46 CFR 183.320.

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB. To enhance protection of life and property it is mandatory that each 406.0-406.1 MHz EPIRB be registered with NOAA before installation and that information be kept up-to-date. Vessel was unable to provide a current NOAA EPIRB registration. Provide current registration.

Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck from all passenger and crew accommodation spaces located below the main deck. Lower bunk rooms are not fitted with emergency lighting. Provide emergency lighting.

On passenger decks of a ferry or of a vessel on an excursion trip the following must be installed: (i) Bulwarks; (ii) Chain link fencing or wire mesh that has openings of not more than 4 inches in diameter; or (iii) Bars, slats, rail courses, or an equivalent spaced at intervals of not more than 100 millimeters (4 inches). Bow gate was found broken off. Replace gate or provide sufficient rail courses.

An internal structural examination conducted in compliance with § 176.600 may be conducted while the vessel is afloat or out of the water and consists of a complete examination of the vessel's main strength members, including the major internal framing, the hull plating and planking, voids, and ballast, cargo, and fuel oil tanks. During inspection excessive decay was found in the port side of the engine space, specifically along horizontal hull stiffeners, in way of the deck sink drain penetration and in way of the forward end of the outboard longitudinal deck stiffener. Remove rot to good wood and treat remaining wood.

Suitable storm rails or hand grabs must be installed where necessary in passageways, at deckhouse sides, and at ladders and hatches. The starboard side ladder rail was loose on the external ladder from the main deck to the upper deck. Repair rail.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. A wood screw was accidently driven into a 100 Amp circuit cable running through the forward bunk room space. Replace cable in kind or provide plans for modification.

All cable and wire must not be installed with sharp bends. Cable installations throughout vessel were found installed with sharp bends. Remove bends.

Each connection to a conductor or terminal part of a conductor must be made within an enclosure. Terminal blocks were found throughout vessel with no protective enclosure. Provide enclosures.

Fuses must be of the cartridge type only and be listed by Underwriters Laboratories or another independent laboratory recognized by the Commandant. In-line fuses for watch alarm system were found to be an improper type. Provide approved cartridge type fuses.

Each circuit breaker must meet UL 489 (incorporated by reference; see 46 CFR 175.600) or other standard specified by the Commandant, and be of the manually reset type designed for: (1) Inverse time delay; (2) Instantaneous short circuit protection; and (3) Switching duty if the breaker is used as a switch. Vessel was unable to demonstrate compliance of newly installed breaker for roving watch alarm system.

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. Vessel did not install an interconnected fire detection system by 27DEC2022. Vessel is restricted to a LIMITED COASTWISE route until it's compliance with the interconnected fire detection requirements of 46 CFR 181.405 have been verified by a USCG marine inspector.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Repairs or alterations that affect the safety of the vessel include, but are not limited to: replacement, repair, or refastening of deck or hull planking, plating, and structural members; repair of plate or frame cracks; damage repair or replacement, other than replacement in kind, of electrical wiring, fuel lines, tanks, boilers and other pressure vessels, and steering, propulsion and power supply systems; alterations affecting stability; and repair or alteration of lifesaving, fire detecting, or fire extinguishing equipment. The owner or managing operator shall submit drawings, sketches, or written specifications describing the details of any proposed alterations to the cognizant [name withheld] alterations must be approved by the OCMI before work is started. Vessel replaced galley stove/oven, chest freezer and added a dishwasher. Provide data plates showing change in amperage draw for affected circuits.

An individual may not be engaged or employed, including self-employment, on a vessel in a position as master, operator, or person in charge for which a credential is required by law or regulation unless all crewmembers covered by this section are subject to the random testing requirements of this section. Vessel crew included two mariners who were part of a different marine employer's program. Provide documentation that shows marine employer's notification in case of positive random drug testing results.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessels operated more than 100 nautical miles but not more than 200 nautical miles from the nearest land must be equipped as required in 47 CFR 80.905(a)(3). Vessel does not have a functioning DSC-MF, DSC-SSB or GMDSS-approved Inmarsat ship earth station. Issued FCC Safety Radiotelephony certtificate states that the vssel is equipped with MF and HF radios. Provide appropriate radio equipment of provide FCC authorization for use of satellite phone in lieu of the above equipment.

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. Visual alarm for anchor locker high level alarm not operable. Repair and demonstrate proper operation.

Each connection to a conductor or terminal part of a conductor must be made within an enclosure and have either: (1) A pressure type connector on each conductor; (2) A solder lug on each conductor; (3) A splice made with a pressure type connector to a flexible lead or conductor; or (4) A splice that is soldered, brazed, or welded to a flexible lead or conductor. The bilge high level alarm in the fore peak was found with an improper splice. Properly splice conductor.

The owner, managing operator, or a qualified servicing facility as applicable must conduct the following inspections and tests: Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10 (incorporated by reference, see § 175.600 of this chapter) as amended here: Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Vessel did not have records of monthly portable fire extinguisher inspections. Provide record.

If your vessel's Certificate of Inspection is renewed less often than annually, the marine inspector must check the settings of the safety or relief valves on all pressure vessels, except cargo tanks, twice every 5 years. No more than 3 years may elapse between any check and its immediate predecessor. Vessel was unable to demonstrate relief of safety valve at set pressure. Demonstrate safety relief valve to satisfaction of the attending USCG marine inspector. Until resolved, vessel is prohibited from using installed pressure vessel.

Bilge systems must be fitted with a check valve in the bilge line. Bilge system found witout check valves. Provide check valves in bilge lines.

Rail courses are required every four inches. Upper deck rails not fitted with courses. Provide courses.

VHF DSC radio must be provided with MMSI and GPS input. VHF installation found without MMSI and GPS. Provide ppoperly configured DSC VHF radio.

Ring life buoy grab lines deteriorated. Renew grab lines or replace ring buoy.

Records of EAP training for employees and supervisors not available. Provide training and documentation.

Significant rot found throughout lazarette. Remove rot to satisfaction of attending USCG Marine Inspector. Following verification of removal, treat wood. Areas of concern include but are not limited to: port side forward overhead in way of water tank vent, port water tank supports, decking supports forward/midships, longitudinal floor beams.

Provide ammeter for battery charging system in pilot house.

Repair latch and gasket for port engine room ventilation closure and repair hole in starboard engine room ventilation passage.

Properly mount bilge high level float switch in lazarette.

Provide single version of emergency procedures.

Matress must meet 46cfr1632 unless O/O can prove they were installed prior to 03/11/1996. O/O shall change out matresses or provide a statement attesting to its compliant to old T.

Vessel shall have covered metal trash containers in all lounge areas. Vessel had plastic trash containers in these spaces. Vessel shall provide metal trash containers.

Vessel shall have a P.A. system and a General alarm for overnight accommodations. Vessel did not have adequate coverage of both systems. Provide a speaker in the bunk room.

Renew expired FCC safety certificate.
