The ship shall be fitted either with permanent bulwarks at least 1m in height, specially stiffened on the upper edge and supported by strong bulwark stays attached to the deck and provided with necessary freeing ports, or with efficient rails of the same height and of specially strong construction. There are (3) stiffener brackets on the forward weather deck (IVO mooring chocks) that are holed. D-rings and/or cleat adjacent to the wasted stiffeners are also corroded and not suitable for service. Additionally, the railing by the port gangway hydraulic control box is severed. ICLL II/43(3)

A marine casualty occurred on 27 May 2018 in which a mooring line parted and severely injured multiple crew. Upon inspection, the vessel's mooring lines were found heavily worn and deteriorated and the vessel had no accurate records of installation, maintenance, or inspections. Vessel shall develop and implement planned maintenance as outlined in the vessel's SMS. 33 CFR 96.240(b) ISM Code Part A 10.2
