Vessel shall be maintained in a safe and seaworthy condition. Vessel has an installed module on deck that is determined to be a Portable Accommodation Module (PAM) IAW CG-ENG Policy Letter 01-16 (PAM is an offshore lab where person(s) will take shifts to monitor equipment and readings) that has deficiencies in Electrical installation, Lifesaving, Fire Detection and fire-fighting equipment, Fire Hazards, and Working Conditions. During the inspection, the following was observed, but not limited to: The electrical wiring was using extension cords and splitters were interconnected ("daisy chained"). Module lacked fire detection, emergency lighting, emergency communication of the general alarm and PA system, and lifejackets. Flammable wooden shelving (2X4 and Plywood) were built into the module. Fire/Safety plan was not updated with the modules. Vessel's electrical system has been modified as Module and survey equipment is being powered by ship's electrical system with power cords being run from the top of the bridge down to the modules.

Fuel system piping including RACORs shall be steel, copper or nickel copper. Vessel did not have a metal bottom shell for the RACOR.

Vessel shall remail watertight. Vessel had a top watertight penetration and QAWTD or ER Escape had loose dogs.

A survey shall be made every time an accident occurs which affects the safety of the vessel. vsl was involved in a marine casualty causing damage. vsl shall be repaired in accordance with all regulations in applicable subchapters. vsl may movee to local shipyard for reparis upon notification of msu houma marine inspectors.

Vessel must undergo an annual inspection within 3 months before or after each anniversary date. vessel did not complete inspection within required timeframe. 46 CFR 126.510 17c

No repairs or alterations may be made to the hull or machinery without notice to the cognizant OCMI in the inspection zone where the repairs are to be made.

Vessel must have adequate emergency lighting fitted along the line of escape to the main deck. Emergency lighting in the engine room and accommodations is inadequate.

Electrical installations on vessels must ensure protection of crew from electrical hazards. Main switchboard has an opening where the methanol breaker was installed.

Electrical installations on vessels must ensure maintenance of services necessary for safety under normal conditions. Light indicator on fuel oil service transfer pump is inoperable.

Each propulsion control system operable from the pilot house must enable shutdown of each propulsion engine. Shutdowns currently malfunctioning.

Clean generator windings (Port)

Provide proper installation of liferafts. (painter lines not installed properly)

Replace line on lifebuoys at port wheelhouse light and handline on STBD O2 deck.

Show proper operation of watertight doors and emergency escape hatch from engine room.

Remove oily bilge water from rudder room, show proper disposal reciept or proof.

Vessel shall operate within ABS issued stability letter dated 19MAY2005.

Show proper operation indicators for bus in engine room.

Install life buoy on stern of vessel.

Replace "E" lighting on 02 deck.

Repair fuel leak on manifold.
