The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. During inspection of port engine room with engines running and underway, inspector’s 4 gas meter alerted for high levels of H2S and CO. Determine cause and affect repairs so an operational test of port engine room can be completed. 46CFR177.100

A vessel of more than 19.8 meters (65 feet) in length must carry a minimum of three life buoys. One life buoy was in deteriorated, unserviceable conditions and must be replaced. 46CFR180.70

Each vessel must meet the applicable requirements in Subchapter S of this chapter. Each hatch exposed to the weather must be watertight. FWD starboard hatch cover gasket was not watertight and must be replaced. 46 CFR 177.13-1 OLD T; 46 CFR 171.124 (a)

A portable or temporary electric cord or cable must be constructed and used in compliance with the requirements of § 111.60-13 in subchapter J of this chapter for a flexible electric cord or cable. No flexible cord may be used except:(1) As allowed under Sections 400.10 and 400.12 of NFPA 70. Uses Not Permitted (400.12) Unless specifically permitted in 400.10, flexible cables, flexible cord sets, and power supply cords shall not be used for the following: As a substitute for the fixed wiring of a structure...Where run through doorways, windows, or similar openings. Freezer on aft deck must be permanently wired or removed. 46 CFR 183.340(r) 46 CFR 111.60-13(b)(1) NFPA 70 Section 400.12

The Coast Guard will issue a medical certificate to a mariner meeting the medical and physical standards for merchant mariners. Master must provide valid medical certificate. 46CFR10.301

Each buoyant apparatus on the vessel before March 11, 1996 may be used as long as its in good and serviceable condition. It was deemed that the one of the 4 buoyant apparatus has reached the end of its serviceable life and shall be replaced with a life float meeting 46CFR160.027. 46 CFR 180.15

The vessel must have on board a number of child size life jackets equal to at least 10% (16) of the number of persons permitted. The vessel only had on board 15 of the 16 required child life jackets. Vessel operations are limited to 15 persons under 90lbs until the additional child life jacket is on board and labeled. 46 CFR 180.71

Each life float and buoyant apparatus shall be fitted with lifelines and pendants. The life lines and pendants on the buoyant apparatus' are deteriorated and dry rotting. Replace with new dark colored synthetic line. 46 CFR 180.175

In the design of a control, interlock, or indicator circuit, the disconnect device and its connections, including each terminal block for terminating the vessel's wiring, must not have any electrically unshielded or uninsulated surfaces. MI Observed an open section in the distribution panel. 46 CFR 183.330

Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. MI Observed multiple dead in wires in the engine machinery space. 46 CFR 183.200

Each lighting fixture globe, lens, or diffuser must have a guard or be made of high strength material. MI observed the globe in the emergency light in the engine room to not be a high strength material. 46 CFR 183.410

At each initial and subsequent inspection for certification, reinspection and at every other vessel inspection the marine inspector shall require that all observed unsafe practices and hazardous situations be corrected. MI observed port emergency fuel shut off valve was leaking fuel into the bilge. 46 CFR 176.25-50

A dual voltage generator installed on a vessel shall be of the grounded type, where the neutral of a dual voltage system must be solidly connected at the switchboard's neutral bus. The neutral bus shall be connected to ground. If a grounded distribution system is provided, there must be only one connection to ground, regardless of the number of power sources. This ground connection must be at the switchboard or at the common ground plate, which must be accessible. The neutral for the dual voltage generator is not connected to the ground at the switchboard. The dual voltage generator is not grounded presenting an electrocution hazard to all persons onboard. 46 CFR 183.376 and 46 CFR 183.10-30

Receptacle outlets and attachment plugs for portable lamps, tools, and similar apparatus operating at 100 volts or more, must have a grounding pole and a grounding conductor IAW 46 CFR 183.370. The generator ground is not installed in the distribution panel and the distribution panel does not have a ground bus bar or grounding conductors to each electrical outlet/device powered by AC current throughout the entire vessel which presents an electrocution hazard to all persons onboard. 46 CFR 183.10-20 and 46 CFR 183.370

The neutral of a dual voltage system must be accessible for checking the insulation resistance of the generator to ground before the generator is connected to the bus. Ground detection must be provided that: For an alternating current system, meets § 111.05-27 in subchapter J of this chapter. Ground fault detection is not installed at the switchboard. 46 CFR 183.10-30 and 46 CFR 183.324

Except as provided otherwise in this subpart, all electrical equipment including motors, generators, controllers, distribution panels, etc., shall be drip proof protected. Drip proof protected means that equipment and machinery is so constructed or protected that its successful operation is not interfered with when subjected to falling moisture or dirt, and rotating or uninsulated parts cannot be inadvertently touched or approached nearer than a safe distance by any person. Switchboards shall be fitted with drip hoods and hand rails. Non-conducting mats or grating shall be provided on deck in front and rear of switchboards. The 240V switchboard was found without an enclosure or drip shield. The equipment is open in the back, bottom, sides and top and all live conductors can be touched by the crew. 46 CFR 183.10-10 and 46 CFR 183.330

All wiring and cables shall be suitable for marine service and for the particular installation used. All wiring shall be installed as high as possible above the bilge. Welding cable found installed between battery installations in the port/stbd hull. Welding cable is connected to each battery system between the cross deck of the catamaran hull. Welding cable does not have an adequate thermal plastic to protect the conductor from heat due to resistance and presents a fire hazard. Replace with Marine Grade electrical conductors rated for the current carrying capacity of the electrical load. The starboard generator starting conductor and negative conductor were found installed in the bilge. 46 CFR 183.10-20 and 46 CFR 183.340

Except as further modified in this section, where fire extinguishing systems or equipment are not required, but are installed, the system or equipment and its installation shall meet the requirements of this Part. The vessel is outfitted with two fire pumps on each engine in each separate hull. The starboard fire pump belt drive was found cracked and damaged making the unit inoperable. The port fire pump operated as intended. 46 CFR 181.05-5

Except where all or a part of the emergency instructions are deemed unnecessary by the Officer in Charge of Marine Inspection the emergency checkoff list shall contain not less than the applicable portions of the Recommended Emergency Checkoff List §185.25-5. Fire at sea. (1) Cut off air supply to fire-close hatches, ports, doors, and ventilators, etc. The engine room fire damper closures are constructed of fiberglass for this aluminum vessel. The port damper closing hatch was found damaged and does not cover the damper. Fire damper construction should match the construction material of the vessel. 46 CFR 185.25-1

An adult life jacket must be provided for each person carried on board a vessel. 18 adult life jackets were found damaged and destroyed by the crew. Passenger count reduced to 135 plus 3 crew. 46 CFR 180.71

EPIRB installation must be a float free arrangement. The Hydrostatic release is expired 46 CFR 180.64

Portable fire extinguishers must be maintained and inspected in accordance with NFPA 10. Vessel did not have a record of monthly checks on their extinguishers. 46 CFR 176.810(b)(1)

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or many overboard conditions. Vessel did not have emergency instructions written or posted on board. 46 CFR 185.510 (a)

Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. 02 portable extinguishers, meant to protect accommodation space on main deck, were found to be stored in closet with no brackets to secure from movement. 46 CFR 181.520

The pump outlet must be fitted with a pressure gauge. Fire pump pressure gauge was stuck at 40psi. MI verified that pressure and discharge did increase despite gauge remaining at 40 psi. 46 CFR 181.300 (b)

Each buoyant apparatus on the vessel on March 11, 1996, may be used to meet the requirements of this part for these survival craft as long as the survival craft is continued in use on the vessel, and is in good and serviceable condition. 01 buoyant apparatus found to have resin failure. Passenger count reduced to 120 until chosen to repair or replace. 46 CFR 180.15 (b)

At each initial and subsequent inspection for certification, the marine inspector may require that an abandon ship or man overboard drill be held under simulated emergency conditions specified by the inspector. While conducting man overboard drill, the crew experienced difficulty communicating and identifying respective responsibilities to retrieve the man in the water. At one point, no crew member had eyes on the man in the water. The master could not see and was not receiving directions from the crew. 46 CFR 176.808 (g)

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. Kit had expired ammonia inhalants, aspirin, and eye wash. 46 CFR 184.710

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, as indicated by the vessel’s stability letter, the maximum number of passengers allowed on an upper deck must be indicated by a durable marking of at least 2 mm (1 inch) numbers and letters at the entranceway to that deck. Maximum passengers allowed on upper deck was not found at the entranceway to that deck. 46 CFR 185.602 (g)

A vessel must comply with the applicable design, equipment, personnel, procedures, and record requirements of 33 CFR parts 151, 155, and 156. Vessel printed out Garbage Management Plan while MI was on board. Plan meets requirements in accordance with 33 CFR 151.57. 46 CFR 184.702

Each door, hatch, or scuttle, used as a means of escape, must be capable of being opened by one person, from either side. Hatches identified as emergency escapes routes were over-tightened and unable to be opened by one person from inside the space. Crew members loosened handles to allow persons inside the space to be able to open. 46 CFR 177.500 (h)

Protection must be approved in compliance with 177.970 of this chapter at such locations where persons or equipment might come in contact with an exhaust pipe. MI found small portion of dry exhaust pipe exposed. Crew member repaired insulation to prevent future injuries. 46 CFR 182.430 (a)

Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018. MI found 03 adult life jackets and 02 child life jackets with deteriorated retroreflective material. Crew members replaced retroreflective material on the life jackets identified. 46 CFR 185.604

Safety Radiotelephony Certificate expired. Provide a valid Safety Radiotelephony Certificate. 46 CFR 184.502

Replace plastic piping arrangement with metallic shut off valve at hull penetration in both engine rooms. 46 CFR 182.520

Replace wasted bilge / fire cross over valve port side as original. Provide proposal for use of non-metallic piping in bilge system or replace flex hose metallic piping as original. 46 CFR 182.710

Replace cracked glass window port side main deck using safety glazing materials IAW 46 CFR 177.1020

Electrical equipment on a vessel must installed and maintained to minimize accidental personnel contact with energized parts. Replace missing breakers in pilot house panel. 46 CFR 183.200(e)

Repair pinhole prior to carrying passengers IAW 46 CFR 176.610.

Found cracked weld at fore peak tunel area, grind down and re-weld to the satisfaction of the attending marine inspector. 46CFR177.300(d)

Found 24 adult lifejackets in not serviceable condition, replace with 24 adult lifejackets in good serviceable condition to the satisfaction of attending marine inspector. Persons count reduce to 124.

Radar dome found cracked at support bracket, repair to the satisfaction of attending marine inspector.

Found nav lights not in compliance with UL1104, replace with appropriate type to the satisfaction of the attending marine inspector. Restricted to day time only.

Found starboard side engine watertight door with a broken handle, repair to the satisfaction of attending marine inspector.

Main dry exhaust systems found with broken lagging, repair or replace to satisfaction of marine inspector. 46 cfr 176.25-10

Bilge pump manifold system not functioning on stbd side at engine room and aft stern. make repairs as needed and prove proper operation.

Vessel is restricted to carry 14 children until 2 more PFDs are provided iaw 46 CFR 180.71(b).

FCC Bridge to Bridge certificate is not onboard and not valid for vessel iaw 47 CFR 80.1101.

Fire main piping has 2 pinhole leaks in each side and must be repaired or replaced prior to operation iaw 46 CFR 81.314.

Main Fire Pump not operating iaw 46 CFR 181.300.

Emergency fuel shutoff valve tested with fuel leaking at valve stem iaw 46 CFR 182.455(b)(4).

Stbd engine exhaust pipe needs to be replaced iaw 46 CFR 182.425.

Stuffing tubes on port and stbd side need to be replaced iaw 46 CFR 182.40-1.

Loose wiring needs to be addressed in port engine room iaw 46 CFR 176.806.
