A vessel of 20 meters or more in length shall be provided with a bell in addition to a whistle ... The bell or gong or both may be replaced by other equipment having the same respective sound characteristics, provided that manual sounding of the prescribed signals shall always be possible. During testing, the horn was found to be inoperable. Conduct repairs to the satisfaction of the attending Marine Inspector. 33 CFR 83.33(a)

The marine employer must ensure that all persons directly involved in a serious marine incident are chemically tested for evidence of dangerous drugs and alcohol in accordance with the requirements of subpart 4.06 of this chapter. Marine Inspector identified that the alcohol test swabs on board were expired. Replace expired test swabs. 46 CFR 16.240

At a minimum, each placard must notify the reader of the operating requirements contained in §§ 151.67 through 151.73 as they apply to that ship. The following requirements should also be prominently stated:(1) The discharge of all garbage is prohibited into the navigable waters of the United States and into all other waters except as specifically allowed;(2) The discharge of all forms of plastic into all waters is prohibited; (3) person who violates the above requirements is liable for civil and/or criminal penalties; and(4) Regional, state, and local restrictions on garbage discharges also may apply. Marine Inspector identified that the garbage placard on board contained insufficient information to meet the requirement. Replace garbage placard with one that meets the requirements of 33 CFR 151.59(c). 33 CFR 151.59(c)

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull. Marine Inspector identified a cracked weld seam near the forward portion of the collision bulkhead overhead. Conduct repairs to the satisfaction of the Attending Marine Inspector. 46 CFR 177.100

A pre-engineered fixed gas fire extinguishing system must: (1) Be approved by the Commandant;(2) Be capable of manual actuation from outside the space in addition to automatic actuation by a heat detector:...(4) Be installed in accordance with the manufacturer's instructions. System has been modified using unauthorized parts. Restore to original and have certified by NFPA certified technician.

A vessel under 46 CFR 175.110(c) requirements must conduct a training program to address firefighting proficiency,emergency egress and must include; Training in the use and location of firefighting equipment and general firefighting knowledge and egress for each member of the crew. No logs were provided showing the crew training for fire fighting and egress. 46 CFR 185.420

The Coast Guard will issue a medical certificate to a mariner meeting the medical and physical standards for merchant mariners. The medical certificate will be issued for various periods of time based upon the endorsements the mariner holds. Master of the vessel has expired medical certification. 46 CFR 10.301

Each ship transporting more than six passengers for hire equipped with a radiotelephone station in accordance with Part III of Title III of the Communications Act must carry a radio operator who meets the following requirements;the radio operator must hold a marine radio operator permit or higher class license. Master of the vessel does not have a marine radio operator permit. 47 CFR 80.159(e)

Each vessel must have permanent draft marks at each end of the vessel. Vessel is missing draft marks. 46 CFR 185.602(c)

Through hull fittings and shutoff valves must be metallic. Through hull fittings on Port/STBD side are plastic. 46 CFR 182.720(3)

A portable hand bilge pump must be: capable of pumping water, but not necessarily simultaneously, from all watertight compartments. The hand bilge pump for the forward void in front of the collision bulkhead does not provide sufficient suction to effectively de-water the space.

A hatch exposed to the weather must be watertight. The hatch to the forward void is not watertight and is permitting an ingress of water from the deck.

A hatch cover must: be attached to the hatch frame or coaming by hinges, captive chains, or other devices of substantial strength to prevent its loss. The hatch cover to the forward void is not secured to the vessel by any means.

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. The anchor light is not operational.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. The Communications Act Safety Radiotelephony Certificate is expired.

The construction and arrangment of a vessel must allow the safe operation of the vessel IAW the terms of its COI. A cracked weld was observed in the STBD engine room FWD outboard side on an overhead frame where the frame is welded to the deck. Another crack was found on the deck/frame used for support for the ladder. Deficiency must be corrected prior to next credit drydock.

For fixed-Gas fire extinguishing system, inspections and tests required by table 176.810 (b) owner or operator must provide satisfactory evidence to the [name withheld] noticed there were no service records for the Fire Boy equipment being properly tested by qualified servicing facility. Vessel is to have Fire Boy tested in accordance with Table 176.810(b).

A vessel must carry adequate and up-to-date charts of large enough scale to make safe navigation possible for the intended voyage. The chart carried on board was out dated and did not have corrections/updates. Replace with a new and updated chart.

The master and crew of a vessel shall have mounted emergency instructions containing actions to be taken in the event of fire, heavy weather, or man overboard conditions. The posted emergency instructions for man overboard do not accurately reflect the procedure followed by the crew. Update the emergency instructions using 46 CFR 185.512 as guidance to accurately reflect vessel procedures and mount at the operating station.

Submersible electric bilge pumps must meet the requirements in UL1113. Vessel's bilge pumps did not meet standard. UL 1113 approved electric bilge pumps shall be installed. 46 CFR 182.520(e)(1)

Fuel tank vent pipes shall be installed gradient upward to prevent fuel from being trapped in the line. Tank vent line was installed without upward gradient, instead utilizing a drain to prevent the entrapment of fuel oil. Drain is made of PVC in lieu of approved piping. Vessel shall install drain meeting requirements of 46 CFR 182.455 (a)(2). 46 CFR 182.450(h)

All cable and wire must be installed with metal supports spaced not more than 24 inches apart and in such a manner as to avoid chafing and other damage. 46 CFR 183.340(b)(4)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts. Current chart is from 2013. Update chart from 2013 or buy an up-to-date chart. 46 CFR 184.420(a)

At each initial and subsequent inspection for all observed unsafe practices must be corrected. Supports to 3-way valve in both engine room spaces were made of looped plastic zip ties. Provide permanent supports for 3-way valve. 46 CFR 176.830(a)

Must provide satisfactory evidence of required servicing of all three portable fire extinguishers and both pre-engineered fixed gas fire extinguishing systems to the marine inspector. Servicing is due at the beginning of March. Provide servicing receipt once complete. 46 CFR 176.810(b)

An inflatable buoyant apparatus must be serviced at a facility specifcally approved by the Commandant no later than the month and year on its servicing sticker. Although no overdue, servicing of the IBA and hydrostatic release is due at the end of this month. Provide servicing receipt once complete. 46 CFR 185.730(a)(1)

Make proper repairs to crack in stbd hull outboard at the chine in amidship void. Prove sideshell is watertight to the satisfaction of marine inspector.

Remove portable cables (extension cords) connecting coffee maker and microwave.
