At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: (1) Inspection of all accessible parts of the exterior and interior of the hull, the watertight bulkheads, and weather decks. Two windows in main deck dining room were removed due to damage. Vessel will cover openings with two sheets of 5/8" plywood. Passengers are not permitted in space until windows are replaced. Crewmembers are to conduct rounds of the space every 30 minutes. Vessel not to operate when actual weather conditions exceed three foot waves, 40kt winds from any direction, or 30kt winds from NE to SE. Vessel restricted to north of lighthouse when winds exceed 20kts from NE to SE.

A door in a stairtower, stairway, and main vertical zone bulkhead must meet the following additional requirements: (i) A door must be of the self-closing type capable of closing against a 3.5 list of the vessel. Door from the main deck to the lower galley is missing the closing mechanism.

Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck from all passenger and crew accommodation spaces located below the main deck. Emergency light near galley deep fryer is missing.

A door in a bulkhead required to be A-60, A-30, or A-15 Class must be of hollow steel or equivalent material construction, solidly filled with approved structural insulation, and capable of meeting the requirements of an A-15 Class bulkhead. Port stern second deck stairway fire door is missing a strike plate.

At each inspection for certification and at every other vessel inspection, quarters, toilet and washing spaces, galleys, serving pantries, lockers, and similar spaces may be examined to determine that they are serviceable and in a sanitary condition. The area behind the walk-in refrigerator in main deck galley contains trash and mold. Main deck female head toilet is clogged.

Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck from all passenger and crew accommodation spaces located below the main deck. Emergency lighting fixture in engine room across from switchboard is not operational.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Emergency escape hatch for engine room was found blocked by table and chairs.

Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space. Light at access to machinery space was missing.

Electrical equipment on a vessel must be installed and maintained to: (a) Provide services necessary for safety under normal and emergency conditions. Light fixtures in the lazarette and bow thruster spaces are extinguished.

A water heater must meet the requirements of Parts 53 and 63 in subchapter F of this chapter if rated at not more than 689 kPa (100 psig) and 121 °C (250 °F), except that an electric water heater is also acceptable if it: (1) Has a capacity of not more than 454 liters (120 gallons): (2) Has a heat input of not more than 58.6 kilowatts (200,000 Btu per hour); (3) Is listed by Underwriters Laboratories (UL) under UL 174, “Household Electric Storage Tank Water Heaters,” UL 1453, “Electric Booster and Commercial Storage Tank Water Heaters,” or other standard specified by the Commandant; and (4) Is protected by a pressure-temperature relief device. Vessel is installing a new hot water heater in lower galley. Ensure heater is installed as per 46 CFR 119.320 as applicable, with a stated UL rating, and a pressure-temperature relief device.

Electrical equipment on a vessel must be installed and maintained to: (c) Minimize accidental personnel contact with energized parts. Potable water pump switches in galley void have exposed holes.

Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Stairs to upper deck along egress route have wastage.

Employers must maintain records of chemical tests as provided in 49 CFR 40.333 and must make these records available to Coast Guard officials upon request. Vessel could not provide proof of enrollment in a random drug testing program.

Each individual bilge suction must be fitted with a suitable strainer having an open area of not less than three times that of the suction pipe. Replace strainer in compartment #3.

The general construction of the vessel must be such as to minimize fire hazards insofar as it is reasonable and practicable. Port aft main deck stairwell door frame has wastage.

Cable and wire to power and lighting circuits must meet NEC 310-13, UL, UL Marine cable or UL boat cable. Various section of battery cable on the STBD Genset is welding cable.

Fire Hazards and all other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. The Dry Stores overhead light diffuser is cracked.

Each vessel must have adequate E-lighting fitted along the line of the escape route to the main deck from passenger and crew accommodation spaces. E-Light near the main deck exit is OOC.

EPIRB is missing the registration decal. Obtain and install the registration decal.

Provide document from consortium with list of employees covered in random pool and records of EAP training for new employees.

Properly post fire control plan. Plan was not posted nor onboard. Plan must be Coast Guard approved.

Provide records of VGP inspections and provide records of PARI forms being completed and submitted. None could be produced at attendance.

Remove the hold back hooks and provide closure device for main deck aft port side stair tower door self closing.

Make repairs to the door handle on fire screen door 2nd deck aft and forward, guide plate was removed, and handle was gapped and miss aligned.

Outdoor aft 03 deck outlet cover was broken and exposed to weather. Replace cover.

3rd deck general service 208/120 VAC panel on bridge has an open breaker slot. provide cover to prevent shock hazard.

Underneath bridge console there are multiple surge protectors daisy chained. Permentantly mount and provide additional circuits decicated for loads.

Provide labels for breakers being used on panel on the portside in the pilothouse.

Galley general service 208/120 VAC panel needs knockout hole covered on side of panel.

Overhead Fan in galley Portside aft was full of link and missing a filter. Clean and install filter.

In Void No. 5 the power cables for the Refer/Freezer plants wire need supported 24" on center with metal supports.

Void 5 has multiple lights extinguished. Relamp all extinguished lights.

Outlet cover for 220vac outlet at the main deck service station is missing the cover. Provide correct cover.

Properly mount switch for vent fan main deck bow.

Properly mark escape hatch from Void No. 5. "Emergency Escape Keep Clear" on main deck side of hatch.

Aft emergency light in the engineroom was extinguished. Replace light.

Properly mount lighting cable between lights above the dishwasher in galley.

Properly adjust weathertight door leading to bow to ensure full closure.

Immersion suit manufacture date is 2010. Properly service suit IAW manufacture guidance and Coast Guard Policy ref. NVIC 01-08

The hatch to the #2 void requires a new gasket. The #2 void is showing signs that galley liquids are seeping into void 2 which is starting to cause corrosion. The gasket must be replaced and verified to the satisfaction of a CG Marine Inspector.

One of the starboard side aft windows on the main deck is fractured. Replace the broken glass to ensure water/weather tight integrity. To be verified to the satisfaction of a CG Marine Inspector.

Fuel valves need labeling for emergency shutoff.

Station Bill needs posting at operating station and in each crew accommodation spaces.

Post emergecny instructions at operating station.

Machinery space ventilation manual pull stations on main deck need to be labeled.

Emergency escape hatches need labeling.

Vessel must perform drills underway and operate propulsion and auxiliary equipment in presence of OMCI prior to carrying passengers.

Vessel must receive and post approved stability letter prior to carrying passengers.

EPIRB battery and hydrostatic release are expired. Replace prior to carrying passengers.

Inflatable life rafts are not stowed with automatic release ability. All three life rafts are missing hydrostatic release. Install hydrostatic releases prior to carrying passengers.

All lifejackets onboard must be marked/labeled with current vessel name prior to carrying passengers.

03 Deck: A media box is open with exposed wiring on stbd aft side. Complete installation.

02 Deck: There is an open ended cable located port fwd side. Complete installation or remove.

Main deck female head has a leaking pipe union in the overhead for grey water system. Make repairs.

Main deck fire doors are not closing with activation from the bridge. Make repairs and test.

Install/post fire control plans and current emergency evacuation plans prior to passengers.

Due to COVID-29, modifications were delayed and vessel is not in service. Vessel must undergo full annual inspection prior to carrying passengers.

Vessel undergoing modifications which effect the vessel's stability, structural fire protection, and electrical system. Carriage of passengers restricted until the Fire Control Plan, Emergency Evacuation Plan, and Electrical Plans are approved/verified and stability test is conducted. 46 CFR 115.700

Steering system must be of suitable condition. Steering system leaking hydraulic fluid from valve during operation. Make repairs to the satisfaction of the attending marine inspector. 46 CFR 115.814

Drills must test individual elements of security plan. Log book did not properly recird elements of drill,deeming drills unsatisfactory. COnduct drills to the satisfaction of attending marine inspector. 33 CFR 104.230 (a)(b)

Adjust latch on Port Side main deck FSD entrance to galley.

GFI port forward galley work area faulty, renew GFI.

E light 2nd deck inop.
