All vessels, 26ft up to 65ft and carry not more than 49 passengers, shall be provided with 01 fixed power pump and 01 portable hand pump. Fixed bilge pump was found to be inoperable. 46CFR182.25-10

A number of child size life jackets equal to at least 10% of the number of persons permitted on board must be provided. 01 child life jacket was found dry-rotted. Decrease passenger count to 23, max child passengers limited to 2. 46CFR180.71

Each buoyant apparatus must be fitted with a lifeline, pendants, a painter and a light. Floating water light was missing. Vessel restricted to daylight operations. 46CFR180.175(d)

Master that was present during this inspection had a valid MMC but did not have the Medical Certification (card)as required by 46 CFR 10.301(a) available for review. Master did not know if he had it or lost it. Provide medical certificate or attain new medical card showing proof of meeting medical and physical standers IAW 46 CFR 10. 302

Air test of the weld repair to the collision bulkhead failed. Repair two leaks found during the air test and re-test to the satiscation of the attending marine inspector. Operator proposed to grind and apply a new complete weld that secures the forward portside of the collision bulkhead to the port bow side shell. this should ensure water tightness.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. Medical kit had multiple items which had exceeded its exipry date. Replace expired items or replace medical kit.

Chemical testing of personnel must be conducted as required by this subpart and in accordance with the procedures detailed in 49 CFR part 40. Vessel operates on a L,B,S, and LC route and was found with expired Alcohol test strips. Replace alcohol test strips new.

All portable fire extiguishers installed on vessels shall be of an approved type. MI found 02 fire extinguisher brackets are not of the approved type.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date:(1) Charts of large enough scale to make safe navigation possible;(2) U.S. Coast Pilot or similar publication;(3) Coast Guard Light List;(4) Tide tables. MI noted the vessel had charts and nautical publication out of date or not on vessel.

In addition, a number of child size life jackets equal to 10% of the number of persons permitted on board must be provided. MI noted that 01 child life jacket was unservicable.

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspection. Marine inspector observed vessel generator failed to start. Vessel owner determined that generator starter was faulty. Replace starter with new. 46 CFR 183.01-15

At each initial and subsequent inspection for certification, re inspection and at every other vessel inspection the marine inspector shall require that all observed unsafe practices and hazardous situations be corrected. Marine inspector observed bilge piping system susceptible to failure and flooding. Threaded cap needs to be installed on bilge piping system. 46 CFR 176.25-50

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Vessel missing children life jacket donning procedures. Master needs to display proper child life jacket procedures. 46 CFR 185.516

In systems and applications where flexible hoses are permitted to be clamped. Double hose clamping is required where practicable. Inspector found missing hose clamps on port and startboard wet exhaust. Install required hose clamps. 46 CFR 182.410(d)

A vessel not required to have a power driven fire pump by 181.300 must have at least three 2.5 gallon buckets. Marine inspector observed only two buckets on board vessel. Master will need to acquire additional fire bucket. 46 CFR 181.610

Each major generator and motor shall be fitted with a name-plate of corrosion-resistant material. Generator on board was missing name-plate with applicable information. Install manufactures name-plate. 46 CFR 183.10-5

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. Upon examination of required EPIRB, it was found not functional. Vessel owner/operator is not allowed to operate the vessel beyond three miles from the coast line until a replacement EPIRB is properly installed. 46CFR180.64 705c.

The master of the vessel shall ensure that: Each EPIRB, other than an EPIRB in an inflatable liferaft, must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative; The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. Operator was not able to provide any required logs or documentation regarding monthly testing of the EPIRB. 46CFR185.728 50c.

Each buoyant apparatus must have Type II retroreflective material meeting subpart 164.018 of this chapter on each side and end. The material must be in strips at least 50 mm (2 in.) wide extending from top to bottom over the side or end and continuing over the top and bottom surfaces of the apparatus. For peripheral body apparatus each strip must extend completely over the top and bottom surface of the body. For box type apparatus, the strip must extend at least 300 mm (12 in.) inboard from the edge over the top and bottom surface. Each strip must be positioned near the center of the side or end, but so that it is not obscured by any strap. A typical arrangement is shown in Figure 160.010-3(p). MI observed degraded retro reflective material on both buoyant apparatus. Owner/Operator to replace as per reference with on hand stock. 46CFR160.010-4 16c.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. Upon examination of the first aid kit, it did not contain the required items as per the applicable reference. 46CFR184.710 50c.

The engine cooling water system shall be in accordance with the following provisions: A suitable hull strainer shall be installed in the circulating water intake line. Upon examination of the main propulsion engines, MI observed the main engine sea water strainers had been removed and straight piped from the hull to engine driven pump. 46CFR182.20-10 50c.

Except as allowed by paragraphs (b) and (c) of this section, before getting underway on a voyage or as soon as practicable thereafter, the master of a vessel shall ensure that suitable public announcements are made informing all passengers of the following: (1) The location of emergency exits, survival craft embarkation areas, and ring life buoys; (2) The stowage location(s) of life jackets; (3) Either: (i) The proper method of donning and adjusting life jackets of the type(s) carried on the vessel including a demonstration of the proper donning of a lifejacket, or (ii) That passengers may contact a crew member for a demonstration as appropriate, prior to beginning an oceans or coastwise voyage; (4) The location of the instruction placards for life jackets and other lifesaving devices; (5) That all passengers will be required to don life jackets when possible hazardous conditions exist, as directed by the master; and (6) If the vessel is operating with reduced manning or equipment requirements in § 176.114 of this chapter. Operator was only able to verbally recite a portion of the information required by the safety orientation. Operator is required to inform passengers of all require required information listed above. 46CFR185.506 16c.

Portable fire extinguisher must be located so that they are clearly visible and readily accessible from the space being protected. The installation and location must be to the satisfaction of the [name withheld] fire extinguishers not properly mounted on to approved mounts and found loose allowing for possible accidental movement. Use proper mounting bracket as provided by manufacture.

Life saving equipment must be clearly marked as required. Life rings and life floats found with faded vessel name and/or number of persons. Re-mark lifesaving equipment as required.

Crew training and drills shall be logged and made available for inspection. No logs made available. Conduct drills, training and log. Make avaliable.

Port side most outboard MDE exhaust pipe leaking cooling water from the pipe's non-metallic joint into bilge. Tighten loose clamp to ensure no water leaks out.

All vessels must undergo an annual inspection within the 3 months before or after each anniversary date of the vessel's inspection for certification. Vessel did not have an annual inspection conducted in CY2020. Vessel must complete an annual inspection during CY2021 prior to the carriage of passengers.

Missing all required items in first aid kit.

Post correct EAP contact phone numbers.

Have a log bok for fire and abandon ship drills and for survey/repair of lifesaving equipment.

Connect GPS to VHF-DSC radio and program in the MMSI number.

Post in bridge a compass deviation chart.

EPIRB battery was inoperable. Request owner provide new battery prior to traveling 3 miles offshore.

Replace one child lifejacket with approved type prior to carrying more than 2 child passengers. 46 CFR 180.71(b)

Tighten mountings on port side main engine prior to carrying passengers. 46 CFR 182.200

Tighten rudder post packing.

During testing of ship's horn, MIs observed a homemade pressure vessel made out of an old propane tank supplying air to ship's horn. A means to relieve pressure was not installed and the system was not a CG approved design. Ordered owner remove and disconnect tank; recommended temporarily using canned air until an appropriate sound producing arrangement was installed.

Replace rotting grablines on both lifefloats.

Replace worn retroreflective material on both lifefloats.

Replace non-servicable ring bouy and install properly arranged floating light.

Windings on generator are dirty and require cleaning.

Replace expired items in first aid kit.

Vessel needs a valid FCC radio safety certificate.
