At each initial and subsequent inspection for certification, the owner or managing operator must be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment, including the following: Inspection of piping, controls, and valves, and the inspection and testing of alarms and ventilation shutdowns, for each fixed gas fire extinguishing system and detection system to determine that the system is in operating condition. Vessel operator was unaware of functions of fixed Fire Boy system. Vessel operator was also unaware on how to operationally test system nor comfortable with the process. Conduct operational test of system in accordance with manufactures manual with certified third party and USCG to ensure full operation of system. 46 CFR 176.810 (a)(2)

Penetrations and openings in watertight bulkheads must have means to make them water tight. Port and starboard longitudinal beams running through watertight bulkheads have gaps between bulkheads and beams. Seal all identified gaps and make watertight. 46 CFR 171.114(a)(2)(OLD T)

Protective gratings shall be provided at such locations where persons gear might come in contact with the exhaust pipe. Insulation lagging on starboard main engine exhaust has worn and exposed exhaust piping is visible. Replace insulation to cover all exposed exhaust piping. 46 CFR 182.15-20(g) (OLD T)

Where required.(a) All vessels required to be provided with a power-driven fire pump shall also be provided with a fire main system Including fire main, hydrants, hose and nozzles. Starboard side fire pump could not produce adequate pressure when initially tested. Operator replaced drive belt during course of exam. MI retested fire pump and found satisfactory. 46 CFR 181.15-1 (OLD T)

Fire hose shall be 1 1/2 inches in diameter and 50 feet in length. During testing, port side hose ruptured and was cut down by three feet to stop leaking. Replace hose with lined commercial fire hose to satisfaction of MI. 46 CFR 181.15-10(c) OLD T

The bilge suctions shall be fitted with suitable strainers having an open area not less than three times the area of the bilge pipe. Multiple strainers found broken/clogged. Repair or replace to satisfaction of MI. 46 CFR 182.25-5(b) OLD T

Flexible hose shall be fabricated with an inner tube and a cover of synthetic rubber or other suitable material reinforced with wire braid. The hose cover shall be adjudged self-extinguishing per ASTM D1692, and the hose assembly be capable of withstanding free burning gasoline for 2 minutes. Fuel hoses to and from fuel filter on generator are chaffed into the wire braiding. Replace with approved hoses to satisfaction of MI. 46 CFR 182-20-40(a)(ii)OLD T

The Marine Inspector shall examine the following items to determine that they are in proper operating condition. Hydraulic hose in forepeak is corroded to wire braid. Replace to satisfaction of MI. 46 CFR 182.40-1(a)(4) OLD T

Suitable covers, guards, or rails shall be installed in way of all exposed and hazardous places such as gears, machinery, etc. Starboard engine alternator is missing protective guard. Install guard to satisfaction of MI. 46 CFR 177.35-15(a) OLD T

Suitable storm rails or hand grabs shall be installed where necessary in all passageways, at deckhouse sides, and at ladders and hatches where passengers or crew might have normal access. Forward ladder from main deck to upper deck is loose. Repair to satisfaction of MI. 46 CFR 177.35-5(a) OLD T

Except as otherwise provided in this section, rails or equivalent protection shall be installed near the periphery of all weather decks accessible to passengers and crews. Port side chain used in lieu of rail is not affixed properly. Repair to satisfaction of MI. 46 CFR 177.35-1(a) OLD T

Remote fuel shutoff stations shall be marked in at least 1-inch letters indicating purpose of the valve and direction of operation. Fuel shutoffs were not marked. Additionally, tool used to open shutoff access were not in the vicinity of the openings. Mark port and starboard shutoffs and place tool near shutoff openings to satisfaction of MI. 46 CFR 185.30-20 (a)OLD T

The maximum number of passengers allowed on the upper decks must be indicated by a durable marking of numbers and letters at least one inch in height at the entranceway to each such deck. Upper deck pax marking is not at entranceway. Relocate marking to entranceway to upper deck to satisfaction of MI. 46 CFR 185.30-3(e)OLD T

It is further intended that all machinery be installed and maintained in such a manner as to afford adequate protection against fire, explosion, machinery failure and personnel injury. Port and starboard main engine flexible hoses, including cooling, oil, and fuel hoses, show signs of degradation. Replace hoses with approved types to the satisfaction of MI. 46 CFR 182.01-5(a)OLD T

A pre-engineered fixed gas fire extinguishing system must: automatically shut down...all engines that draw intake air from within the protected space. While testing the equipment, the pre-engineered fixed fire system did not shut down the main engines or the generator in the engine room. Additionally, the pressure gauge that determines the amount of agent in the cylinder indicated that there was abnormality with the volume of the product in the cylinder. Service pre-engineered system, prove proper operation, and provide servicing report for fixed and portable fire fighting.

...[O]n a vessel of more than 19.8 meters (65 feet) in length, each hose must: Be lined commercial fire hose...When testing the fire hose, a hole was detected. Replace fire hose.

Each paddle must be...lashed to the life float to which it belongs...Paddles were not lashed. Lash paddles in the lifefloats.

The lifeline and pendants must be as furnished by the manufacturer with the approved life float or buoyant apparatus. Replacement lifelines and pendants must meet the requirements in Subpart 160.010 of this chapter. Lifelines for lifefloat were found ziptied together. Replace zip ties for hand lines with breakable ties.

Each item of survival craft equipment must...be secured by lashings, stored in lockers, compartments, brackets, or have equivalent mounting or storage arrangements that do not: Interfere with launching, recovery, or rescue operations...The water light lines for the life floats were not organized. Properly stow water light lines.

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets. Adult and child life jackets were not separated. Stow and mark child life jackets in a separate location from adult life jackets.

The regulations contained in this part are substantially in accord with, and in general are based upon established codes and recognized marine standards and practices...A list of the publications follows: Standard for Marine Type Electric Lighting Fixtures, Subject 595, published by Underwriters' Laboratories, Inc....One lighting fixture in the lazarette was missing the globe. Replace globe on the lighting fixture in lazarette. 46 CFR 183.01-15(b)(4) "OLD T"

Each ungrounded conductor of circuit supplying lights, motors or appliances shall be equipped with a circuit breaker or switch and fuses at the point of attachment to the power source. Fuse plugs were missing in the electrical panel at the bottom of the engine room ladder. Replace missing fuse plugs in electrical panel at the bottom of the engine room ladder. 46 CFR 183.05-15(c) "OLD T"

COI has expired and is no longer valid. Contact Sector Delaware Bay to schedule COI Renewal Inspection. COI Renewal inspection must be completed prior to carrying passengers.

Vessel's EPIRB was not registered. Vessel restricted to 03 NM until proof of registration received.

Provide EPIRB registration, FCC station license, radio telephony certificate, update MMSI in VHF, update emergency broadcast placard.

Replace rotting wormshoe in forward section of keel. Repair and epoxy superficial wood rot where the keel meets the hull.46 CFR 176.802.

Wire run that penetrates the bulkhead in the #3 space needs to be resealed. 46 CFR 176.806.

Remove minor delignification in engine room and re epoxy to prevent further rotting. 46 CFR 176.802.

NAVIGATION LIGHTS: Port and Starboard Navigation Lights on side of wheelhouse were missing the bulb mounting sockets causing improper focal plane for nav lights per conversation with manufacturer. Repair/Replace with appropriate navigation lights/sockets. 33 CFR 83.20

STARBOARD MDE TURBO: Slight oil leak noted at seal for turbocharger on the starboard main engine. Investigate, clean, and properly repair. 46 CFR 182.425

ELECTRICAL EXTENSION POWER-STRIP - GALLEY: Remove power-strip from the galley drain. 46 CFR "Old T" 183.01-5

FREEZER POWER CORD - GALLEY: Repair/Replace power-cord plug for the galley freezer. 46 CFR "Old T" 183.01-5

NIGHT FISHING LIGHTS, WIRING - UPPER DECK: Either remove all flood lights and associated wiring from the upper deck for historic night fishing, or repair and replace installation to comply with marine installation.46 CFR "Old T" 183.01-5

110V ELECTRICAL PANEL - WHEELHOUSE: Install blank cover in 120V electrical panel in wheelhouse to prevent/protect from accidental contact. 46 CFR "Old T" 183.10-20(f)

DRAFT MARKS: At next dry dock scheduled for March 2021, affix permanent draft marks to vessel. 46 CFR 185.602 705c - Completed at Mar 2021 DD/ISE.

LIFEFLOAT: Remove all zip ties from life float grab lines and netting. 46 CFR 180.200

POLY-LINE ON SOME LIFE FLOATS & LIFE RINGS: Replace all yellow poly-lines affixed to life floats and life rings with UV marine black poly-line. Corrected on the spot. 46 CFR 180.200

BILGE PIPING: Multiple hoses were on bilge lines in the #2 and #3 spaces. Replace all rubber hose and hose clamped pipe connection on copper bilge piping with properly installed copper couplings or unions OR Install valves at watertight bulkhead in accordance with 46 CFR "Old T" 40-1(a)(ii). 46 CFR "Old T" 182.40-1(a)

WINCH HYDRAULIC LINE FOR WINCH - ANCHOR LOCKER: Provide for chafing protection. 46 CFR "Old T" 176.25-40

GENERATOR SWITCH BOARD: Re-secure switchboard cover for generator in main engine space. Install blank in open wire hole in side of switchboard. 46 CFR "Old T" 183.10-20(f)

OUTLETS: Multiple Electric outlets inside main deck accommodation area were found to have open grounds or the hot and neutral wires were reversed. Conduct testing on all outlets and make repairs as necessary. 46 CFR "Old T" 183.10-20(o)

FIRE EXTINGUISHER - GALLEY: Perform housekeeping around galley fire extinguisher. Corrected on the spot. 46 CFR 81.500

MAIN ENG EXHAUST, TEMP PROTECT: Address the high-temp of surrounding wood in way of main engine exhaust. (The wood surrounding the exhaust from the main engines thru the overhead is being damaged from excessive heat. The wood temp was measured at 190 degrees and insulation at 300 degrees; prolonged impingement can lead to degradation of the wood and increase chance for fire. Installation of add'l insulation is required. 46 CFR "Old T" 182.20-15

Vessel required six red hand and six orange smoke distress signals; all smoke signals expired. Replaced prior to operation outside inlet. 46 CFR 180. 68 (a)

VHFDSC does not have MMSI or location. Program MMSI number and provide GPS location information to VHFDSC radio. 46 CFR 184. 502

Monthly testing of the EPIRB is not being conducted. Conduct monthly test and login. USCG witnessed test is satisfactory for December 2019 test. 46 CFR 185.728 (c)

Two hatch covers are not secured to the vessel by securing device. Secure hatch covers to vessel. 46 CFR 171.124 (b)

Bilge alarm light for #5 on panel displays light whenever another space alarms. Fix alarm panel so each alarm only gives visible indication for appropriate space. 46 CFR 182.530

MAB03: Sewage overboard discharge is not properly secured: secure by either ziptie, lock, or remove handle of discharge.

MAB04: Containers for lifejackets labeled w/ incorrect #: relabel.

MAB05: Provide updated Coast Pilot and tide tables for operating area.

Make visual and audible indicators of bilge high level in #5 & #6 compartments indicate/alarm as labeled at operating station. (#5 visual indicator indicator lights up when all other spaces' float is raised and #5 & #6 audible alarms do not work.

Install automatic shutdown for generator upon activation of pre-engineered fixed fire system in engine room.

Remove flexible hose penetrating watertight bulkheads and use proper penetrations

Make #5 compartment hatch watertight.

Crew shall conduct drills underway.
