Electrical equipment, cables and wiring shall not be installed in the hazardous locations unless it conforms with the standards not inferior to those acceptable by the organization. PSCO observed light fixture on the deck tank 1P was broken. The portside cat-work following the deck tanks had a signifcant amount of water inside the light fixture.

PSCO observed fly wheel gear cover on the MDE missing.

0220-Every candidate for certification in advance for chemical tanker cargo operations shall be required to provide evidence of having achieved the minimum knowledge, understanding, and proficiency listed in column 2 of Table A-V/1-1-3. The 2/E was not able to operate the IGG properly. This resulted in a small discharge of fuel oil into the waterway. STCW A-V/1-1.3.2.1.
