09108 - Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck from all passenger and crew accommodation spaces located below the main deck. The emergency light above the starboard engine was inoperable. 46 CFR 183.432(a)

For a life float—Approved under approval series 160.027 or other standard specified by the Commandant. Buoyant apparatus may be used to meet requirements for life floats if the buoyant apparatus was installed on board the vessel on or before March 11, 1996, and if the buoyant apparatus remains in good and serviceable condition. 46CFR180.200(a)(2)

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Repairs or alterations that affect the safety of the vessel include, but are not limited to: replacement, repair, or refastening of deck or hull planking, plating, and structural members; repair of plate or frame cracks. Damage to hull was identified on the port side stern area of the vessel. 46 CFR 176.700

11127 - The color of the life float must be primarily vivid reddish orange. The bottom life float needs to be painted. 46 CFR 180.200(a)(2) 46 CFR 160.027-2 46 CFR 160.010(n)

11127 - Each life float must have Type II retro-reflective material meeting subpart 164.018 of this chapter on each side and end. The retro-reflective tape on the top life float needs to be replaced. 46 CFR 180.200(a)(2) 46 CFR 160.027-2 46 CFR 160.010(p)

09209 - Electrical equipment on a vessel must be installed and maintained to provide services necessary for safety under normal and emergency conditions. The PA system was plugged into a power strip. 46 CFR 183.200(a)

Vital systems: Mechanically propelled "S" vessels which are ferry vessels or which carry more than 49 passengers and mechanically propelled "L" vessels shall comply with the provisions of Subchapter F of this chapter. The subject vessel is a "S" vessel (65 feet or less in length), certificated to carry 103 passengers. Therefore, piping for vital systems must meet the provisions of Subchapter F. There is evidence of a temporary repair via a JB-weld type material on the fire main system near the u-bend of the pipe in the engine room overhead. The fire main is a vital system as per 46 CFR 56.07-5 (f)(1)(ii). Provide a repair proposal to the OCMI for permanent repairs. The repair proposal should also address how the lengths of flexible hose in the fire main exceeding 30 inches in length either meet the applicable regulations in 46 CFR 56.60-25, or will also be replaced. *Due at next scheduled dry dock (31 October 2024).

The individual bilge suction lines shall be led to a central control point or manifold. Each line shall be provided with a stop valve at the control point. One of the valve handles for the bilge manifold is partially broken off. 46 CFR 182.25-5 (c) in “old T”

each life jacket must be approved in accordance with either § 160.002, § 160.005, or § 160.055 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.155 or 160.176. (04) life jackets were inoperable.

Through hull fittings and shutoff valves shall be metal. Through hull fitting / shutoff valve for bilge overboard piping has a flex hose between the shutoff valve and the hull. 46 CFR 182.40 (a)(3) in “old T”

Piping used in a vital system must: (1) Be composed of ferrous materials except when: (i) Nonmetallic piping materials are permitted by § 182.720; or (ii) Nonferrous metallic piping materials are permitted by § 182.730; and (2) If subject to a pressure of more than 1,034 kPa (150 psig), be designed, fabricated, and inspected in accordance with the principles of ANSI B 31.1. There is a leak on the connection line/hose between the bilge and fire main piping.

At each initial and subsequent inspection for certification, the owner or managing operator must be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment, including each portable fire extinguisher to check for excessive corrosion and general condition. Aft cabin fire extinguisher was found to be under-pressurized.

All fittings on fire hose shall be of brass, copper, or other suitable corrosion-resistant metal. Garden hose fittings were plastic. Old T- 46CFR181.15-10(f)

Vsl is broadcasting: No static draft information and no dimension information (AIS Antenna Position ABCD values equals 0).

Vessel reported port tail-shaft seized in place with shaft coupling spinning on locked tail-shaft. Forward water lubricated shaft bearing material broke free from its' molding and became lodged around shaft in its' housing internal of vessels shaft log, this seizing of material caused the coupling key internal of shaft coupling to sheer in half, thus allowing the coupling to free spin on tail-shaf

Conduct maintenance and regularly exercise screw down valve located on aft side of collision bulkhead.
