Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. When repairs are made during an emergency, the owner, managing operator, or master shall notify the OCMI as soon as practicable after such repairs or alternations are made. Repairs or alterations that affect the safety of the vessel include, but are not limited to: replacement, repair, or refastening of deck or hull planking, plating, and structural members; repair of plate or frame cracks; damage repair or replacement, other than replacement in kind, of electrical wiring, fuel lines, tanks, boilers and other pressure vessels, and steering, propulsion and power supply systems; alterations affecting stability; and repair or alteration of lifesaving, fire detecting, or fire extinguishing equipment. Make proper repairs and demonstrate proper operation of main engine and starter which suffered a fire while underway, to the satisfaction of attending marine inspector. 46 CFR 176.700(a)

The float-free link require by §180.130(a)(1) must be certified to meet §160.073 in subchapter Q of this chapter, or other standard specified by the Commandant. The float-free link was not certified due to missing identification tag. Replace with a Certified float-free link meeting §160.073 in subchapter Q of this chapter. Vessel route reduced to no more than 3 miles from shore. Must be resolved to the satisfaction of the attending Marine Inspector by May 21, 2025. 46 CFR 180.137(b)(1)

Each item of life saving equipment determined by the Marine Inspector to not be in a serviceable condition must be repaired or replaced. Ring life buoy plastic plug missing exposing floatation foam. Replace ring life buoy to the satisfaction of the attending Marine Inspector by May 16, 2025. 46 CFR 176.808(b)

The floating water light must be attached around the body of the life float or buoyant apparatus by a 10 mm (3/8 inch) lanyard, resistant to deterioration from ultraviolet light, and at least 5.5 meters (18 feet) in length. The light lanyard was 16 feet in length. Provide light lanyard as described by the regulation above to the satisfaction of the attending Marine Inspector. 46 CFR 180.175(f)(4)

Equipment specifications for life floats and buoyant apparatus. The equipment required for life floats and buoyant apparatus must meet the following specifications: each paddle must be of at least 1,220 millimeters (4 feet) in length, lashed to the life float to which it belongs and buoyant. The paddles were less than 4 feet in length. Replace paddles that are the correct length an buoyant to the satisfaction of the attending Marine Inspector. 46 CFR 180.175(f)(2)

Overcurrent protection must be provided for each ungrounded conductor for the purpose of opening the electric circuit if the current reaches a value that causes an excessive or dangerous temperature in the conductor or conductor insulation. Installed battery charger missing fuse(s) for each positive conductor from the charger to the battery/ battery bank. Install the required UL approved cartridge type fuse(s) as described by 46 CFR 183.380(k)(1) of the appropriate size for the model of battery charger installed as per the manufacturer's installation instructions to the satisfaction of the attending Marine Inspector. 46 CFR 183.380(k)(1)

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Battery charger case observed to be not grounded 1) Prove the batery charger case does not require being grounded in accordance with the manufacturers installation instructions; 2) Prove the battery charger case is grounded with the manufacturer's installation instructions; or 3) Ground battery charger case in accordance with the manufacturer's installation instructions to the satisfaction of the attending Marine Inspector. 46 CFR 183.200(b)

A durable placard must be posted next to all radiotelephone installations with the emergency broadcast instructions and information, specific to the individual vessel. Vessel did not have emergency broadcast instruction. Post emergency broadcast instructions, to the satisfaction of the attending Marine Inspector. 46 CFR 184.506

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Vessel did not have 48 analgesic medications (aspirin) or 10 antiseptic preparations (iodine swabs). Provide 48 aspirins, and 10 iodine swabs/wipes for first aid kit to the satisfaction of the attending Marine Inspector. 46 CFR 184.710

Fuses must be of the cartridge type only and be listed by Underwriters Laboratories or another independent laboratory recognized by the Commandant. Several fuses under the operators console are blade type fuses. Replace with fuses described by the regulation above to the satisfaction of the attending Marine Inspector. 46 CFR 183.380(1)

Each pressure type wire connector and lug must meet UL 486A or other standard specified by the Commandant. The use of twist-on type wire nuts is permitted under the following conditions:(1) The connections must be made within an enclosure and the insulated cap of the connector must be secured to prevent loosening due to virbation; and(2) Twist-on type connectors may not be used for making joints in cables, facilitating a conductor splice, or extending the length of a circuit. Several conductor connections under the operator console use twist-on connectors. Replace twist-on connector with either:(1) A pressure type connector on each conductor;(2) A solder lug on each conductor;(3) A splice made with a pressure type connector to a flexible lead or conductor; or(4) A splice that is soldered, brazed, or welded to a flexible lead or conductor to the satisfaction of the attending Marine Inspector. 46 CFR 183.340(i)

Where flexible nonmetallic hose is permitted for use in piping systems by this section, it must meet SAE J-1942 or be specifically approved by the Commandant. Provide documentary evidence that the main engine fuel filter and reduction gear Continental non-metallic hoses meet the SAE J-1942. Additionally, prove the generator Gator fuel retunr hose meets J-1942 to the satisfaction of the attending Marine Inspector. 46 CFR 182.720(e)

An EAP training program must be conducted for the emplyer's crew members and superviosry personnel. Supervisory personnel must recieve at least 60 minutes of training. Vessel Captain did not recieve supervisory training. Provide a written record of superviosry training conducted for company superviosry personnel to the satisfaction of the attending Marine Inspector. 46 CFR 16.401(b)

At each initial and subsequent inspection for certification, reinspection and at every other vessel inspection the marine inspector shall require that all observed unsafe practices and hazardous situations be corrected. Harness for rescuing a man overboard is a medium size and may not be large enough for the deckhand to donn properly. Obtain a large harness which will allow proper fit for all deckhands. 46 CFR 176.25-50

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspections. VHF antenna had a cut of the wires exposing the conductor and a crack at the top of the the antenna. Have antenna serviced to ensure these two areas of concern are corrected. 46 CFR 183.01-15(a)

Repair cracked bracket and replace missing bolt on ladder in accommodation space for credit drydock.

All machinery must be installed and maintained in such a manner as to afford adequate protection against fire, explosion, machinery failure and personnel injury. Flexible hose on port engine exhaust assembly is cracked. Replace in kind. 46 CFR 182.01-5 (a)

The discharge ends of fuel tank vent pipes must be fitted with removable flame screens or flame arresters. The flame screens must consist of a single screen of corrosion resistant wire of at least 30 x 30 mesh. Mesh on port fuel vent was not installed. Install flame screen. 46 CFR 182.450

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. Vessel life float's retro reflective tape in need of replacement. Replace retro reflective tape. 46 CFR 176.808(b)

Deck rails must be designed and constructed to withstand a point load of 200 pounds applied at any point in any direction. Fractured weld on port railing at base of stanchion. Repair damaged railing. 46 CFR 177.900 (b)

Exhaust gas must not leak from the piping or any connection. Crack discovered at elbow in generator exhaust piping above waterline in steering compartment. Monitor weekly, and repair to the satisfaction of attending marine inspector by dry dock completion date. 46 CFR 182.430 (b)

Each fixed power bilge pump must be self-priming. It may be driven off the main engine or other source of power. It must be permanently connected to the bilge manifold and may also be connected to the fire main. Vessel’s power bilge pump belt was disconnected. Reattach belt and prove to Marine Inspector that it is working properly. 46 CFR 182.520(c)

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel. Vessel's alcohol test strips are expired. Provide new test strips. 46 CFR 4.06-15

All fuel tanks must be electrically bonded to a common ground. Grounding wire for centerline fuel tank was missing. Replace common grounding wire to fuel tank. 46 CFR 182.440(b)(4)

The buoyant apparatus shall be equipped with a painter that has a breaking strength of 1500 pounds. The vessels current painter line is deteriorated, fraying, and shall be replaced. 46CFR183.15-5(c)(2)

A suitable hull strainer shall be installed in the circulating water intake line for a water cooled diesel engine. The vessel has a water cooled diesel engine, but does not have a strainer on the water intake line. A strainer shall be installed as required. 46CFR182.20-10(b)(3)

At each initial and subsequent inspection for certification the marine inspector shall require all observed unsafe practices and hazardous situations be corrected. Vessel has exposed wire conductors and connectors that are not properly secured in the space under the operating station. Also identified diodes that were partially connected to terminal bars and wires from the marine radio that were not secured and laying on hot surfaces in the same space. All unsafe wiring hazards shall be corrected. 46CFR176.25-50(a)

At each initial and subsequent inspection for certification the marine inspector shall require all observed unsafe practices and hazardous situations be corrected. Vessel has an unlabeled toggle switch on the circuit breaker panel in the engine room. Switch shall be properly labeled. 46CFR176.25-50(a)

At each initial and subsequent inspection for certification the marine inspector shall require all observed unsafe practices and hazardous situations be corrected. The vessel has a circuit breaker for the Main For FWD Panel on the circuit breaker panel within the engine room that has a max amperage of 30 amps, however the current installed fuse is rated at 40 amps. A fuse with the correct amperage shall be installed.46CFR176.25-50(a)

At each initial and subsequent inspection for certification the marine inspector shall examine all accessible parts of the exterior and interior of the hull. Marine inspectors identified a fractured longitudinal frame stiffener beneath the deck plate on the starboard side in the forward passenger compartment.46CFR176.25-5(a)(1)

At each initial and subsequent inspection for certification, reinspection and at every other vessel inspection the marine inspector shall require that all observed unsafe practices and hazardous situations be corrected. The attending marine inspector observed the vessel's generator prime mover leaking fuel oil into the bilge. Repair the generator prime mover to the satisfaction of the attending marine inspector. 46 CFR 176.25-50.

Life floats and buoyant apparatus, together with their oars and paddles, shall be conspicuously marked with the vessel's names. The attending marine inspector observed both life float paddle markings to be faded and no longer serviceable. Remark the life float paddles with the vessel's name to the satisfaction of the attending marine inspector. 46 CFR 185.30-5 (a)
