WHEN ALARM OPERATES OR WINTERGREEN SCENT IS DETECTED VACATE IMMEDIATELY.” The reference to wintergreen scent may be omitted for carbon dioxide systems not required to have odorizing units and not equipped with such units. Vessel was equipped with a fixed CO2 system stored adjacent to below decks accommodations and did not have required signage.

Each nozzle must be of corrosion-resistant material and be capable of being changed between a solid stream and a spray pattern. A nozzle on a vessel of more than 19.8 meters (65 feet) in length, must:(1) Be of a type approved in accordance with approval series 162.027; or(Be of a type recognized by the Commandant as being equivalent in performance. Nozzle onboard was not a known CG approved nozzle.

Unless other means are provided to ensure that a potential waste receptacle fire would be limited to the receptacle, waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Vessel had several combustible trash cans aboard containing dry combustibles.

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. After a marine incident, it was reported to the OCMI that the vessel sustained various structural and electrical damage. Conduct a damage assessment of the vessel to include all damaged items/areas of the vessel that occurred due to the incident, and submit a repair proposal to address the items discovered to the satisfaction of the OCMI within 7 days. - 46 CFR 185.100

Vessel reported loss of propulsion on the port and starboard engine. Provide findings of cause, make repairs, and prove proper operation of main engines to attending marine inspector prior to movement.

Vessel placed in laid up status as requested by vessel operator on 25 March 2020. Complete all applicable inspections to the satisfaction of the attending Marine Inspector prior to returning to certificated service.

Vessel reported their AIS was inoperative. Repair or replace AIS.

Vsl is broadcasting: Greater/smaller than expected vessel length.

Submit incline experiment test procedure to Marine Safety Center. Vessel is permitted to operate on the [name withheld] with a reduced sail plan in accordance with their stability letter dated 23 August 2007 until the new test results are approved.

Vessel is limited to carrying 89 passengers at the pier only. Vessel may not operate underway with passengers until satisfactory stability information is provided to the OCMI and Marine Safety Center.
