Each self-propelled vessel, except as provided in paragragh (d) of this section, must have an electric or mechanical engine order telegraph system from the navigating bridge to the engine room. Vessel's engine order telegraph from the #1 pilothouse to the engine room has an inoperable lever in the engine room that does not accurately display commands. Vessel is to repair the engine order telegraph communication from the #1 pilothouse to the engine room.

Vessel reported 1 main eng drive controller tripped offline, still within redundancy. diagnosed to a loose ribbon cable. Vessel issued 835 for 14-days. MI to clear when available.

In general, compliance with the standards established by the American Bureau of Shipping, see subpart 70.35 of this subchapter, will be considered satisfactory evidence of the structural efficiency of the vessel. Vessel reported structural damage to vessel while making hard contact with pier. Provide permanent repair proposal.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel. Vessel reported #1 ship's service diesel generator's ammeter showing significant spikes while carrying load. Vessel to troubleshoot, submit root cause analysis and proof of repair to the satisfaction of the attending marine inspector. 46 CFR 71.40-1

No repairs or alterations affecting the safety of the vessel, with regard to the machinery, shall be made without knowledge of the OCMI. Vessel reported structural damage to vessel while making hard contact with pier. Schedule MI attendance to witness repairs. 46 CFR 71.55-1

46 CFR 61.05-10(c): Boilers must be subjected to a hydrostatic test of 1.25 times the maximum allowable working pressure. #2 boiler unable to hold 1.25 x mawp for credit hydrostatic test. Boiler shall not be energized until a USCG Marine Inspector witnesses and approves credit hydrostatic testing.

The manual sprinkler system shall be so designed and arranged so that the overhead is effectively sprayed and all portions of the deck are covered. Sprinkler test failed with 15 heads over 3 zones. Vessel to stay on reduced sprinkler testing intervals. Provide advance notification to CG prior to testing for potential Marine Inspector attendance.

Doors in "A" class bulkheads shall meet the following requirements: door frames shall provide at least a 1/2 inch door stop at all sides and on top. The door leading to the compactor room ("galley stores" on FCP) is to be an A-class door per the FCP, but there is a gap on the top of the door between the door leaf and door frame. To be addressed at next credit dry dock.

Bulkheads of "A" class shall be constructed to be capable of preventing the passage of flame and smoke for one hour. The bulkhead between the 2nd Mate's office and cleaning gear locker is rated as an A-class bulkhead per the FCP, but there are two multi-cable penetrations in the bulkhead that are not properly sealed. There is also a single cable penetration above the cleaning gear locker door leading into the passenger accommodation that also needs to be sealed. To be addressed at next credit dry dock.

All marine engineering details…. Shall be designed, constructed, and installed in accordance with the provisions of subchapter F (Marine Engineering) of this chapter…..The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Marine Vessel Rules. Vessel experienced a high vibration on the #1 end. Upon investigation, it was found that the propeller was missing a blade. Vessel to submit repair proposal to local [name withheld] must be corrected to the satisfaction of the marine inspector before the vessel is considered in compliance. 46 CFR 70.20-1 46 CFR 58.05-1

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Marine Vessel Rules. Vessel experienced an automatic shutdown of the #3 main engine. Vessel to continue troubleshooting and provide root cause analysis. Vessel to repair maine engine to the satisfactory of marine inspector.

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. While conducting boat checks on the #2 Rescue Boat, the fuel system became over pressurized causing the filter assembly to rupture. Vessel intends to switch the rescue boat on the Bremerton side. Vessel authorized to operate at 50% capacity until rescue boat is replaced. Master to submit statement of satisfactory service of the replacement boat and submit to Sec Puget Sound duty marine inspector prior to operation at 100%. 46 CFR 199.190

#2 Shaft Ally Class 3 Water Tight Door (WTD) is malfunctioning (chattering when operated in the closing direction) and takes longer to close than allowed by ASTM F1197. WTD requires service. Reliability of this WTD is questionable, until door is serviced and operating properly it shall remain closed unless crew are passing through. Should this door fail the OCMI shall be notified directly.

Ring Lifebuoy adjacent the pilot house, #1 end, mounted outboard of railing starboard side, is not available for rapid deployment due to bracket damage. Repair bracket so buoy can be removed. 46 CFR 70(a)(1)(i)

Sprinkler heads shall be so arranged that no portion of the overhead is more than 7 feet from a sprinkler head. 15 sprinkler heads found inoperable or insufficient water flow.

Doors in "A" class bulkheads shall be of rigid construction, and shall provide at least a 1/2 inch door stop at the sides and top. Fire Screen Door #4 door stop found to be unbolted/missing.

All marine engineering details, such as boilers, pressure vessels, main and auxiliary machinery, piping, valves, and fittings, shall be designed, constructed, and installed in accordance with the provisions of this sub-chapter. The #4 prop generator inoperable due to ground fault in engine control system. 46CFR 50.01-10(b)

All doors, except those that are kept normally closed, shall be of a type which are capable of release from the control station and from a position at the door. The release mechanism shall be so designed that the door will automatically close in the event of disruption to the control system; however, approved power operated watertight doors will be considered acceptable for this purpose. Hold back hooks, or other means of permanently holding the door open, not subject to control station release, will not be permitted. Vessels Fire Screen Door Mechanical release system not operational. Doors were witnessed to be held open using trash cans. 46CFR72.05-25(b)(9)(ii)

Each passenger vessel of 500 tons gross tonnage and over must carry on each side of the vessel at least one rescue boat approved under approval series 160.156 that is equipped as specified in table 199.175 of this part. Vessel reported 1 of 2 rescue boat davits inoperable; Operations limited to half capacity until rectified.

A survey shall be made every time an accident or a defect is discovered which affects the safety of the vessel or the efficacy of its equipment. VSL reported extreme vibration on the #1 end during service and suspect a propeller blade sheared off. Notify CG of findings from the divers and repair plans if necessary.

Vessel experienced loss of propulsion due to failure of Vital to SSDG bus tie breaker to open and low frequency issue on the #1 SSDG. Operating generators must provide a continuous and uninterrupted source of power for the ship's service loads under normal operational conditions. Vessel is to make permanent repairs, prove proper operation of the electrical plant in all conditions, and provide root cause analysis. Additionally, WSF is to provide maintenance records for bus tie breaker, generator governor, and fuel system for CG review.

Failure of main propulsion and subsequent inability to restore plant operations was due to motor exciters on three of the six exciters to be reset. Make permanent repairs to main propulsion system and prove proper operation of main propulsion system during sea trail in all modes to the attending Marine Inspector. Additionally, WSF is to provide maintenance records for exciter cabinets to the CG for review.

A survey according to the circumstances shall be made every time an accident occurs that affects the safety of the vessel. The survey shall ensure that necessary repairs have been made and the vessel complies with all regulations under which it is inspected. Vessel is to complete underwater survey detailing any damage that occurred as a result of the grounding. This is to include renewal and proper installation of all primary lifesaving systems onboard the vessel IAW the Certificate of Inspection.

A survey according to the circumstances shall be made every time an accident occurs that affects the safety of the vessel. The survey shall ensure that necessary repairs have been made and the vessel complies with all regulations under which it is inspected. Provide samples of fuel from day tanks and contaminants for testing at acceptable laboratory and provide results to the CG.

Vital systems that are automatically controlled or remotely controlled must be provided with instrumentation to monitor parameters necassary for the safe and effective operation of the system. Vessel is to install fuel oil pressure gauges for the generator set prime movers in the EOS.

All automatically or remotely controlled or monitored vital systems must be subjected to test and inspections to evaluate the operation and reliability of control systems. Revise PSTP to include provision for testing of the ssdg to vital bus tie breaker.

All passenger vessels shall have efficient guard rails or bulwarks on decks and bridges. Vessel has section of course that is zip-tied and not securely fastened to rails in way of No. 2 end port side, top deck. 46CFR72.40-5

Electric installations on vessels must ensure protection of passengers, crew, other persons, and the vessel from electrical hazards. Water tight door #118 at frame 42 had an exposed junction box with no cover plate. 46CFR111.01-1

#2 Rescue boat davit leaking hydraulic fluid. Vessel to operate at half capacity until rectified.

Repair and prove proper operation of sliding watertight door between engine room and drive motor room. Vessel reported WTD only operable from local control and does not provide indication of position on bridge. Vessel permitted to operate with door in closed position and radio comms to alert bridge of postion change during opening for personnel transit. 46 CFR 170.270 (e) 46 CFR

The watertight door indicator repeater panel that is on the #2 end pilot house is INOP and not showing the status of the water tight doors.46CFR170.270(e)

All automation must be suitable for the marine environment and design and operated indefinitely with due consideration for static discharges and electromagnetic interferences. The use of shipboard radios near the AMS HMI resulted in the HMIs black-out.

Alarms must audibly annunciate until acknowledge. Alarms associated with the slave HMI failures do not audibly annunciate.

Submit plans detailing modifications made to the alarm and monitoring system and the revised DVTP to the CG Marine Safety Center for approval.

Repair the modified deck cable penetration on the right side of the control console.

UPS monitoring system unable to operate a maximum voltage. System approved for temporary operation with hard line bypass, must discover root cause analysis and rectify to bring UPS monitoring system back to full operation.

Drive Motor 2B had unexpected excitation, vessel must rectify issue and ensure Drive Motor 2B is fully operational.

Accumulation of oil in the 'V' of the four main engines create a fire hazard. Until the root cause of the oil accumulation can be identified and corrected, each main engine shall be cleaned oil free every night. Exhaust lagging shall be properly affixed to each engine prior to carrying passengers. [name withheld]. to inspect 'V' of all main engines and log findings prior to sailing each day until resolved.

The exhaust discharge on both Cummins generators are not lagged. Provide lagging for unjacketed exhaust piping over 150 degreees F.

Vessel is required to have test procedure documents to demonstrate proper operation of the primary and alternate controls, alarms, power sources, etc in a step-by-step or checkoff list format. PSTPs do not reflect tests as performed and have crew conducting potentially unsafe procedures (AGT-4, AGT-10, EGT-3, SST-1). Revise/submit to USCG for approval.

Boilers are required to be inspected as in 46 CFR Table 61.05-10. Boilers #1&2 have not completed due waterside & hydro inspections; boiler #1 safety valve was not tested sat.

Sufficient illumination must be provided by the emergency lighting source under emergency conditions to effect damage control procedures and to provide for safe egress from each space. Three emergency lights were out (two in main tunnel, one in starboard upper car deck).

Each firehose on each hydrant must have a combination solid stream and water spray firehose nozzle that meets the requirements in 46 CFR 162.027. The nozzle being tested during a fire drill had a loose gasket, preventing transition from solid stream to water spray.

Each fire hydrant must at the least one length of fire hose, a spanner, and a hose rack or other device for stowing the hose. Fire hose at Fire Station #7 ruptured during operational checks.

The capacity shall be such that at least 12 gallons of water per minute are applied to each 100 sqft of deck area. 61 Sprinkler heads failed in various safety zones. 2 were in a critical area near the #2 rescue boat, #1 end of vessel.

Each vessel must carry a life jacket for each person on board. Vessel had 19 lifejackets with discrepencies; 8 condemned, 9 needing and 2 needing stenciling.

For each watertight door (subdivsion) which is in a required bulkhead, an indicator light must be installed in the pilothouse and at each other vesel operaring station from which the vessel door is not visible . The indicator must show wheher the door is open or closed. Watertight door #4 failed to indicate closed when panel activated to close. Digital systems port engineer stated WTD #4 unauthorized change to engine rool panel. Verify all required watertight doors are on the panel and correct panels.

A pinhole leak was discovered on the car deck leading down into the #2 potable water void. A repair plan must be submitted to address any plate which is pitted more than a quarter of its original thickness. Any welding proposed as part of the repair plan must be conducted by a qualified welder using an approved procedure. 46 CFR 71.55-1

A vessel owner or operator must ensure security measures to control access to the vessel. Re-key or replace locks for at risk secure spaces. Increase security patrols until deficiency has been rectified.

Every crewmember must be familiar with their assigned duties. During the abandon ship drill no crew member assigned to launching the topside IBA's could demonstrate what the tether (blue line) was used for or how to position an IBA if a rescue boat was not available.

A current deviation table is required in the wheelhouse. Current deviation table is off by approximately 31 deg on #2 end and 12 deg on #1 end. 33 CFR 164.35(c)

No. 1 Boiler failed hydrostatic test. Lower water drum was found leaking during the test. Boiler not to be used until repaired to the satisfaction of the USCG. 46CFR61.05-10 - POC: Bob Pace [phone withheld]

Prove proper coliform count for potable water system. Potable water system to remain secured until flush and testing results at acceptable levels IAW 46 CFR 78.17-25 & 21 CFR 1250.84.

A 2.5-inch fire hose burst during charging during quarterly inspection fire drill. Renew fire hose.

Crew must complete manual draft readings twice daily (beginning & end of operations). Additionally, if any unusual listing occurs crew must complete manual draft readings to ensure vessel is within correct range IAW vessel stability letter. Manual readings shall continue until system is functioning as intended.

Public address system was not operable from the #1 and #2 pilot houses. Prove proper operation from operating stations IAW 46 CFR 113.50-5(a).

Fire Screen Doors #19 and 20 are designed to be normally closed and self closing. Self closing function does not latch door closed. Adjust doors to prove self closing and latching.

Davit wire damaged during routine drills. Replace davit wire rope.

Reverse power trips were tested and failed to open generator breakers for each of the three units connected to the main bus. Properly protect #1, #2, and vital generators from reverse power event. 46 CFR 111.12-11(f)

The temporary repair proposal of 18MAR2014 is approved. A permanent repair shall be conducted no later than next drydock with reports every six months, validating the condition of the temporary repair. Validations shall include NDT of the affected area. Provide next six month report on 18SEPT2014.

Permanently repair bilge suction line in the #2 end pt. wt. void to the satisfaction of the attending Marine Inspector.

Make repairs iaw approved plan on frames 82 and 83 in #1 end void and frame 82 in #2 end void at next dockside availabilty NLT 30 Dec 2010. Inspect cracks monthly starting 30 October until repair is made. Notify the CG of findings and the repair must be to the satisfaction of a CG Marine Inspector.

See activity # 3556599. Provide power monitoring equipment in EOS to indicate power failure to propulsion motor UPS's located on stbd side car deck. (46CFR58.05-1(a)).

22 lifejackets were found to be unserviceable. Replace with approved lifejackets in satisfactory material condition.

Approximately 50% of the vessel's lifejackets located on the passenger deck had inadequate retro reflective tape. Repair/replace lifejackets to meet regulations as specified in 46CFR199.70(b)(3).

04 fire dampers did not close all of the way when activated. Repair to ensure proper operation.
