Each life float must be fitted with a light. The light must be a floating water light approved under approval series 161.010. The floating water light must be attached by a 3/8inch
lanyard resistant to deterioration from UV light and at least 18 feet in length. Life float lights was burnt out and one was not connected by a lanyard. Replace light/battery in burnt out light and reconnect lanyard to other light. 46 CFR 180.175(d)

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies. Crewmember was unable to don a lifejacket himself nor could he instruct passengers how to don the lifejacket. He did not keep eyes on the item overboard, did not communicate properly and was overall uncertain of actions. Additionally, he required assistance during the fire drill in how to secure the fire dampers. 46CFR185.520
