All passenger accommodations must be arranged and equipped to provide for the safety of the passengers in consideration of the route, modes of operation, and speed of the vessel. Due to marine casualty, vessel has damage to 02 windows midships starboard side (01 missing and 01 cracked) adjacent to gangway.

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. MIs observed rotating machinery not protected by guards or rails on both the port and stbd service generators. Rectify deficiency within 7 days. 46 CFR 177.960

Fire main pipe has a hole IWO galley Ice Machine, must make proper repairs.

Vessel has missing structural fire protection insulation in engine room, engine room trunk, and behind bar at the corner. Must fix to original and approved type.

Electrical distribution panel in galley, engine room, engine room trunk, and top deck locker need to be properly labeled with name plate.

The public address system was not located throughout the vessel, zero on the second deck, and the speaker on the main deck was not working properly, must add enough speakers to cover entire vessel in all operating conditions.

Life jacket stowage on 2nd deck needs proper labeling.(number)

Both gauges for lube oil & jacket water at the helm are glazed over and/or not working, must replace or repair as needed.

Vessel needs updated and current charts of operating area. (2018)

Port side main deck 2nd window is cracked causing a haz to passengers

All scuttles on the main deck must be weathertight, have a retention line, and work properly.

All rugs and carpets shall be of wool or other material having equivalent fire resistive qualities. (Flame spread rating 75 & smoke developing rating not to exceed 100; ASTM E701-84)

Vessel needs to update the General Arraignment and structural fire protection plans to reflect current arrangement, specifically taking into consideration space categorization, fire doors, fire loads, and means of escape.

(b) An abandon ship drill must be conducted as follows: (1) Each member of the crew shall participate in at least one abandon ship drill each month. Last documented abandon ship drill was in September 2023.

(e) Each rescue boat required in accordance with § 117.210 of this subchapter must be launched with its assigned crew aboard and maneuvered in the water as if during the actual man overboard situation; (1) Once each month, if reasonable and practicable; but (2) At least once within a 3 month period before the vessel gets underway with passengers. Last documented Man Overboard drill was in September 2023.

(a) Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels... Insufficient documentation onboard for three of four crew members enrollment in a DAP screening program.

(b) Qualifications. The Vessel Security Officer (VSO) must have general knowledge, through training or equivalent job experience, in the following: (1) Those items listed in § 104.210 (b)(1) and (b)(2) of this part; (2) Vessel layout; (3) The VSP and related procedures, including scenario-based response training; (4) Crowd management and control techniques; (5) Operations of security equipment and systems; (6) Testing and calibration of security equipment and systems, and their maintenance while at sea; and (7) TWIC. The Captain, the VSO by default, was unaware of positional responsibilities & the vessel's VSP.

...all vessels shall be provided with not less than two avenues of escape from all general areas accessible to the passengers or where the crew may be quartered or normally employed... The Galley's original aft means of escape has been permanently blocked by a dance floor over the exit.

Vessels contracted for on or after July 1, 1961, which carry more than 150 passengers shall meet the requirements of subpart 72.05 of Subchapter H... Evaluated in accordance with 46 CFR 72.05 and the vessel's approved Structural Fire Protection (SFP) Plan, MI observed multiple missing fire doors at the top of stairwells or doors of an unapproved type.

Rugs and carpet may be used in addition to deck coverings. Rugs and carpets must be constructed of 100 percent wool or equivalent as determined by the regulation, MI observed deck coverings main dk & upper deck not of an approved type.

Furniture such as chairs, sofas, and similar items are tested and meet the requirements in UL 1056 “Fire Test of Upholstered Furniture,” or meet the requirements in § 72.05–55 in subchapter H of this chapter. MI doubts the fire resistance of furnishings observed on main deck and upper deck. Operator will provide proof that installed all furnishings are of an approved type and in accordance with the vessel's SFP Plan or regulation.

Ceilings, linings, and any furring incidental to their installation in control spaces, passageways, stairways, accommodation spaces and service spaces must be of noncombustible material in accordance with § 164.009 in subchapter Q of this chapter, or other standard specified by the Commandant. MI observed plastic decorative flowers. Operator will remove all decorations not of an approved type or accounted for in SFP Plan.

The general construction of the vessel shall be such as to minimize fire hazards insofar as reasonable and practicable, the vessel has some modifications Without documented MSC approval, MI notes the following modifications to the vessel not captured in the construction, general arrangement, and SFP plan drawings: 1)enclosure of upper deck with combustible materials 2)construction of security room and lifejacket locker with combustible materials

"All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspection." -Electrical outlets on main deck are not properly secured. -Electrical outlet on fwd port side, main deck has cracked face plate. -Light is missing bulb and cover in the stowage locker on main deck aft. -Junction box in the stowage locker on main deck aft is not properly secured to the wall. -Dead end wires found in the stowage locker on main deck aft. -Junction box found daisy chained to another Junction box in the stowage locker on main deck aft.

VESSEL WILL BE DETAINED UNTIL THE FOLLOWING ARE ALL COMPLETED/APPROVED BY THE USCG: 1)24/7 BILGE WATER LEVEL WATCH UNTIL APPROVED TEMPORARY REPAIRS ARE MADE AND WATER INGRESS INTO GALLEY SPACE HAS BEEN SECURED. 2)SUBMIT A REPAIR PLAN FOR TEMPORARY PATCH FOR SAFE PASSAGE TO DRYDOCK YARD. 3)SUBMIT A TOW PLAN TO DESIRED DRYDOCK YARD. 4)VISITING MARINE INSPECTORS MUST VERIFY ALL APPROPRIATE TEMPORARY MEASURES ARE PUT IN PLACE PRIOR TO INITIATING TOW PLAN TO DRYDOCK YARD. 5)ONCE AT DRYDOCK YARD, VESSEL WILL BE REQUIRED TO COMPLETE ALL WORK LIST ITEMS IDENTIFIED FROM NOVEMBER 2020 DRYDOCK EXAM (MISLE #7089456) AND ANY ADDITIONAL ITEMS FROM SUBSEQUENT JULY 2021 DAMAGE SURVEY (MISLE #7268708).
