A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. The FCC Safety Radio Certificate was expired. A new FCC Safety Radio Certificate must be obtained and placed onboard the vessel.

Vessels operated beyond the 20 nautical miles, but not more than 100 nautical miles from the nearest land, must be equipped with a MF-DSC frequency transmitter meeting the requirements of §80.1101(c)(3) and capable of transmitting J3E emission and a receiver capable of reception of J3E emission within the band 1710 to 2850 kHz, in addition to the VHF-DSC radiotelephone installation required by paragraph (a)(1) of this section. The DSC function was not operable on the MF radio. Provide MF radio installation with functioning DSC.

The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. If any of the equipment or records have not been properly maintained, a qualified servicing facility must perform the required inspections, maintenance procedures, and hydrostatic pressure tests. A tag issued by a qualified servicing organization, and attached to each extinguisher, may be accepted as evidence that the necessary maintenance procedures have been conducted. No record of monthly or annual maintenance being performed on portable fire extinguishers. Provide evidence that contains a record of all monthly and annual maintenance being performed on portable fire extinguishers.

Any vessel of at least five net tons which engages in the fisheries on the navigable waters of the United States or in the Exclusive Economic Zone, or coastwise trade, unless exempt under § 67.9(c), must have a Certificate of Documentation bearing a valid endorsement appropriate for the activity in which engaged. The vessels certificate of documentation is expired. Vessel owner must obtain a valid certificate of documentation.

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels who: (1) Occupy a position, or perform the duties and functions of a position, required by the vessel's Certificate of Inspection; (2) Perform the duties and functions of patrolmen or watchmen required by this chapter; or, (3) Are specifically assigned the duties of warning, mustering, assembling, assisting, or controlling the movement of passengers during emergencies. There were no crewmembers employed on the vessel at the time of this inspection. Any future crewmembers employed on this vessel must comply with the random testing requirements of 46 CFR 16.230.

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. There were no crewmembers employed on the vessel at the time of this inspection. Upon employment of new crewmembers, and prior to carrying passengers, a Coast Guard Marine Inspector shall witness the vessel crew perform fire, safety, and lifesaving drills.

Carbon dioxide cylinders forming part of a fixed fire extinguishing system must be maintained as follows: (1) Cylinders must be retested at least every 12 years. If a cylinder is discharged and more than 5 years have elapsed since the last test, it must be retested before recharging. The fixed CO2 fire extinguishing system storage cylinder has not undergone a hydrostatic pressure test for more than 12 years. The CO2 storage cylinder must pass a hydrostatic pressure test and be marked with the test date.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. The fiber reinforced plastic framing structure in the area of the forward compartment bilge access hatch, was wasted. Make proper repairs to restore the fiber reinforced plastic framing to original scantlings.

Each door, hatch, or scuttle, used as a means of escape, must be capable of being opened by one person, from either side, in both light and dark conditions. The method of opening a means of escape must be obvious, rapid, and of adequate strength. Handles and securing devices must be permanently installed and not capable of being easily removed. A door, hatch or scuttle must open towards the expected direction of escape from the space served. The hinges on the escape hatch leading outside from the forward berth are broken making opening of the escape hatch difficult. The hinges must be repaired so that the escape hatch can easily be opened by one person.

Electrical equipment exposed to corrosive environments must be of suitable construction and corrosion resistant. There are several corroded wiring connections in the machinery space that are not protected from the corrosive environment that the vessel operates in, including the starting motor cable connections at the starter solenoid terminals. All wiring connections in the machinery space shall be cleaned of corrosion, and properly protected from corrosion caused by the saltwater environment.

A submersible electric bilge pump discharge line must have a positive shutoff valve installed at the hull penetration. The submersible electric bilge pump discharge line for the starboard side engine room submersible electric bilge pump, did not have a positive shutoff valve installed in the discharge line at the hull penetration. A positive shutoff valve must be installed in the discharge line of the submersible electric bilge pump at the hull penetration.

Remote fuel shutoff stations must be marked in clearly legible letters at least 25 millimeters (1 inch) high indicating purpose of the valve and direction of operation. The remote fuel shutoff valves were not marked. Mark the remote fuel shutoff valves in accordance with the above paragraph.

All electrical equipment and circuits must be clearly marked and identified. The main electrical distribution panel and switch board is not marked to identify which equipment the switches and breakers control. Label all switches and breakers to identify what equipment they control.

Piping, valves, and fittings must be securely supported, and where necessary, protected against damage. The CO2 piping for the main machinery space fixed fire extinguishing system was not adequately supported. Install supports where necessary to protect the CO2 piping against movement and damage.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: (1) Charts of large enough scale to make safe navigation possible; (2) U.S. Coast Pilot or similar publication; (3) Coast Guard Light List; (4) Tide tables; and (5) Current tables, or a river current publication issued by the U.S. Army Corps of Engineers or a river authority. (b) Extracts from the publications listed above for the areas to be transited may be provided instead of the complete publication. The charts and nautical publications aboard the vessel were not up to date. Provide up to date charts and nautical publications for use aboard the vessel.

Each life jacket carried on a vessel on oceans, coastwise, or Great Lakes route, must have a life jacket light approved in accordance with § 161.012 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.112. Each life jacket light must be securely attached to the front shoulder area of the life jacket. Several of the life jacket lights were not operating. Ensure all life jacket lights operate properly.

A vessel on an oceans, coastwise, limited coastwise, or Great Lakes route must carry, Six hand red flare distress signals approved in accordance with §160.021 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.121; and, Six hand orange smoke distress signals approved in accordance with §160.037 in subchapter Q of this chapter, or other standard specified by the Commandant. 8 of the required flares were expired. Provide 8 non-expired flares in accordance with the above paragraph for use aboard the vessel.

Alcohol testing. (1) The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. The alcohol testing devices carried aboard the vessel were expired. Obtain non expired approved alcohol testing devices, and place them in service aboard the vessel.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. The first-aid kit did not contain the required contents. Provide first-aid kit that contains the required contents.

A vessel must be provided with a main steering gear that is: (1) Of adequate strength and capable of steering the vessel at all service speeds. The hydraulic steering ram mount was loose allowing the ram to move unintentionally as the rudders moved. Properly secure the hydraulic steering ram to prevent unwanted movement.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. The starboard propulsion engine was non operable. Repair the starboard propulsion engine so that it operates as originally intended for use in the vessel.

Each officer employed on any vessel subject to this subchapter must have his or her license or merchant mariner credential onboard and available for examination at all times when the vessel is operating. There was no Master employed on the vessel at the time of this inspection. A Coast Guard Marine Inspector shall ensure that an appropriately licensed Master is employed aboard the vessel prior to carrying passengers.

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. There is no interconnected fire detection system installed on the vessel. Install a properly functioning interconnected fire detection system in all required spaces aboard the vessel.

Propulsion machinery must... capable of operating at constant marine load under such requirements without exceeding its designed limitations. Port engine fuel injector line leaks during operation. Repair/replace fuel line IAW manufacturer's instruction.

Fire hazards to be minimized. Identified fuel in salon bilge. Remove fuel.

The person in command of a documented vessel must have a certificate of documentation(COD). COD expired, renew COD.

Exhaust gas must not leak from the piping or any connection. Identified water leak on port exhaust backflow pipe. Repair/replace water leak from exhaust backflow pipe.

On a vessel of at least 7.9 meters in length, a visual and audible alarm must be provided at the operating station. No audible in lazarette high level alarm. Repair/replace audible alarm.

Navigation compass must be illuminated. Compass light inop. Replace light bulb.

One-time use fire detector tripped during operational test, rendering detector incapable of further use. Audible alarm was unable to be heard from pilot house. The system must be so arranged and installed that the presence of a fire in any of the protected spaces will be automatically registered visibly and audibly in the pilothouse or fire control station. Repair fire detection system so that upon testing of operational readiness a visual and audible fire alarm is indicated in the pilot house. 46CFR76.27-80

Lazarette bilge high water alarm did not indicate any alarm when tested. On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in a space with a through-hull fitting below the deepest load waterline, such as a lazarette. Repair/replace lazarette bilge high water alarm with functioning sensor. 46CFR182.530

Missing 01 distress signal. A vessel on an oceans route must carry 06 hand red flare distress signals and 06 hand orange smoke distress signals or approved substitutions. Acquire 01 distress signal. 46CFR180.68

Master unable to provide FCC operator permit. Where the station power does not exceed 250 watts carrier power or 1500 watts peak envelope power, the radio operator must hold a marine radio operator permit or higher class license. Provide FCC operator permit. 47CFR80.159(e)
