An internal structural examination may be conducted while the vessel is afloat or out of the water and consists of a complete examination of the vessel's main strength members. V3 - steel hull showed wastage rust/ water leaking from port fwd overhead. Provide repair plan for approval by USCG Marine Inspector.

Drydock inspection shall include the underwater hull and appendages, propellers, shafting, stern bearings, rudders, through hull fittings, sea valves, and strainers, and shall be of sufficient scope to determine that these items are in a satisfactory conditions. Full 18"x18" grid UT gauging is required at 2028 dry dock exam, aft of foot mark 89, measured from the stern.

Each item of survival craft equipment must be of good quality, and efficient for the purpose it is intended to serve. MI observed deteriorated and crumbling lifelines, and pendants on the aft life float. Total persons permitted are reduced to 675. 46 CFR 117.175(a)

A marine employer whose vessel is likely to become involved in a serious marine incident, shall comply with the requirements of 4.06 in subchapter A of this chapter. MI witnessed a lack of drug and alcohol audits to ensure full compliance with parts 46 CFR Parts 4, and 16 and 33 CFR 95. 46 CFR 122.212

At each initial and subsequent inspection for certification the marine inspector shall examine the main and auxiliary steering apparatus to the extent necessary to determine that its condition is satisfactory. MI observed significant hydraulic fluid to be accumulating below the wheelhouse deck in vicinity of steering system hydraulic hoses. 46 CFR 176.25-35 (Old T)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or equivalents. Equivalent kits must be stowed in a watertight container. MI observed broken lid to the container. 46 CFR 121.710

Life floats and buoyant apparatus, together with their oars and paddles, shall be conspicuously marked with the vessel’s name. MI observed degraded and worn markings on life float oars. 46 CFR 185.30-5(a)

The master of a vessel fitted with loading doors shall assure that all loading doors are closed watertight and secured during the entire voyage. Bottom right dogging device seized and unable to actuate. 46 CFR 185.20-17(a) (Old T)

All unfired pressure vessels must be installed to the satisfaction of the cognizant OCMI. The design and testing of such unfired pressure vessels must meet the applicable requirements of subchapter F. Starboard air receiver relief pressure gauge was inoperable and bench test was unsatisfactory. 46 CFR 119.330

At each initial and subsequent inspection for certification, the MI shall view the vessel afloat...examine all accessible parts of the exterior and interior of the hull. MI observed cracked welds within; 1. the mid-forward void on the forward deck support frame just port side of the centerline. 2. a failing weld on the starboard longitudinal web framing within the sewage holding tank void 3. Flange face on the check valve servicing the gray water to overboard discharge within the sewage holding tank void is wasted and deteriorating. 4. Port side frame directly below the port side deck insert does not meet the deck and tack welds are cracked. 46 CFR 176.25-1 (Old T)

Each life float must be fitted with a lifeline, pendants, two paddles, a painter and a light. MI observed in operative light on the forward bow life float. 46 CFR 117.175(d)

The float-free link required by 117.130(a)(1) must be of proper strength for the size of the life float as indicated on its identification tag. 46 CFR 117.137(b)(2)

Each item of lifesaving equipment carried on board a vessel but not required under this part, must be of an approved type meeting the specifications for lifesaving equipment in subchapter Q of this chapter. Each buoyant apparatus must have Type II retroreflective material meeting subpart 164.018 of this chapter on each side and end. The material must be in strips at least 50 mm (2 in.) wide extending from top to bottom over the side or end and continuing over the top and bottom surfaces of the apparatus. For peripheral body apparatus, each strip must extend completely over the top and bottom surface of the body. For box type apparatus, the strip must extend at least 300 mm (12 in.) inboard from the edge over the top and bottom surface. Each strip must be positioned near the center of the side or end, but so that it is not obscured by any strap.

Each life jacket must be marked with Type I retroreflective material approved in accordance with § 164.018 in subchapter Q of this chapter, or other standard specified by the Commandant. The arrangement of the retroreflective material applied after March 11, 1996 must be as specified by IMO Resolution A.658(16), “Use and Fitting of Retro-Reflective Materials on Life-Saving Appliances.”

The bilge suctions shall be fitted with suitable strainers having an open area not less than three times the area of the bilge pipe. The bilge pipe in "Void 2" is missing the required strainer.

The general construction of the vessel shall be such as to minimize fire hazards insofar as reasonable and practicable. Wastage found in starboard side engine room entrance ladder well, aft bulkhead is wastage and holed allowing open communication from the engine room, a CO2 protected space into an accommodation space. Area of wastage is to be cropped / renewed to make vapor tight.

Each entrance to a space storing carbon dioxide cylinders, a space protected by carbon dioxide systems, or any space into which carbon dioxide might migrate must be conspicuously marked as follows: Spaces storing carbon dioxide—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. VENTILATE THE AREA BEFORE ENTERING. A HIGH CONCENTRATION CAN OCCUR IN THIS AREA AND CAN CAUSE SUFFOCATION.”.

At each initial and subsequent inspection for certification the owner or managing operator shall be prepared to test the steering systems of the vessel and make them available for inspection to the extent necessary to determine that they are in suitable condition and fit for the service intended. Port steering ram has excessive fluid loss when rudder is brought to full port rudder.

Power-operated steering gear must have arrangements for cutting off power to the gear before the rudder reaches the stops. These arrangements must be synchronized with the rudder stock or with the gear itself rather than be within the control system for the steering gear, and must work by limit switches that interrupt output of the control system or by other means acceptable to the Commanding Officer, Marine Safety Center. Port steering ram extends past the rudder stop when being brought to full port rudder. The improper rudder stop causes the hydraulic ram to leak excessively.

Pipes piercing the collision bulkhead shall be fitted with screwdown valves operable from above the bulkhead deck and the valve shall be fitted inside the forepeak tank adjacent to the collision bulkhead. Vessel has a ball valve installed in the forepeak.

At each inspection for certification and at every other vessel inspection, quarters, toilet and washing spaces, galleys, serving pantries, lockers, and similar spaces may be examined to determine that they are serviceable and in a sanitary condition. Potable water tank has a temporary repair which is leaking. 60 days to make repairs.

Weld seam on insert IWO port bow found to be undercut resulting in weld failure. Repair failed area.

The deck plating on the second deck from the stern to the forward end of the house was found to be significantly uneven, which causes significant risk for injuries and may indicate significant structural wastage of the deck. Clear the coatings on top of the affected areas of the second deck for inspection by USCG marine inspectors. 46 CFR 176.830 and 46 CFR 176.840.

Markings for the emergency fuel shutoffs were found to be illegible. Repaint the emergency fuel shut off labels so they are clear and legible. 46 CFR 185.608

Observed portside fuel vent, not fully screened, and installed properly. Vessel is to correctly install fuel vent, install a 30X30 mesh or two screens of at least 20X20 mesh with less than ½” apart. 46CFR56.50-85(a)(8)

Observed dead-end wires in the forward fuel tank space. Vessel is to place wires in a terminal box with approved connection. 46CFR120.340,

Observed multiple wasted steel locations along the chine on port and stbd side of vessel. Vessel is to provide 24 UT shots (12 on each side) with the presence of a CG Marine Inspector. 46 CFR 115.610

The setting of safety valves or relief valves on boilers and unfired pressure vessels shall be checked. MI observed inoperable relief device on pressure vessel. 46 CFR 176.25-30

A suitable metal marine type strainer shall be fitted in the fuel supply line in the engine compartment. MI observed 02 fuel strainers serving both generators within engine room not of the approved marine type. 46 CFR 182.15-40(b)(5)

As appropriate for the intended voyage all vessels must carry adequate and up-to-date Charts. Operator unable to prove that charts were up-to-date on chart plotter. MI observed backup paper charts were not up-to-date. 46 CFR 185.20-35

A vessel on a coastwise, limited coastwise route must carry six hand red flares and six orange smoke distress signals. All hand flare and smoke distress signals are expired. Replace all distress signals.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date U.S. Coast Pilot and Coast Guard Light List. Pubs were out dated. Provide updated publications.

Every power-driven vessel of 20 meters or more in length must cary a certificate with a valid endorsement while subject to the Bridge-to-Bridge Act. Vessel does not have Bridge-to-Bridge Certificate. Provide valid certificate.

Each lifefloat must be fitted with a light. Life float on bow has an inoperable water light. Provide functional water light.

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. Vessel must demonstrate that all lifejackets have been serviced to ensure waist straps operate as intended.
