A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a dry dock inspection and an internal structural at least once every two years. Vessel's drydock inspection and ISE were overdue as of March 31, 2026. Vessel is no sailed until such time as a satisfactory drydock inspection is complete.

Vessel had a through hull in the starboard, aft bunkroom that was not properly sealed. Showed vessel rep and he will correct the issue and notify local OCMI when repairs are complete.

(g) Each entrance to a space storing carbon dioxide cylinders, a space protected by carbon dioxide systems, or any space into which carbon dioxide might migrate must be conspicuously marked as follows: (1) Spaces storing carbon dioxide— ‘‘CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. VENTILATE THE AREA BEFORE ENTERING. A HIGH CONCENTRATION CAN OCCUR IN THIS AREA AND CAN CAUSE SUFFOCATION.’’. (2) Spaces protected by carbon dioxide—‘‘ CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. WHEN ALARM OPERATES OR WINTERGREEN SCENT IS DETECTED, DO NOT ENTER UNTIL VENTILATED. LOCK OUT SYSTEM WHEN SERVICING.’’ (3) Spaces into which carbon dioxide might migrate— ‘‘CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. DISCHARGE INTO NEARBY SPACE CAN COLLECT HERE. WHEN ALARM OPERATES OR WINTERGREEN SCENT IS DETECTED VACATE IMMEDIATELY.’’ MI found no signage on CO2 protected spaces. Provide proper verbiage and signage to all CO2 spaces as required. Notify local OCMI once corrected.

Vessel's 50 person inflatable buoyant apparatus (IBA) is due for servicing. Remove IBA from vessel until it has been serviced. Total person count is reduced for oceans route to no more than 50 persons. Total person count is reduced for limited coastwise route to no more than 74 persons.

Starboard most remote fuel shutoff handle is inopperable. Repair the affected handle and demonstrate a satisfactory test of the shut off in the presence of a USCG marine inspector. To be rectified prior to departure.

Vessel only had 82 adult lifejackets available for inspection. Make remain lifejackets available for inspection. Total person count of vessel is reduced to 82 persons until this deficiency is rectified.

Vessel does not have a general alarm or PA speakers in the lower berthing areas. Add PA system speakers in the lower berthing areas. To be rectified by 23 August 2024.

A vessel with installed toilet facilities must have a marine sanitation device that complies with 33 CFR part 159. Starboard toilet holding tank was leaking water from the piping connections at the tank into the bilge. Vessel is to secure water connection and is restricted from toilet use until defective piping and tank is examined and repaired to the satisfaction of the attending marine inspector.

An inflatable life raft or inflatable buoyant apparatus must be serviced at a facility specifically no later than the month and year on its servicing sticker affixed under 46 CFR 160.151-57(n). Service dates on both inflatable life rafts were expired. Vessel is redistricted to no more than 74 passengers.

All vessels shall be provided with a satisfactory arrangement for draining any watertight compartment other than a small buoyancy compartments. Bilge pump in lazarette was in-operable. 46CFR182.25-1

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire. 02 wires in the aft void were not connected to anything and just had electrical tape on the end of them and could injury crew in the space.

Main Engine serpentine belt rotating covers were not properly protected by cover, guard or rail. Must provide guard/cover over belt area to the satisfaction of the USCG Marine Inspector.

Each vessel that relies on electricity to power the loads to vessel vital systems must be arranged so that the loads can be energized from two sources of electricity. Vessel generators unable to take raw water cooling due to an inoperable pump and valve closure device. Must rectify to satisfaction of USCG Marine Inspector.

The discharge ends of fuel tank vent pipes must be fitted with removable flame screens or flame arresters. The flame screens must consist of a single screen of corrosion resistant wire of at least 30 × 30 mesh. Flamescreen on starboard fuel vent is completely wasted. Must repair to satisfaction of USCG Marine Inspector.

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. Ring buoy on upper port side of vessel must be replaced and destroyed. Ring buoy on lower deck level must have vessel name stenciled on it.

Vessel is overdue for dry dock exam and internal structural exam. Complete a credit dry dock exam and internal structural exam prior to carrying passengers.

VSL’s main f/o service tank sightglass has been removed and piping was found open to atmosphere IVO the center cutout valves. Cap piping to ensure f/o does not enter engine space.

A wooden face plate was found on a electrical switch enclosure in the engine space. Replace with an metal type enclosure suitable for marine environments.

Vessel was missing 20 adult lifejackets. Replace the missing adult lifejackets with ones approved by 46 CFR 160.002, 160.005, or 160.055. Total adult person count of the vessel is reduced to 20 until this deficiency is corrected.

FUEL TANK SIGHTGLASS WAS FOUND UNSECURED AND COULD ALLOW FUEL TO SPILL INTO MACHINERY SPACE. REPLACE WITH SIGHTGLASS THAT IS ADEQUATELY PROTECTED FROM MECHANICAL DAMAGE WITH DEVICES THAT AUTOMATICALLY CLOSE.

A BROKEN MACHINERY SPACE LIGHT WAS FOUND WHICH COULD IGNITE FLAMMABLE VAPORS. REPLACE OR REPAIR.

REPLACE LIFE JACKET LIGHTS FOUND INOPERABLE DURING INSPECTION.

Repair broken port engine room hatch and treat rot.

Remove rot and treat wood around engine room man hatch.

Remove rot and seal transverse frame aft of tool box in engine room.

Seal unused fuel fill fitting on main deck leading to engine room.

Remove rot and seal wood around bolt protruding from forward starboard section of hull.

After refloating, make vessel available and conduct in water check ride with attending USCG marine inspector.

Mark upper deck with maximum allowed passengers (119).

Repair or replace (06) wasted upper deck rail stanchions.

Vessel has been identified as having various A/C electrical modifications since initial certification. Sector San Diego does not have vessels electrical plans on file or current installations do not match what we have on file. Provide a one-line electrical drawing for all A/C switchboards and distribution panels. Include all circuit loads, conductor sizes, over current protection, and max panel ratings in amps.

While underway or drifting at sea, every vessel shall at all times maintain a proper look-out by sight and hearing. The designated watchmen required by 46 CFR 185.410 cannot preform both roving patrolling and look-out watch simultaneously. Vessel has been instructed to operate with a look-out and watchman while conducting drift operations.

Provide accommodation compartment with additional 04 emergency exits lights at all exits.

Provide all electrical panels with blank cover for missing breakers.

Remove all solid copper cabling (Romex) from forward electrical sub panel and renew with proper sized stranded boat cable.

Remove or enclose dead ended cable fed from forward electrical sub panel.

Removed permanently installed extension cord from forward electrical outlet.

Provide complete but simple instructions for for the operation of a fixed gas fire extinguishing system at each pull box and in the space where the cylinders are stored.

Properly service and prove operation on the forward watertight hatch.

Label all emergency hatches on both sides(at least 1" high "EMERGENCY EXIT, KEEP CLEAR"

Properly secure port and starboard main engine raw water strainers

Renew flexible hose for starboard main engine raw water inlet.

Remove fire hazard on grill hood exhaust. Install leak proof ducting and properly drain

Repair portside main engine jack water cooling exchanger

Remove unauthorized electric chord on water maker and install marine type and properly sized cable.

Properly secure water maker pump

Renew portside fire hose and prove proper operation. (cleared on the 07/24/2018 CJB
