Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. (04) Type 1 life jackets installed on vessel found to be deteriorated and not in serviceable condition. Renewal required for defective jackets. Vessel carriage capacity reduced to 20 passengers until replacement jackets installed on vessel. 46 CFR 176.808(b)

Each life jacket and ring buoy must be marked in clearly legible block capital letters with the vessel's name. (02) life jackets had another vessel's name. Provide proper marking and identification on all life jackets. 46 CFR 185.604

The Certificate of Inspection (COI) issued to a vessel describes the vessel, the route(s) that it may travel, the minimum manning requirements, the survival and rescue craft carried, the minimum fire extinguishing equipment and...; vessel has aboard (01) 5 B:C and is missing the (01) 10 B:C portable fire extinguisher as required by COI. 46 CFR 176.103

A vessel not required to have a power driven fire pump by 181.300 must have at least three 2 1/2 gallon buckets, with an attached lanyard satisfactory to the cognizant OCMI, and the words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. One fire bucket was broken in several places, restored with a new bucket. 46 CFR 181.610

Gauges to indicate engine revolutions per minute (RPM), jacket water discharge temperature, and lubricating oil pressure must be provided for all propulsion engines installed in the vessel. Starboard engine RPM gauge broken, repair or replace. 46 CFR 182.410 (b)

Each survival craft must be... Stowed in a way that any protective covers will not interfere with launching and embarkation. Life float was fitting with tight fitting tied cover that inhibited easy access. Remove cover or make arrangements to improve access to life float in an emergency.46 CFR 180.130 (a) (8)

A suitable metal marine type strainer... will specify if a flame shield is required. Vessel is equipped with 02 twist on fuel filters below deck (Racor B32013) equipped with plastic bowls. Replace with Racor metal bowls. 46 CFR 182.455 (b) (6)

A vessel on a limited coastwise route must carry six hand red flare distress signals and six hand orange smoke signals. Vessel was missing (03) red hand flares. 46 CFR 180.68

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. High water float did not provide audible alarm. 46 CFR 182.530

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. Alcohol swabs on board were expired. Requires replacement.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant [name withheld] or alterations that affect the safety of the vessel include, but are not limited to: replacement, repair, or refastening of deck or hull planking, plating, and structural members; repair of plate or frame cracks; damage repair or replacement, other than replacement in kind, of electrical wiring, fuel lines, tanks, boilers and other pressure vessels, and steering, propulsion and power supply systems; alterations affecting stability; and repair or alteration of lifesaving, fire detecting, orfire extinguishing equipment. In 2017 vessel changed collar structures and did not notify local USCG [name withheld] structural approvals have not be granted for this vessel. Provide collar structural plans to the Marine Safety Center for approval. 46 CFR 176.700(a)

All vessels must have operable navigation lights in accordance with the Navigation Rules. Port running light housing unit was cracked and light was inoperable. Provide navigation lights that are in compliance with the Navigation Rules. 46 CFR 183.420

The minimum number of portable fire extinguishers required on a vessel must be acceptable to the cognizant OCMI, but must be not less than the minimum number required by table 46 CFR 181.500(a) and other provisions of this section. Operating station missing 01 B-1, C-1 potable extingisher. Provide 01 B-1, C-1 fire extinguisher at operating station. 46 CFR 181.500(a)

The authority issuing the stability letter may dispense with the requirements of the simplified stability proof test when the the vessels stability can be adequately assessed by alternate means. KANALOA had a change to its hull in 2017 by switching inflatable collars. No means of adequately assessing the vessels stability could be made after this date. Vessel required to conduct stability test in accordance with MARINE SAFETY CENTER TECHNICAL NOTE (MTN) NO. 1-08, CH-2. 46 CFR 178.320

An embarkation ladder, approved IAW 160.017 in sub-chapter Q of this chapter. Port side embarkation ladder bolt holes are worn. Repair holes. 46 CFR 180.50 (b)

Construction and arrangement must allow for safe operation. Cracks located on outboard side of aft mounting plate starboard side for bimini. Make repairs to mounting plate. 46 CFR 177.100

Rescue boat must be marked with retro-reflective material. Retro-reflective materiel has faded/wasted. Provide new retro-relective materiel. 46 CFR 185.604(i)

Floating water light lanyard for ring buoy (3 to 6 ft in length) and life float (18 ft in length) must be provided. Both lanyards showing damage due to UV light. Provide new lanyards. 46 CFR 180.70

Stability letter must be on board in weather-tight container readily available for use by the crew and display to passengers and other on request. Stability letter not on vessel. Provide stability letter. 46 CFR 176.306

The master and crew will be familial with and have mounted at the operating station emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard. Emergency instructions not provided. 46 CFR 185.510

All circuits must be clearly marked and identified. Circuits at helm panel not marked or identified. Provide marking/identification. 46 CFR 183.220(e)

Blower duct must be rigid and made of same materiel as the hull or noncombustible material. Blower ducting under helm holed at connection. Provide new ducting. 46 CFR 182.460

A vessel must have operational navigation sidelights. Port navigation sidelight extinguished. Provide operational port navigation sidelight. 46 CFR 183.420

Construction and arrangement must allow for safe operation. Cracks located on outboard side of aft mounting plate of both port and starboard sides for bimini. Make repairs to mounting plate. 46 CFR 177.100

Vessel has a mechanical exhaust system powered by blower with ventilation ducts. Ventilation ducts holed and have temporary repairs. Replace ducting to make permanent repair. 46 CFR 182.460

Dead end wire leading from transom to engine. Trace and remove wire or install according to manufacturer.

Electrical equipment exposed to the weather must be watertight. A hole on helm where switch used to be under stereo allows rain and seawater to drip on to electrical equipment. Make electrical equipment watertight or provide blank to close hole. 46 CFR 183.210(b)

Vessel exhaust blower interlock not operational on vessel that has fuel tanks for gasoline. Blower switch must be of the type interlocked with the starting switch and the ignition switch so that the blowers are started before the engine starter motor circuit or the engine ignition is energized. Provide operational interlock. 46 CFR 182.460

Provide Letter of Regulatory Compliance (LORC) for Reliable Drug Testing Services Consortium.

Update EAP Information to reflect current changes to DAPI program (i.e., Provide EAP information posted in vessel common space; Identify SAP and update EAP information with contact information; Hot-line information, etc).

Provide the DAPI covered employees list for BWR vessels. Confirm all employees have record of preemployment test or subject to random since COVID-19 shutdown.

An EAP training program must be conducted for the employer's crewmembers and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug and alcohol use and abuse; and documentation of training given to crewmembers and the employer's supervisory personnel (continued on second page).Supervisory personnel must receive at least 60 minutes of training. Provide proof of completed EAP training for all personnel.

Confirm Blue Water Rafting's policy regarding employment for mariner's who have tested positive on any of the five types of DOT chemical tests and the MRO return to work process.

Gauges at helm not operating. Provide operational RPM, oil pressure, and cooling water temp gauges at helm.

Engine alarm due to O2 sensor sounding on starboard engine. Repair engine to ensure all alarms cleared and system made fully operational.

Radio Safety Certificate was expired. Provide current certificate in accordance with 46 CFR 184.502.
