A vessel on a limited coastwise route must carry six hand red flare distress signals and six hand orange smoke distress signals. Taking substitutions into account, the vessel was short 05 distress signals. Vessel limited to Lakes, bays, sounds and rivers routes.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the FCC, as set forth in 47 CFR part 80. Vessel does not have valid station license, VHF radio MMSI information is not correct. EPIRB registration requires update.

Vessel requires a visual and audible alarm at operating station to indicate high water level; high water alarms are inoperative. Rectify prior to commercial operations.

Materials used in piping systems must meet the requirements of this subpart and be otherwise acceptable to the cognizant [name withheld] engine raw water piping showed signs of corrosion at flex hose interface. Remove hose and present for inspection.

A pre-engineered fixed gas fire extinguishing system must be capable of manual actuation from outside the space. Manual pull was not connected to actuator. Rectified prior to completion of inspection.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: (1) Charts of large enough scale to make safe navigation possible. Provide proof of last update to electronic charts.

The owner, managing operator, or a qualified servicing facility as applicable must conduct the following inspections and tests: Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10. For fixed-gas fire extinguishing systems, the inspections and tests required by Table 176.810(b) of this section, in addition to the tests required by 46 CFR 147.60 and 147.65. If any of the equipment or records have not been properly maintained, a qualified servicing facility must perform the required inspections, maintenance procedures, and hydrostatic pressure tests. On UTOPIA, all portable extinguishers and fixed gas extinguishing system in E/R are past due for servicing. Provide proper documentation of servicing, and test pre-engineered suppression system engine shutdown and override function to satisfaction of attending MI.

A vessel must comply with the applicable requirements for any radio including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. On UTOPIA, FCC Safety Radio Cert expired. RESTRICTED TO 1000 ft. from shore until current cert provided.

Each vessel that operates on the high seas,must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. On UTOPIA, EPIRB Battery expired. RESTRICTED TO 3 NM from shore until deficiency rectified.

All vessels shall be provided with bilge pumps in accordance with Table 182.25-10(a): 1 fixed power pump, and 1 portable hand pump with suitable hoses for pumping bilges on the suction and discharge end. UTOPIA does not have a portable hand pump. 46 CFR 182.25-10

The owner, charterer, master or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard required by §185.510 of this part and, when applicable, the duties listed in the station bill required by §185.514 of this part. Crew training shall be logged or otherwise documented for review by the Coast Guard upon request. The training entry shall include date and general description of training topics. On UTOPIA, no crew training records were provided.

On Life floats, the floating waterlight must be attached around the body of the life float or buoyant apparatus by a 10 mm (3/8 inch) lanyard, resistant to deterioration from ultraviolet light, and at least 5.5 meters (18 feet) in length. On UTOPIA,lanyard is < 18'.

A submersible electric bilge pump may be used as a power bilge pump required by Table 182.520(a) provided that a positive shutoff valve is installed at the hull penetration. On UTOPIA,several submersible electric bilge pump discharge lines are installed with no positive shutoff valve at the hull penetration. CORRECT DURING NEXT DRY DOCK, BUT NLT 31MAY2021.

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons who have fallen overboard, or in case of a fire. These drills shall be logged/documented in accordance with this cite. On UTOPIA, no drill logs were provided, but owner conducted drills in presence of MI, and was advised to conduct and log drills as required moving forward.
