The intent of this part is to insure that the scantlings, material, fastenings, workmanship, and arrangement of each vessel are such as to provide for safe operation in accordance with the terms of its certificate inspection. Primary consideration shall be given the the provision of a seaworthy hull. Vessel sustained damage to PORT BOW in an approximate 9' x 5' area affecting hull planking, transverse and longitudinal framing, deck supports, and collision bulkhead. Submit repair proposal prior to commencing work satisfactory to attending Marine Inspector. Proposal and repair shall be approved/completed. Authorized a one-time voyage for perminant repairs at Sector CORAM. 46 CFR 177.01-5 "OLD-T"

Propulsion machinery shall be suitable in type and design for propulsion requirements of the hull in which it is installed. MI discovered STBD Main Engine throttle linkage damaged, resulting in inability to control RPM from the operating station. Replace damaged throttle linkage and conduct satisfactory sea trial to satisfaction of attending Marine Inspector. 46 CFR 182.05-1 "OLD-T"

Exhaust gas must not leak from the piping or any connections. MI identified exhaust leak on port engine. Repair prior to credit dry dock. 46 CFR 182.430(b)

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. MI identified no lifesaving donning placards posted.

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. MI identified no separation between adult and children life jackets and adult life jackets were not properly marked.

A vessel of 12 meters or more in length shall be provided with a whistle. MI identified push button connected to the horn is missing, whistle is inoperable. Repair and label within 14 days. 33 CFR 83.33(a)

Propulsion machinery shall be suitable in type and design... it is installed and capable of operating constant marine load. Observed starboard engine raw water leak on pipe to sea strainer with soft patch installed.

...the inspection shall be such as to ensure that the vessel as regards main and auxiliary machinery... is in satisfactory condition and fit for service. Observed fuel leak on port main engine's forward fuel filter housing. 46 CFR 176.05-10(a)

...the inspection shall be such as to ensure that the vessel as regards main and auxiliary machinery... is in satisfactory condition and fit for service. Observed oil leak from filter on starboard main engine's inboard, forward side. 46 CFR 176.05-10(a)

...the inspection shall be such as to ensure that the vessel as regards main and auxiliary machinery... is in satisfactory condition and fit for service. Observed fuel, oil, and jacket water leak on generator. Must clean for re-inspection by USCG inspector with generator running under load.

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system...Vessel's starboard ventilation closure does not operate as designed. Operator must prove proper operation upon re-inspection.

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspection. Observed lack of proper electrical wire isolation/removal of wires no longer in use. 46 CFR 183.01-15(a)

Vessel must carry a either a first aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. Vessel had expired items within first aid kit. 46 CFR 184.710

Gauges to indicate engine cooling water discharge temperature and lubricating oil pressure shall be provided for all propulsion engines and located so as to be in view of the operator. Vessel's engine cooling water discharge temperature and lubricating oil pressure gauges do not function properly.

Each life jacket, immersion suit, and ring life buoy must be marked and clearly legible block capital letters with the vessel's name... Vessel had several lifejackets life that faded labeling and a two vessels' names on it. Lifejackets must have only one vessel's name. 46 CFR 185.604(b)

Each floating water light must be approved in accordance with 161.010 and subchapter Q of this chapter or other standard specified by the Commandant, including, but not limited to, approval series 160.110. Vessel ring buoy water light forward of the pilothouse is no longer water tight/operating as designed. The light's covering is broken and exposed to the elements.

This part is intended to provide a minimum standard of regulations for electrical installations and equipment that will result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment, and reasonably safe from electrical shock of personnel. Observed overhead wire runs entirely by supported by plastic tie wraps a fire and personnel hazard. 46 CFR 183.01-5(a)

Complete but simple instructions for the operation of the systems must be located in a conspicuous place at or near all pull boxes, stop valve controls and in the CO2 cylinder storage room... Vessel does have CO2 discharge instructions posted and the bottle locally or at pull station. 46 CFR 181.20-15(d)

The painter must - Be stowed in such a way that it runs out freely when the life float or buoyant apparatus floats away from the sinking vessel. Vessel cannot use zip ties to keep the life float's painter lines tidy as they prevent them from floating freely. 46 CFR 180.15-5(c)(4)

In accordance with Table 182.25-10(a), "S" vessels carrying more than 49 passengers and all "S" ferry vessels of 65 feet or less must have 1 fixed power pump and 1 portable hand pump. Vessel operator could not find portable hand pump at time of inspection. 46 CFR 182.25-10(a)

Where a flexible fill pipe section is necessary, suitable flexible tubing or hose having high resistance to salt water, petroleum oils, heat and vibration, may be used... the flexible section is a nonconductor of electricity, the metallic sections of the filling pipe separated thereby shall be joined by a conductor for protection against static spark when filling. With a flexible hose mid-section of the fuel fill pipe for both starboard and port fuel tanks, observed no electrical bonding between separated metal sections of this pipe. 46 CFR 182.20-30(d)

Flexible hose runs shall be visible, easily accessible and shall not penetrate watertight bulkheads. Flexible hose run observed from starboard fuel tank penetrating aft watertight bulkhead to engine room. 46 CFR 182.20-40(a)(2)(i)

This part is intended to provide a minimum standard of regulations for electrical installations and equipment that will result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment, and reasonably safe from electrical shock of personnel. Representing a significant fire hazard, observed battery start cable chaffing against port engine fuel line on the inboard side of the port engine.46 CFR 183.01-5(a)

Portable fire extinguishers shall be installed and located to the satisfaction of the Officer in Charge, Marine Inspection. Portable fire extinguisher not properly secured in correct type of bracket in engine room. 46 CFR 181.30-10(b)

This part is intended to provide a minimum standard of regulations for electrical installations and equipment that will result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment, and reasonably safe from electrical shock of personnel. Observed improperly secured and exposed wires from bilge high level alarms to their respective junction boxes in the forepeak and in the engine room. 46 CFR 183.01-5(a)

This part is intended to provide a minimum standard of regulations for electrical installations and equipment that will result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment, and reasonably safe from electrical shock of personnel. In cabinet containing CO2 pull remote pull station on main deck, several cable's sheathing is loose resulting exposed wires representing a fire hazard. 46 CFR 183.01-5(a)

The marine employer shall ensure that all individuals engaged or employed on board a vessel are fully indoctrinated in the requirements of this subpart, and that appropriate vessel personnel are trained as necessary in the practical applications of these requirements. Vessel operator did not show evidence of entire crew's enrollment in a drug and alcohol program nor evidence of training to the crew on the program. 46 CFR 4.06-1(e)

An aggregate of more than 7.6 liters (two gallons) of flammable or combustible liquids stowed outside of an accommodation, control, or service space (other than a paint locker) or outside of a machinery space must be stowed in a paint locker that is marked with a warning sign indicating flammable or combustible liquid storage. Found an oxyacetylene bottle in a service locker and multiple pails of paint stored about the vessel all of which representing a fire hazard. 46 CFR 147.45

A vessel of not more than 19.8 meters (65 feet) in length is not required to carry a rescue boat unless: (1) The vessel carries passengers on an open or partially enclosed deck; and (2) The cognizant OCMI determines that the vessel is designed, arranged, or involved in operations so that the vessel itself cannot serve as an adequate rescue craft. Although authorized, the rescue platform is not properly secured to the vessel when deployed during a man overboard (MOB). Acting as a rescue boat in a man overboard, vessel will determine a way to more securely fasten the rescue platform to mitigate hazard to crew and increase effectiveness during a MOB. 46 CFR 180.210(b)

The intent of this part is to insure ... that the auxiliary machinery is suitable for the purpose intended. It is further intended that all machinery be installed and maintained in such a manner as to afford adequate protection against fire, explosion, machinery failure and personnel injury. Excessive amount of oil present on generator windings and pooled in winding casing representing a fire hazard. 46 CFR 182.01-5(a)

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Vessel is not logging the drills above.

A vessel at anchor shall exhibit where it can best be seen: (i) In the fore part, an all-round white light or one ball; (ii) At or near the stern and at a lower level than the light prescribed in paragraph (i) of this Rule, an all-round white light. Vessel's anchor light was found inoperable.

Fire pump pressure gauge not operational.

STBD shaft seal leaking excessively.

46CFR182.115 (d): On or before March 11, 1999, an existing vessel must comply with the bilge high level alarm requirements in § 182.530. 46CFR182.530(b): Vessels constructed of wood must, in addition to paragraph (a), provide bilge level alarms in all watertight compartments except small buoyancy chambers. Forepeak and rod locker required high level alarm.

Rails or equivalent protection shall be installed near the periphery of all weather decks accessible to passengers and crews. Rails shall consist of evenly spaced courses and the spacing between courses shall be not greater than 12 inches. On passenger decks of vessels engaged in a ferry or excursion type operation, rails shall be at least 42 inches high. The space below the rail shall be fitted with bulwarks, chain link fencing, wire mesh, or the equivalent. The vessel has incomplete rails installed on the main (entire deck)and second decks(aft starboard quarter). Additionally, the vessel engages in excursion operations. On the main deck the space below the top rail is not equipped with bulwarks, chain link fencing, wire mesh, or the equivalent on the second deck the netting serving as an equivalent to bulwarks/chain link on the rails is torn and missing. 46 CFR 177.35-1

It shall be the duty of the master of any vessel to assure that all exposed hatches are properly secured before getting underway for a voyage on other than protected waters. The hatch to the entrance of the vessel's second compartment is cracked and is being moved by the operator to make the entrance to the #2 compartment from the interior of the vessel however this repair is not complete. 46 CFR 185.20-15

Each life buoy must be of an approved type, constructed in accordance with subpart 160,050 if this chapter. Life buoy on vessel was deteriorated and cracked. Replace life buoy. 46 CFR 180.30-1

All pyrotechnics distress signals shall be stowed in a portable water-tight container. Vessel pyrotechnics were not in watertight container. Provide watertight container for flares and label. 46 CFR 180.35-10

Vessel must carry either first aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions and stowed in watertight container. Vessel had incorrect first aid kit. 46 CFR 184.710

Repairs or alterations to the hull or safety equipment of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Prior to carrying passengers submit a repair proposal for the following to the satisfaction of the attending marine inspector: make available for inspection newly installed hand rails on the vessel's bow.

Post emergency instructions on bridge.

Generate DAPI folder with company policy.

Vacuum out all PFD storage boxes.

Replace ROMEX wiring on top deck lighting with boat cable.

LED lighting above stbd entryway to main cabin - place wire connection in enclosure.

Life jacket signs shall be of contrasting colors.

Provide portable bilge pump.

Update emergency broadcast placard.

Label bilge system with water resistant placards. Permanently mount the placards.

Repair stbd engine RPM gauge in pilothouse.

Replace pool pump with approved marine grade pump.

Replace PVC piping connecting non-PTO pump to bilge system with metallic piping.

Provide lock and chain for sewage overboard valve.

Provide marpol placard at overboard valve in engine room.

The port side indepented diesel fuel tank is leaking. Operator needs to submit repair proposal and complete repairs to tank prior to carrying passengers.

1) Make first aid kit readily accessible and contents easily accessible.

2) Replace 02 children's life jackets. Children's count limited to 07 until replaced.

3) Properly repair opening in deck from fore peak space so as to make water tight.

4)Properly clean all battery terminals and clamps.

5) Properly repair opening in forward engine room bulkhead so as to make water tight.

6) Make non-metallic fuel lines no more than 30 inches in length and marine grade.

7) Properly install double hose clamps on fresh water feed for port main engine.

8) Properly install guards on port main engine fly wheel and both shaft couplings for rotating machinery in engine room.
