IMO detentions dated 13 Feb 2025 and 24 July 2025 identified deficiencies, and follow-up ISM audits identified nonconformities, that continue to persist. Further examination today revealed additional objective evidence of an inability to implement an effective Safety Management System.

Every oil fuel pipe, which, if damaged, would allow oil to escape from a storage, settling or daily service tank situated above the double bottom, shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of a fire occurring in the space in which such tanks are situated. PSCO observed eight of the ten hydraulic reservoirs for activating the vessel's quick closing valves did not have the required amount of hydraulic fluid. Upon testing points 1,2, & 9 the crew was unable to activate the quick closing valves in a timely manner. A review of the vessel's maintenance history shows the maintenance for the system was listed as "good" on work done date, 21 June 2025. Vessel's crew refilled reservoir which drained themselves again within two hours.

Four sets of full protective clothing resistant to chemical attack shall be provided in addition to the fireman's outfits required by regulation 10.10 and shall be selected taking into account the hazards associated with the chemicals being transported and the standards developed by the organization according to the class and physical state. The protective clothing shall cover all skin, so that no part of the body is unprotected. PSCO noted that all four chemical protective clothing suits had severely dry-rotted rubber seals on either the neck or wrist, making the suits ineffective in a chemical emergency. A review of the vessel's monthly checks revealed that the crew inspected the suits, but these checks did not report the observed condition.
