Additionally, reaffix the label for the “vent fans fwd” button, which has fallen off.

Where air pipes to ballast tanks extend above the freeboard or superstructure deck, the exposed parts of the pipes shall be of substantial construction. -The forepeak ballast tank air vent line is holed where it passes through the rope locker. Crop and renew to the satisfaction of attending ABS surveyor. -SWB no. 8 gooseneck vent (located on aft mooring deck) is holed near the top of the u-bend. Crop and renew to the satisfaction of attending ABS surveyor. Update 3/15/24: at the request of the vessel operator, the due date has been extended 60 days as outside contractor support is obtained to make the necessary permanent repairs.

After any survey of the ship has been completed, no change shall be made in the structural arrangements and other items covered by the survey, without the sanction of the Administration. The cargo hold watertight door at C deck, frame 198, port side, has an approximately 6-8 foot section of taped on dissimilar material on top of the normal gasket at the hinged side of the door. Provide documentation this arrangement is in accordance with manufacturer's specifications or remove. Subsequently prove watertightness of door to the satisfaction of the attending ABS surveyor.

An efficient bilge pumping system shall be provided, capable of pumping from and draining any watertight compartment. 3 bilge line valves in the lower anchor windless room were found seized/frozen. Free up and prove proper operation.

The Company should identify equipment and technical systems the sudden operational failure of which may result in hazardous situations. The safety management system should provide for specific measures aimed at promoting the reliability of such equipment or systems. The current inventory for various critical spares is below the minimum required ROB established by the ISM Company. It appears the updates to the critical spare parts listed enacted on the sister ship Antares following the CG finding in report #7769465 have not been applied to the Denebola. Apply these changes and acquire any missing critical spare parts to the satisfaction of ABS and CG. Update 3/15/24: the deficiency due date has been extended to 6/13/24. The critical spare parts list has been updated and POs issued for the missing/needed critical spares.

Precautions shall be taken to prevent any oil that may escape under pressure from any pump, filter, or heater coming into contact with heated surfaces. The EDG is missing lagging on the turbocharger and exhaust side of the turbocharger.

The machinery, associated piping systems and fittings shall be adequate for the service intended and installed and protected so as to reduce to a minimum any danger to persons on board. The SSDG has an exhaust leak and missing bolts on the exhaust flange.

A master key control system shall include a list of all crew, identified by position, allowed to access and use the master key. The list shall be included in the vessel’s safety management system. Master key control logs shall be utilized for all master keys. The master key control procedures in section 4.5.4 of the SQEM do not meet all applicable statutory requirements. Additionally, the master key control log has not been maintained up-to-date. See CG-CVC Policy Letter 23-06 for additional guidance.

Two means of escape shall be provided from each category A machinery space consisting of two sets of steel ladders. There is broken deck grating on the starboard ladder at the bottom level of the engine room near the condensers.

The machinery, associated piping systems and fittings shall be adequate for the service intended. The piping on the discharge side of the #2 freshwater service pump has a leak between the cutoff valve and the check valve.

All waste receptables shall be constructed of non-combustible materials with no openings in the sides or bottom. Plastic trash cans found in use in various locations throughout the vessel, with various combustible trash inside (plastic waste receptacles are only permitted if they are used solely for wet waste, glass bottles, and metal cans and are suitably marked). Replace all plastic trash cans not for above-mentioned specified use prior to COI-renewal / renewal of applicable SOLAS certificates (30 November 2025). Code 705 - by next COI-renewal (30Nov2025)

Fire Control Plans shall be kept up to date. The current Fire Control Plan, Revision E, does not show the new fire detection system that was installed in the vessel within the past several years. The fire control plan is to be updated and onboard verification of its accuracy is to be completed by ABS prior to COI-renewal / renewal of relevant SOLAS certificates (30 November 2025). Code 705 - by next COI-renewal (30Nov2025)

The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: structural fire protection, including fire-resisting divisions, and protection of openings and penetrations in these divisions. Cable penetrations found with improper or missing sealing arrangements in the following locations: -Navigation bridge in main cabinets where penetrating deck underneath. -Electrical locker in aft house on several levels -Main deck in the forward house, on the forward. bulkhead, including in thin forward house main deck, from passageway to pump room and in vicinity, found not properly sealed.

The lighting required by sub-paragraphs (a)(ii), (b)(ii), and (b)(iii) of regulation 19 of this chapter (e.g. lighting at lifeboat launching and embarkation stations) shall be capable of being supplied by the emergency source of power. Some of the marked emergency lighting at the port and starboard lifeboat embarkation and launching stations found not illuminating. Additional lighting found not marked and not illuminating.

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. The lifebuoy light for the ring life buoy on the starboard quarter, forward of fire station 46, was found with the battery expired and of the incorrect type. Ships shall be provided with a main steering gear to the satisfaction of the Administration. When operating on emergency steering from the steering flat, there is an intermittent “steering failure” alarm that activates on the console at the main steering station. This seems to routinely activate when passing around 15-20 degrees to port or starboard.

All ships, irrespective of size, shall have a properly adjusted standard magnetic compass, independent of any power supply, to determine the ship’s heading and display the reading at the main steering position. At time of attendance the gyrocompass read 022° and the magnetic compass 049°. The local variation in Baltimore harbor per the provided nautical chart is currently 11°W. This would lead to a corresponding deviation of ~16°, which is substantially different than the listed deviation on the compass card of <1°. Update 3/29/23: at the request of the operator (communicated by ABS), this deficiency has been extended until 7/2/23, to support re-evaluation of the deviation at the soonest available opportunity (vessel activation or adjacent vessel movement).

Update 3/29/23: at the request of the operator (communicated by ABS), this deficiency has been extended until 6/17/23. It is reported that a PO has been issued for testing to be carried out.
