Under all sailing conditions, including maneuvering, the speed, direction of thrust and, if applicable, the pitch of the propeller shall be fully controllable from the navigation bridge. Sector Miami was notified that the vessel was unable to direct thrust from the navigation bridge and suffered a loss of propulsion.

whenever an accident occurs to a ship which affects the safety of the ship, the master/owner shall report to admin or RO who shall cause investigation. The main engine suffered crankcase overpressurization causing crankcase safety covers to release. Chief Engineer shut down maine engine, assist tugs returned vessel to dock.
