Each life jacket carried on a vessel on oceans, coastwise, or Great Lakes route, must have a life jacket light approved in accordance with § 161.012 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.112. 16 of the water activated lights attached to the adult jackets were inoperable. Total persons limited to 134.

CO2 cylinders must be retested at least every 12 years. If a cylinder is discharged and more than 5 years have elapsed since the last test, it must be retested before recharging. Cylinders were not marked with last hydro- static test. Provide tech report showing last test.

Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space. Localized pitting observed in engine room bottom plating under stuffing boxes with deepest pit gauged at 1/4". Submit repair proposal to effect temporary repair to the satisfaction of marine inspector.

A vessel must carry one floating waterlight, unless it is limited to daytime operation, in which case no floating waterlight is required.(1) Each floating waterlight must be approved in accordance with § 161.010 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.110.(2) Each ring life buoy with a floating waterlight must have a lanyard of at least 910 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the waterlight around the body of the ring life buoy. Line attaching waterlight to the ring buoy was deteriorated, and battery used to operate the floating waterlight was expired.

Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space. Vessel did not have a portable battery operated light at the operating station and at the access to the propulsion machinery space.

A marine employer whose vessel is involved in a casualty or incident that is, or is likely to become, a serious marine incident as defined in § 4.03-2 of subchapter A of this chapter shall comply with the requirements of § 4.06 in subchapter A of this chapter. Alcohol test strips on board were expired. Provide receipt of purchase or picture of new testing strips.

The master of the vessel shall ensure that:(a) Each EPIRB, other than an EPIRB in an inflatable liferaft, must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative. EPIRB was not being tested and logged.

Each space accessible to passengers or used by the crew on a regular basis must have at least two means of escape, one of which must not be a watertight door. The two required means of escape must be widely separated and, if possible, at opposite sides or ends of the space to minimize the possibility of one incident blocking both escapes. The vessel's passenger accommodation space is currently configured such that both the main ladder well and the emergency egress hatch lead to the same space on the deck above, and therefore does not meet the requirement that the two points of egress cannot be compromised by one incident (i.e., a fire in the space above blocking both exits). Vessel is restricted from operating with overnight accommodations available to passengers until satisfactory modifications are complete. 46 CFR 177.500(b)

Waste receptacles must be constructed of non-combustible materials with no openings in the sides or bottom. The waste receptacle in the vessel's main passenger cabin was found to be made of combustible material. 46 CFR 177.405(f)

A marine employer whose vessel is involved in a casualty or incident that is, or is likely to become, a serious marine incident as defined in 46 CFR 4.03-2 shall comply with the requirements of 46 CFR 4.06. At the time of inspection, the means for conducting alcohol testing in compliance with the regulation above were not onboard the vessel.

A vessel of at least 7.9 meters (26 feet) in length must be provided with individual bilge lines and bilge suctions for each watertight compartment. Not able to identify bilge lines, label all lines. 46 CFR 182.510

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts of large enough scale to make safe navigation possible. Electronic charts are not up-to-date. 46 CFR 184.420(a)(1)

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment, under all practicable conditions. Pin hole found in bilge line at forepeak. Crop/ren in kind, test to satisfaction of OCMI. 46 CRF 182.510

A weather-tight door must be provided for each opening located in a deck house or companionway. Most weather-tight doors are missing gaskets. 46 CFR 179.360

A hatch exposed to the weather must be watertight. Forepeak hatch missing gasket. 46 CFR 179.360

Each vessel certificated to operate on an oceans route in cold water must: Be provided with inflatable buoyant apparatus of an aggregate capacity that will accommodate 100% of the total number of persons permitted on board. IBA for 100 provieded. Ltd to 100 total persons. 46 CFR 180.202

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels who occupy a position, or perform the duties and functions of a position, required by the vessel's Certificate of Inspection. No evidence provided to verify that deckhands are enrolled. 46 CFR 16.230

Each device must have a sewage placard suitable for posting on which is printed the operating instructions, safety precautions, and warnings pertinent to the device. The size of the letters printed on the placard must be one-eighth of an inch or larger. Placard missing on sewage tank. 33CFR159.59

Must have a pollution placard of at least 5 by 8 inches, made of durable material fixed in a conspicuous place in each machinery space, or at the bilge and ballast pump control station. Placard missing in engine room. 33CFR155.450(a)

Operator must secure each Type III device in a manner which prevents discharge of sewage. Gravity discharge line valve not locked. 33CFR159.7(c)

Each life jacket carried on a vessel on oceans route must have a life jacket light. Each life jacket light must be securely attached to the front shoulder of the life jacket. Only 50 adult jackets have lights. Ltd to 50 adult passengers only. Provide lights for 150 adult and 15 child life jackets. 46CFR117.75

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons who have fallen overboard. Demonstrate ability to recover helpless person from the water. 46CFR185 .520(a)

THE VESSEL'S OWNER/OPERATOR HAS NOT REQUESTED AN INITIAL OR UPDATED STABILITY LETTER OR HAS NOT CERTIFIED COMPLIANCE WITH THE REVISED STABILITY CRITERIA FOR ASSUMED AVERAGE WEIGHT PER PERSON (AAWPP) OF 185 POUNDS. BASED UPON A WEIGHT RATIO CALCULATION, THE VESSEL'S CAPACITY IS THEREFORE LIMITED TO: 198 PERSONS (PASSENGERS AND CREW), WITH 64 PERSONS PERMITTED ON THE UPPERMOST DECK 46 CFR 170.090.

Conduct annual service of the fixed fire fighting system.
