Owner is to submit repair plan to local OCMI. Prior to completing repair the OCMI will give permission. Owner is to submit repair plan to local OCMI. Prior to completing repair the OCMI will give permission. When work is completed the owner will request MI attendance to verify proper repair along with sea trials.This can be completed over time. Ensuring passenger safety first.

At each initial and subsequent inspection for certification the marine inspector shall examine and test the following items to the extent necessary to determine that they are in proper operating condition and fit for the service for which they are intended: (1) Main propulsion machinery. The marine inspector may require that the main propulsion machinery be operated both ahead and astern. The attached auxiliary machinery and cooling water system shall be checked. MI discovered the stbd engine would not start. Make repairs to the satisfaction of the COI, and conduct power trails with attending MI.

At each initial and subsequent inspection for certification, the marine inspector shall view the vessel afloat and shall conduct the following tests and inspections of the hull structure and its appurtenances: (1) Examine all accessible parts of the exterior and interior of the hull. MI discovered small weepage on fwd port side of hull. Submit repair plan, will conduct underway trails with MI to evaluate further damage and repairs.

At each initial and subsequent inspection for certification, re inspection and at every other vessel inspection the marine inspector shall require that all observed unsafe practices and hazardous situations be corrected. MI found a significant amount of water and oil in bilges. Make repairs to the satisfaction of the attending MI.

Gages to indicate engine cooling water discharge temperature and lubricating oil pressure shall be provided for all propulsion engines and located so as to be in view of the operator. MI found gages to be inop at control station. Make repairs to the satisfaction of the attending MI.

At each initial and subsequent inspection for certification the marine inspector shall examine and test the following items to the extent necessary to determine that they are in proper operating condition, safe electrical condition, and fit for the service for which they are intended: MI found several lights to be INOP in Engine room, fwd space and Spot light. Make Repairs to the satisfaction of the attending MI.

As appropriate for the intended voyage, all vessels must carry adequate and up-to-date. MI found charts and pubs were not up to date. Maker repairs to the satisfaction of attending MI.

A vessel must comply with the applicable requirements for any radio, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. MI disvoered that the MMSI was not correctly inputed into radio. Make repairs to the satisfaction of the attending MI.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel's shaft is currently leaking a continuous stream of water when in use. 46CFR182.200(a)

Individual power pumps used for separate spaces are to be controlled from a central control point and must have a light or other visual means at the control point to indicate operation. The engine room port side bilge light was inoperable. 46CFR182.520(a)

Drug testing programs required by this part must be conducted in accordance with 49 CFR part 40, Procedures for Transportation Workplace Testing Programs. The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. Vessel owner did not have proof of consortium enrollment, or alcohol testing devices maintained onboard vessel. 46CFR16.113 46CFR185.212 46CFR4.06-15(a)

Each ring life buoy with a floating waterlight must have a lanyard of at least 910 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the waterlight around the body of the ring life buoy. Each rescue boat and ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. 46CFR180.70(d)(2) 46CFR185.604(i)

The name of a vessel must be marked or painted in clearly legible letters and numbers, on each life float and buoyant apparatus. The number of persons capacity must be marked or painted in clearly legible letters and numbers on each life float and buoyant apparatus. The floating waterlight must be attached around the body of the life float or buoyant apparatus by a 10 mm (3/8 inch) lanyard, resistant to deterioration from ultraviolet light, and at least 5.5 meters (18 feet) in length. Vessel's lifefloat was not marked with vessel's name and person capacity, and waterlight line was found wasted. 46CFR185.604(a)(2)&(d) 46CFR180.175(f)(4)

Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018 or other standard specified by the Commandant. 20 Life jackets had wasted retro reflective tape. 46CFR185.604(h)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. First aid kit onboard with incomplete contents. 46CFR184.710

A vessel on a lakes, bays, and sounds, or rivers route must carry: (1) Three hand red flare distress signals approved in accordance with § 160.021 in subchapter Q of this chapter, or other standard specified by the Commandant; and (2) Three hand orange smoke distress signals approved in accordance with § 160.037 in subchapter Q of this chapter, or other standard specified by the Commandant. Vessel had less than the required amount of hand flares. 46CFR180.68(b)

An adult life jacket must be provided for each person carried on board a vessel. Vessel did not have all required life jackets. 46CFR180.71(a)

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Date of the drill and training, General description of the drill scenario and training topics. Vessel did not have record of MOB drills. 46CFR185.20(f)

A power-driven vessel underway shall exhibit side lights and a stern light. Vessel's port and stern light are inoperable. Vessel's restricted to daylight operations only. 33 CFR 83.23(a)

The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. The vessel could not provide logs for EPIRB testing and drills. 46 CFR 185.728(c)

All cable and wire must be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. Cable and wire runs in fuel compartment space are not properly supported. 46CFR183.340(b)(4)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including inspection of the condition of the superstructure. Deck supports in way of port and starboard fuel tanks are not properly fastened. 46CFR176.802(a)(3)

An internal structural examination consists of a complete examination of the vessel’s main strength members, including the major internal framing, the hull plating and planking, voids, and ballast, cargo, and fuel oil tanks. Multiple frames throughout the engine compartment and fuel compartment have wasted in way of the chine bolts, with all chine bolts showing signs of corrosion and multiple failing when turned with a wrench. Frames identified for repair/replacement were chalked for identification and numbered from forward bulkhead as follows: - Fuel Tank Compartment: 2S,2P,3S,3P,4S,4P,5S,5P,8P,&9P - Engine Compartment: 2P,3S,4S,6S,8S,10S,13S,&14S Submit repair proposal for OCMI approval prior to commencing repairs. Repair prior to credit dry-dock. 46CFR176.610(b)

An internal structural examination consists of a complete examination of the vessel’s main strength members, including the major internal framing, the hull plating and planking, voids, and ballast, cargo, and fuel oil tanks. Chine bolts throughout the engine compartment, fuel tank compartment, and forward compartment under operating station are corroding and failing. Areas requiring replacements marked in chalk. Prior to End of Dry-Dock 46CFR176.610(b)

Propulsion machinery must be suitable in type and design for propul sion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Starboard shaft strut is missing one fastener. Prior to End of Dry-Dock 46CFR182.05-1

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including inspection of the condition of the superstructure. Bulkhead sheathing on port head aft bulkhead is delaminating. Superstructure supports, forward, not connected to vessels frames. Prior to End of Dry-Dock 46CFR176.802(a)(3)

The weather deck on a flush deck vessel must be watertight. Port side forward main deck FRP layer cracked and allowing water penetration; deck soft spot in way of port accommodation entrance. Prior to End of Dry-Dock 46CFR171.140(a)

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with the energized parts. Wiring connections to forward bilge pump exposed and creating a hazard. Prior to End of Dry-Dock 46CFR183.200(c)

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping sys tems, or block emergency escapes. Engine room bilge was excessively filled with oily water, inhibiting the full internal structural exam of the engine compartment. 46CFR176.830(b)

The owner or managing operator shall notify the cognizant OCMI as far in advance as possible whenever a vessel is to undergo repairs or alterations affecting the safety of the vessel, together with the nature of any repairs or alterations contemplated. Hull repairs or alternations that affect the safety of the vessel include but are not limited to the replacement, repair, or refastening of planking, plating, or structural members including the repair of cracks. Side planking in way of port forward chine was replaced without notice to the OCMI. 46CFR176.665(a)

The construction and arrangement of a vessel must allow the safe operation of the vessel giving consideration to provisions for a seaworthy hull. Hull watertight integrity compromised with leaking observed at port stern corner and starboard hull in way of engine reduction gear. 46 CFR 177.100

Fill pipes and sounding pipes must be arranged that overflow of liquid or vapor cannot escape to the inside of the vessel. Fuel was found leaking from top of fuel tank. 46 CFR 182.445(d)

Each lighting fixture globe, lens, or diffuser must have a guard or be made of high strength material, where not subject to damage. 3 lights found exposed, damaged, and missing covers. 46 CFR 183.410(a)

All vessels must have navigational lights that are in compliance with the applicable sections of the Inland Navigation Rules. The vessel's stern light was found inoperable. Limited to daylight operations. 46 CFR 183.420

Each ring life buoy with a floating waterlight must have a lanyard of at least 3 feet in length, but not more than 6 feet, securing the waterlight around the body of the ring life buoy. Waterlights on port and starboard life buoys were secured with lanyards significantly exceeding 6 feet in length and tangled with buoyant lifelines. Limited to daylight operations. 46 CFR 180.70(d)(2)

Each floating waterlight must be approved IAW §161.010 in subchapter Q. Incorrect batteries used in all floating waterlights, not meeting manufacturer requirements. Limited to daylight operation. 46 CFR 180.70(d)(1) 46 CFR 161.010–2

All life jacket hardware shall be brass, bronze, or stainless steel. Steel hardware, protected against corrosion by plating, is not acceptable. 11 life jackets were found with corroding plated steel hardware. Warm water passenger count reduced to 41. 46 CFR 180.71(c) 46 CFR 160.055-3(i)

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. First-aid kit onboard with incomplete contents, missing instructions, and not in a suitable, watertight container. 46 CFR 184.710 46 CFR 160.041-4

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Life jacket donning placards not posted for both types of life jackets. 46 CFR 185.516(a)

Webbing used on 25+ person life float must be at least 2 in. wide and must have a breaking strength of at least 1,500 lb. Life float webbing securing platform netting to float deteriorated. Limited to within 1-mile of shore during cold water months (01NOV to 15MAY). 46 CFR 180.200(a)(1) 46 CFR 160.010-4(i)

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards. Unused MSD power supply connected posing electrical hazard. 46 CFR 183.200(b)

Repairs or alterations to hull, machinery, or equipment that effects safety of the vessel must not be made without the approval of the cognizant Officer in Charge Marine Inspection (OCMI) except in emergency. At vessel annual exam the following items were outlined for repair/replacement as original. Provide repair plan to OCMI for approval prior to commencing. Complete repairs prior to end of vessel dry dock in March 2022: 1. Repair rot and leaks to the top (roof) of the accommodation space overhead. 2. Repair rot and wasted wood in way of aft port bulkhead and trim. 3. Repair rotted main deck bulwark cap rail under the deck railing. 4. Repair rotten base of cleats on P&S stern quarters. 5. Replace rotten port, starboard and forward. superstructure bulkhead from base of main deck upward to below pilot house windows. 6. Replace deck seating mounts under benches attached to superstructure. 7. Replace main deck rotten plywood from just forward of superstructure aft to machinery space forward bulkhead. 8. Reef, re-caulk and fill chine seam full length of vessel both port and starboard. 9. Replace stern plank just below engine exhaust ports. 10. Replace rub rail both P&S length of vessel. 46 CFR 176.600

At vessel annual exam the following item was outlined for repair/replacement as original and added to list issued 27SEP2021. Provide repair plan to OCMI for approval prior to commencing. Complete repairs prior to end of vessel dry dock in March 2022. Vessel is limited to operations within 1-mile from land until permanent repairs completed. 11. Repair port stern corner hull and starboard hull amidship in way of engine reduction gear. 46 CFR 176.600

clean out, reeve, re-calk and seal port side plank numbers 1&2 seam including interface with stern at planks 3, 3&4 above chine board.

Re-caulk between planks 3, 4&5 In way of stern corner port side.

All vessels must have navigational lights that are in compliance with the applicable sections of the Inland Navigation Rules. The vessels stern light was inoperable. 46 CFR 183.420

Each life ring buoy on a vessel must be readily available. The starboard side ring buoy had water inside the buoy and not ready for use. 46 CFR 180.70

Vessel has minor water leak IWO of stbd side engine room above chine and behind rib. Owner/operator to effect temp. repairs and submit repair proposal to effect permanent repairs prior to 01OCT17 when vessel is hauled. Vessel deckhand to periodically check on leak during vessel trips/voyages.

Vessel's stbd chine IWO of engine room leaking water excessively. Submit repair plan to local OCMI.

Stbd rudder port leaking water excessively. Ensure water is not leaking through rudder post excessively.

Stbd fuel tank leaking diesel fuel from crossover valve. Ensure diesel tank/valve is no longer leaking fuel.

Vessel's internal lighting system is not operating properly. Ensure no electrical hazard exists causing lighting system to fail.

Navigation lights are not operaitonal. Ensure nav. lights are operational prior to night time operation.

Vessel did not have valid FCC safety cert. onboard

Ensure old anchor battery is removed and new anchor battery is housed and connected properly.

Ensure enginge room light is properly housed/covered

Remove/propelry dead end electrical wiring in E/R and fwd cmpt.

Magnetic compass has light equipped and light was not operational. Ensure compass light is operational.

Electrical outlet guard was missing in fwd cmpt. Ensure outlet is properly covered.

Life float floating water light battery was expired. Ensure non expired and proper battery is installed.

Remove oil from bilges and operationally test bilge pumps and high water alarms.

Repair excessive water leaking from the Port & Starboard shaft seal, as per 46 CFR 176.800.

Repair Public Announcement system, as per 46 CFR 184.610

Repair wiring on bilge pump located in the Bow Space. The wiring is currently being held together by electrical tape and is currently coming apart (exposed); reference 46 CFR 183.34

Vessel is required to maintain a training and maintenance logs, as per 46 CFR 185.420.

Maintain a First Aid kit onboard vessel, as per 46 CFR 184.710

Maintain emergency instructions that are “ship-specific”; as per 46 CFR 185.510

Repair or replace (03) life vest that were found to be discrepant; as per 46 CFR 180.71 (note: vessel’s passenger count is reduced to 46 until issue has been rectified).

Replace batteries in (02) life ring lights and (01) life float light with approved marine-grade batteries that are designed for life-saving applications; as per 46 CFR 180.70

Ensure all acting crew members are enrolled in the DAPI program; as per 46 CFR 16.203. The ONLY person that can currently serve in the capacity of the crew member is the one actively enrolled in the DAPI program.

Provide company policy for DAPI program and post the “Hotline” phone number; as per 46 CFR 16.401

Provide repair proposal to address the weepage in way of the chine on the Portside / Mid-section in the engine room compartment. Crew member will be required to visually inspect the affected areas of concern while the vessel is carrying passengers for hire; and notify vessel master if conditions worsen. Vessel master will be required to notify this office in the event the rate of weepage increases

Vessel issued CG-835 to completely refasten hull of vessel at the keel and chine both port and starboard side to include area all the way to the transom.

Starboard side of the upper Stem soft. Remove soft wood and make available for inspection.

Attache Frame 2 starboard side in forepeak to planks. Planks have worked themselves away from the frame.

Refasten/replace bolts in chine at scarfs as marked in vessels fuel compartment.

Repair/replace side shell frames as marked in engine, fuel and below op station compartments

Remove and treat furring of frames ([name withheld]) as marked within vessel engine room.

Vessel issued CG-835 during Fastener Pull to refasten all of Stringer 1 and 2 on both the port and starboard sides due to the condition of the fasteners removed from those stringers.
