When the main steering gear includes two or more identical power units, no auxiliary steering gear need be fitted, if— (2) In a cargo vessel, the main steering gear is capable of moving the rudder as required by paragraph (c)(2) of this section while all the power units are operating. While underway, vessel reported a loss of No.2 steering pump and returned to port using No.1 steering pump. Determine cause and prove proper operation of both No.1 and No.2 steering pumps prior to departure. 46 CFR 58.25-10(f)(2) - 17 c.

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. Rescue boat davit inoperable and unable to lower and launch rescue boat in the event of emergency. 46 CFR 199.190(a) – 17 c.

On vessels required by § 112.05-5(a) to have a temporary emergency power source, the following emergency lighting and power loads must be arranged so that they can be energized from the temporary emergency power source: depth sounder. Bridge depth sounder does not power when on emergency battery power. 46 CFR 112.15-5(j) - 17 c.

Each vessel must have a line-throwing appliance approved under approval series 160.040. All the line-throwing appliances on board have expired pyrotechnics. 46 CFR 199.170 – 17c.

Remote valve controls must be accessible under normal service conditions. The port side remote fuel oil shut off valves were not accessible on the main deck due to cargo stowage. 46 CFR 56.50(g)(2)(ii) – 17c.

Any vessel of at least five net tons which engages in the fisheries on the navigable waters of the United States or in the Exclusive Economic Zone, or coastwise trade, unless exempt under § 67.9(c), must have a Certificate of Documentation bearing a valid endorsement appropriate for the activity in which engaged. For each 79,500 L (21,000 U.S. gallons) or part thereof of any Class 3 (flammable) or combustible liquid being transported on board a vessel in a portable tank, there must be provided at least one B-V semiportable foam (152 L/40 gallon capacity) (see 46 CFR 95.50), dry chemical (45.4 kg (100 pounds) minimum capacity) or equivalent fire extinguisher, or a fire hose fitted with an approved portable mechanical foam nozzle with pick-up tube and two 19 L (5 gallon) cans of foam liquid concentrate. Each foam system must be suitable for use with each Class 3 (flammable) or combustible liquid for which it is required. Each fire extinguisher must be accessible to the tank it is intended to cover. Vessel does not have approved semi-portable fire suppression equipped onboard as needed when carrying Class 3 combustible liquids in ISO tanks. Install approved 160-B semi-portable fire extinguisher on deck when carrying Class 3 cargo to satisfaction of MI. 49 CFR 176.315 - 705

All automatic control, remote control, safety control, and alarm systems must be failsafe. The high jacket water temp alarm for the forward ships service diesel generator was not operational at time of inspection. 46 CFR 62.30-1(b) - 16 c.

Each vessel must have an amplifier-type announcing system that will supplement the general emergency alarm. Forward-facing public address system is inoperable. 46 CFR 113.50 – 16 c.

Each lifesaving appliance, including lifeboat equipment, must be inspected monthly using the checklists required under paragraph (b)(1)(i) of this section to make sure the appliance and the equipment are complete and in good working order. A report of the inspection, including a statement as to the condition of the equipment, must be recorded in the vessel's official logbook. Records of monthly maintenance or weekly checks for rescue boat were not on file or available during annual inspection. 46 CFR 199.190(e)(1) - 50 c.

Where fire extinguishing systems or equipment are not required, but are installed, the system or equipment and its installation must meet the requirements of this part. CO2 Bottle and hose reel in the port side deck store, found with cracking hose material and corroded or seized components. Remove from vessel or service, so that it is ready for immediate use. 46 CFR 95.01-5 - 50 c.

An inspection either general or partial depending upon the circumstances shall be made whenever any important repairs or alterations are undertaken. Vessel experienced a reduction in RPM resulting in a shutdown of the port main engine while underway, inbound to Apra Harbor, Guam. Affect repairs and identify the cause of the casualty. Perform satisfactory sea trials or engine tests in the presence of attending Marine Inspector prior to departure from port. 46 CFR 91.45-5 - 17 c.

The cylinders must be installed to provide a space of at least 2 inches between the deck and the bottom of the cylinders. MI identified 01 cylinder is currently on the deck. Vessel to install cylinder off of the deck. 46 CFR 95.16-20(f)

Lifeboat and rescue boat release gear must be operationally tested under a load of 1.1 times the total mass of the lifeboat when loaded with its full complement of persons and equipment whenever overhauled or at least once every 5 years. Prior to the test, the attending marine inspector discovered the rescue boat davit's safe working load was not rated to handle the rescue boat with 1.1 times the full complement of persons and equipment. Vessel is restricted to a Coastwise route (not more than 20 miles from shore), and only authorized to operate on this route if one of the following conditions are met: 1) Table 199.610(c) Exemptions for Cargo Vessels in Specified Services, Rescue boats note 3; or 2) 46 CFR 199.640(g). In addition, a satisfactory rescue boat release gear operational test under a load of 1.1 times the total mass of the lifeboat when loaded with its full complement of persons and equipment must be witnessed by a Coast Guard marine inspector prior to the vessel operating on an Oceans route. 46 CFR 199.190(i)(5)

No repairs affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. Port and stbd bulkhead plating in way of chain locker, frame 10 has areas of wastage noted on gauging report No. 24-125, page 34 (items 18-19), page 35 (item 23), and as bulkhead marked by ABS surveyor. Submit a repair proposal for approval to the attending Marine Inspector. Once the repair proposal is approved by the attending Marine Inspector, complete the repairs to the satisfaction of the attending Marine Inspector. 46 CFR 91.45-1

The deck line is a horizontal line 300 mm in length and 25 mm in breadth. It shall be marked amidships on each side of the ship, and its upper edge shall normally pass through the point where the continuation outwards of the upper surface of the freeboard deck intersects the outer surface of the shell, provided that the deck line may be placed with reference to another fixed point on the ship on condition that the freeboard is correspondingly corrected. Ensure deck line is clearly marked on both sides of the vessel. ILLC (88) Reg 4

Each lifeboat and rescue boat must be plainly marked as follows: The type II retro-reflective material approved under approval series 164.018 must be placed on the boat to meet the arrangement requirements in IMO Resolution A.658(16). Replace all retro-reflective material on the rescue boat that has lost reflective properties. 46 CFR 90.27-1 46 CFR 199.176(4)

No. 2 generator was leaking fuel oil at the high pressure pump. Provide repairs and prove generator is in serviceable condition and no longer leaking fuel oil. 46 CFR 91.27-15

Found excessive lube oil and hydraulic oil in the bilge. Repair dirty oil hose reel back to satisfactory condition and ensure bilges are clean of oil. 46 CFR 91.27-15

Found aft engine room watertight door was hitting and damaging high pressure hydraulic pipe. Install a door stop and prove door no longer can strike the pipe. 46 CFR 91.27-15

Each survival craft, except for inflatable craft, must be stripped, cleaned, and thoroughly inspected and repaired, as needed, at least once each year and each fuel tank must be emptied, cleaned, and refilled with fresh fuel. No evidence of annual maintenance onboard. Provide record of annual servicing and maintenance. 46 CFR 199.190

Cables or individual open service conductors shall be supported as specified in 230.51(A), (B), or (C). (A) Service Cables. Service cables shall be supported by straps or other approved means within 300 mm (12 in.) of every service head, gooseneck, or connection to a raceway or enclosure and at intervals not exceeding 750 mm (30 in.). (B) Other Cables. Cables that are not approved for mounting in contact with a building or other structure shall be mounted on insulating supports installed at intervals not exceeding 4.5 m (15 ft) and in a manner that maintains a clearance of not less than 50 mm (2 in.) from the surface over which they pass. The cable raceways in the engine room are not supported by straps or have connections exceeding 30in. Support cables. 46 CFR 110.10 NFPA _230.51

Approved fire detection and alarm systems must be installed on the following vessels as set forth in subpart 76.27 of this part. Several smoke detectors were non-operational, replace. 46 CFR 76.05-1(a)

Each oil tanker of 150 gross tons and above, ship of 400 gross tons and above other than an oil tanker, and manned fixed or floating drilling rig or other platform shall maintain an Oil Record Book Part I (Machinery Space Operations). (h) Each operation described in paragraphs (d), (e) and (f) of this section shall be fully recorded without delay in the Oil Record Book so that all the entries in the book appropriate to that operation are completed. Each completed operation shall be signed by the person or persons in charge of the operations concerned and each completed page shall be signed by the master or other person having charge of the ship. Previous Master of the vessel failed to sign the Oil Record Book. 33 CFR 151.25

Except under paragraph (a)(2) of this section, vessels that operate in salt water must be examined in accordance with the intervals set forth in Table 91.40-3(a) of this section. Where Table 91.40-3(a) indicates a 2.5 year examination interval, it means a vessel must undergo two examinations within any five year period. No more than three years may elapse between any two examinations. The vessel has failed to complete a drydock examination as required by Table 91.40-3(a). The vessel is prohibited from conducting commercial operations until it completes a drydock examination. The vessel may only operate in a commercial capacity to conduct the National Aeronautics and Space Administration capsule recovery operation as outlined in Sector Honolulu letter 22-235 dated October 28, 2022.

Indication of the rudder angle must be provided both at the main steering station in the pilot house and in the steering-gear compartment. The rudder-angle indicator must be independent of control systems for steering gear. The port rudder angle indicator position at the emergency steering station in the engine room/steering gear compartment was 9 degrees different than the port rudder angle indicator position on the bridge. Repair the rudder angle indicator so that it indicates the actual position of the rudder. Repairs shall be to the satisfaction of the attending Marine Inspector. 46 CFR 58.25-25 (a)

Tests must be completed at periodic intervals specified by the Coast Guard to confirm that vital systems and safety features continue to operate in a safe, reliable manner. The crew was unable to demonstrate the port and starboard main engine overspeed tests in accordance with the vessel’s Coast Guard approved periodic safety test procedures. The proper servicing and of each inflatable liferaft shall be verified at each re-inspection. Vessel has two 25 person liferafts that are required to be serviced on March 22, 2022. Credit for annual inspection will not be given until liferaft servicing documentation is provided. 46 CFR 199.45 (b)(4)

The Type II retro-reflective tape must be placed on the rescue boat in accordance with IMO Resolution A.658(16). Retro-reflective tape on the bow of the rescue boat is severely degraded. Replace. 46 CFR 199.176(a)(4)

When stability limitations for a vessel are prescribed, the assigning authority shall furnish the master the vessel's maximum draft permitted and other conditions, including reference to Commandant approved operating stability features, which may be applicable. Vessel's updated stability documentation is under review by the Marine Safety Center (MSC). Vessel may continue to operate within the parameters of the vessel's existing U.S. Coast Guard stability documentation and American Bureau of Shipping load line assignment. 46 CFR 42.09-1(a)

Except as provided by 15.725 of this part, no vessel may be navigated unless it has in its service and onboard the crew compliment required by the COI. Vessel is not currently operational and does not have required crew onboard. Manning meeting the vessel's COI must be verified by attending marine inspector. 46 CFR 15.515(a)

Every crew member must participate in at least one abandon-ship drill and one fire drill every month. Vessel is not currently operational and does not have required crew onboard. Satisfactory fire and abandon-ship drills must be carried out to satisfaction of attending marine inspector. 46 CFR 199.180(c)(2)

Rescue boats must be launched with their assigned crew aboard and maneuvered in the water each month. Vessel is not currently operational and does not have required crew onboard. Rescue boat launch, maneuvering, and recovery with assigned crew must be carried out to the satisfaction of the attending marine inspector. 46 CFR 199.180(d)(7)

Drug testing programs required by this part must be conducted in accordance with the 49 CFR 40. Vessel could not provide any evidence of a DAPI program. 46 CFR 16.201(a)

The marine employer shall ensure that all persons directly involved in a serious marine incident are chemically tested for evidence of dangerous drugs and alcohol in accordance with the requirements of 46 CFR 4.06. Provide evidence of drug and alcohol testing kits. 46 CFR 16.240

The main steering gear and rudder stock must be of adequate strength for and capable of steering the vessel at maximum ahead service speed. Vessels port steering gear has packing gland studs that appear to have backed out, no longer providing any pressure to the packing gland and allowing the gland to move approx 1/4" when rudder is in operation. 46 CFR 58.25-10 (b)(1)

Except as provided in paragraph (a)(3) of this section no person may operation an oceangoing ship of 400 gross tons and above but less than 10,000 gross tons, excluding a ship that carries ballast water in its fuel oil tanks, unless it is fitted with approved 15 parts per million (ppm) oily-water separating equipment for the processing of oily mixtures from bilges or fuel oil tank ballast. Vessels OWS is inoperable. Prove satisfactory operation to attending Marine Inspector, additionally provide piping diagram for oil content meter as requiring by separating equipment's manufacture's instructions. 33 CFR 155.360 (a)(1)

Containers, brackets, racks, and other similar stowage locations for lifesaving equipment must be marked with symbols in accordance with IMO Resolution A.760(18) indicating the device stowed in that location. Vessel's life jackets and immersion suits stowed in berthings and watch stations not properly marked. 46 CFR 199.178(a)

Electric installations on vessels must ensure: Protection of passengers, crew, other persons, and the vessel from electrical hazards. Vessels energized shore tie cable is laying on engine room deck creating electrical hazard. 46 CFR 111.01-1 (a)(2)

The routine maintenance of the oily water separating equipment and the bilge alarm must be clearly defined by the manufacturer in the associated operating an maintenance manuals. All routine and repair maintenance must be recorded. Vessel could not produce evidence of routine and repair maintenance. Provide schedule IAW manufactures instructions. 33 CFR 155.380 (f)

Vessel does not currently have the sufficient manning to meet the COI requirements for the intended voyage. COI requires that there is 3 oilers, which are not currently on board.

VESSEL'S MARINE TYPE ENGINES SHALL BE SUITABLE FOR INTENDED SERVICE; PORT MDE WAS INOPERABLE. CONDUCT REPAIRS SUCH THAT PORT MDE OPERATES CORRECTLY. 46 CFR 58.10-5

CARGO VESSELS LESS THAN 85m IN LENGTH SHOULD CARRY ON EACH SIDE OF THE VESSEL LIFERAFTS WITH AGGREGATE CAPACITY SUFFICIENT TO ACCOMMODATE 150% OF THE TOTAL NUMBER OF PERSONS ON BOARD; VESSEL SHALL PROVIDE ADDITIONAL LIFERAFT OF WEIGHT PERMITTING FOR EASY SIDE-TO-SIDE TRANSFER AT THE OPEN DECK. 46 CFR 199.261(d)(ii)
