Piping, valves, and fittings in a fire main system must comply with subpart G, part 182, of this chapter. Fire main has a leak (drop a second) close to branching of line (ivo gauge/before the front portion of stbd M/E) to fwd section of the vessel. To be repaired within 7 days (10DEC2025). 46CFR181.310(b)

Exhaust gas must not leak from the piping or any connections. Stbd engine secondary exhaust cooling pump inject point is leaking. 46CFR182.430(b)

All vessels must have navigation lights that are in compliance. Anchor light is not operational. 46CFR183.420

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Onboard first aid kit does not meet standard. 46CFR184.710

Piping used in a vital system must: (1) Be composed of ferrous materials. (2) If subject to a pressure of more than 1,034 kPa (150 psig), be designed, fabricated, and inspected in accordance with the principles of ANSI B 31.1 (incorporated by reference; see 46 CFR 175.600) or other standard specified by the Commandant. Fire main piping has a pin-hole leak. Repair to satisfaction of attending USCG Marine Inspector.

In systems and applications where flexible hoses are permitted to be clamped: Double hose clamping is required where practicable; Two clamps must be used on each end of the hose, or one hose clamp can be used if the pipe ends are expanded or beaded to provide a positive stop against slippage. Install double hose clamps where required.

Fire pumps: (b) On a vessel of more than 19.8 meters (65 feet) in length, the minimum capacity of the fire pump must be 189 liters (50 gallons) per minute at a pressure of not less than 414 kPa (60 psi) at the pump outlet. The pump outlet must be fitted with a pressure gauge. Fire pump pressure gauge is inop, repair/replace gauge as required. To be rectified by 06 NOV 2024.

Each marine sanitation device must - Be designed to prevent the overboard discharge of treated or untreated sewage or any waste derived from sewage. Make repairs to suspected leaks on Port sewage holding tank.

47 CFR 80.401 - Licensees of radio stations are required to have current station documents. Update station license to reflect current EPIRB registration and total persons allowed as per COI.

47 CFR 80.155 - Operation of transmitters of any ship station must be performed by a person holding a commercial radio operator license or permit. Provide documentation that operator has obtained Ship station operator permit.

Provide written proposal for conducting internal inspection of fire-main to determine current overall condition. Inspection to be conducted in the presence of attending Marine Inspector. Proposal to be submitted no later than 08 November 2024.

Provide required OCMI plan approval for fire-main system to include material specifications. Provide requested information no later than 15 November 2024.

Conduct internal inspection of fire-main piping in presence of attending Marine Inspector. Conduct internal inspection no later than 14 November 2024.

Except as otherwise permitted, piping systems must be subjected to a hydrostatic test pressure that at every point in the system is not less than 1.5 times the maximum allowable working pressure for a minimum of 10 minutes. Fire main max pressure 60 psi, test pressure 80psi. 46CFR 56.97-30(d)(e)

Where nonferrous metallic material is permitted for use in piping systems, provisions must be made to prevent or mitigate the effect of galvanic corrosion due to the relative solution potentials of copper used in conjunction with steel, or other metals and their alloys. Replace existing steel fire pump with bronze/non ferrous pump as originally approved and designed. Replacement to be conducted during dry dock scheduled 04Mar2024. To be completed prior to returning to service. 46 CFR 182.730(b)(2)

Nonferrous metallic piping with a melting temperature above 1,700 degrees F may be used when specifically accepted by the [name withheld] un-approved repairs (09) in engine room IWO fire pump and (02) in crew accommodation space with sweat type connections as per original approved design. 46 CFR 182.730(a)(4)

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. EPIRB hydrostatic release unit expired on 09/2023. Renew hydrostatic release unit. 46 CFR 185.740(b)

When, in accordance with § 178.210 of this chapter, a vessel must be provided with a stability letter, the stability letter must be posted under glass or other suitable transparent material, such that all pages are visible, at the operating station of the vessel. Stability letter was posted in the passenger accommodation space on the main deck. Post stability letter at the operating station. 46 CFR 176.306

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: U.S. Coast Pilot or similar publication and Coast Guard Light List. Extracts from the publications listed above for the areas to be transited may be provided instead of the complete publication. Coast Pilot and Light List were dated. Provide up-to-date publications. 46 CFR 184.420

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. First aid kit aspirin and eye wash were expired. Renew medication. 46 CFR 184.710

Remote fuel shutoff stations must be marked in clearly legible letters at least 1 inch high indicating purpose of the valve and direction of operation. Labeling for fuel shutoff valves was either missing or worn. Renew labeling. 46 CFR 185.608

Abandon ship, man overboard and fire fighting drills and training shall be logged or otherwise documented for review by the Coast Guard. The drill entry shall include the following information: Date of the drill and training and general description of the drill scenario and training topics. Drills were not being logged. Educated operator on the proper method of logging drills. 46 CFR 185.520(f) & 46 CFR 185.524(d)

An adult lifejacket must be provided for each person carried on board. One adult lifejacket was found unserviceable. Repair or replace one adult lifejacket. Vessel reduced to 155 total persons allowed until this is completed. 46 CFR 180.71

Engine exhaust pipe installation. Protection must be provided in compliance with § 177.970 of this chapter at such locations where persons or equipment might come in contact with an exhaust pipe. Exhaust lagging iwo the turbo chargers on both MDE's exhaust lagging is worn or missing. 46 CFR 182.430.

Lighting fixtures. (a) Each lighting fixture globe, lens, or diffuser must have a guard or be made of high strength material, except in an accommodation space, radio room, galley, or similar space where it is not subject to damage. (b) A lighting fixture may not be used as a connection box for a circuit other than the branch circuit supplying the fixture. (c) A lighting fixture must be installed as follows: (1) Each fixture must comply with § 183.200. (2) Each lighting fixture and lamp holder must be fixed. A fixture must not be supported by the screw shell of a lamp holder. (3) Each pendant type lighting fixture must be suspended by and supplied through a threaded, rigid conduit stem. The light fixture centerline in the engine room is missing the protective globe and light bulbs. Repair or replace light fixture. 46 CFR 183.410

Cable and wiring requirements. Each connection to a conductor or terminal part of a conductor must be made within an enclosure and have either: (1) A pressure type connector on each conductor; (2) A solder lug on each conductor; (3) A splice made with a pressure type connector to a flexible lead or conductor; or (4) A splice that is soldered, brazed, or welded to a flexible lead or conductor. Wiring under the bridge console and two exterior PA speakers, one aft on the upper deck and one on the stbd side upper deck wire connections are broken and exposed. Properly repair wiring and PA speakers. 46 CFR 183.340.

Stability letter. When, in accordance with § 178.210 of this chapter, a vessel must be provided with a stability letter, the stability letter must be posted under glass or other suitable transparent material, such that all pages are visible, at the operating station of the vessel. If posting is impracticable, the stability letter must be kept on board in a weathertight container readily available for use by the crew and display to passengers and others on request. Properly post vessels stability letter on the bridge. 46 CFR 176.306

Charts and nautical publications. (a) As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: (1) Charts of large enough scale to make safe navigation possible; (2) U.S. Coast Pilot or similar publication; (3) Coast Guard Light List; (4) Tide tables; and (5) Current tables, or a river current publication issued by the U.S. Army Corps of Engineers or a river authority. (b) Extracts from the publications listed above for the areas to be transited may be provided instead of the complete publication. Have onboard updated nautical publications for operating areas vessel has 2021 publications. 46 CFR 184.420

Every seaman referred to in this section, when required, must produce a valid MMC or MMD with all applicable rating endorsements for the position sought, a valid TWIC unless the mariner is exempt under 46 CFR 10.203(b)(2), and a valid medical certificate to the master of the vessel at the time of his or her employment before signing Articles of Agreement. Seamen who do not possess one of these credentials may be employed at a foreign port or place within the limitations specified in § 15.720 of this part. The master onboard the vessel at the time of inspection did not have their medical certificate. 46 CFR 15.403

First-aid kits. A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. First aid kit aspirin was expired. 46 CFR 184.710

Per D17 Inst 16714.1A, vessel is required to have 100% IBA capacity for all persons onboard. Vessel operator has sent out vessels outfitting of IBA's and replaced with three 50 person and one 4 person IBA. Total persons allowed onboard is reduced to 154 total persons allowed. IAW 46 CFR 180.25(b).

An adult life jacket must be provided for each person carried on board a vessel. Three of the adult type 1 life-preservers were condemned due to poor condition. Total persons allowed reduced to 153 until the damaged adult type 1 PFD's replaced to the satisfaction of the attending CG Marine Inspector. IAW 46 CFR 180.71(a)

Vessel is required to have two fixed power bilge pumps. The vessels fire pump which is the second fixed bilge pump was unable to draw bilge suction. Properly repair the fire/bilge pump. IAW 46 CFR 182.520

When a flexible fill pipe section is a nonconductor of electricity, the metallic sections of the fill pipe separated thereby must be joined by a conductor for protection against generation of a static charge when filling with fuel. There are no bonding wires connecting the sections of piping were there are sections of flexible hose. IAW 46 CFR 182.445(g)

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Properly repair hydraulic oil leak on the port stabilizer and clean oil from bilges in the aft steering space. IAW 46 CFR 182.100 & 176.830.

Replace expired battery in life ring buoy light outside of bridge wing starboard side.

Remove old sewage pipe running though the aft bulkhead in the tank room to the Laz. Space

Provide fire sleeve for both generator fuel hoses to meet the requirements of SAE-1942.

Provide metal wire support ever 24” for wire runs in engine room

Provide proper supports for all pipes and hoses in engine room.

Provide bonding for both rudder post.

Repair or replace 37 damaged adult and 13 children lifejackets. Vessel has provided 28 adult temporary lifejackets

Five of vessel's life jackets were damaged beyond usage. Repair or replace damaged life jackets. Vessel limited tto one hundred fifty one (151) persons on board until life jackets are provide.

Two life bouy light's batteries were expired. Renew light batteries prior to night time operations.

Emergency escape from crew quarters was blocked with miscelleneous gear. Remove gear blocking escape.

Navigational charts were out of date and not corrected. Provide updated nav chart.

Electrical cable for outlet switch in bow thruster space was improperly spliced. Renew cable or make proper splice in electrical cable.

Coast Pilot and Light List were out of date. Provide updated navigational publications.

30 second time delay activated at 41 seconds. Replace time delay and demonstrate system to the satisfaction of the attending CG Marine Inspector. Until such time as time delay is replaced/tested, vessel authorized to operate in accordance with approved contingency plan.

VESSEL’S EIGHT PERSON INFLATABLE BUOYANT APPARATUS WAS REPLACED WITH A SIX PERSON INFLATABLE BUOYANT APPARATUS. VESSEL IS RESTRICTED TO CARRYING ONE HUNDRED FIFTY FOUR (154) PERSONS. LIMITED TO CARRY ONE HUNDRED FORTY EIGHT (148) PASSENGERS WITH EIGHT (8) CREW. WHEN ON A VOYAGE OF LESS THAN 12 HOURS, THE PASSENGERS MAY BE INCREASED TO ONE HUNDRED FIFTY (150) WITH SIX (6) CREW. VESSEL RESTRICTED TO REDUCTION IN PERSONS ON BOARD UNTIL EIGHT PERSON INFLATABLE HAS BEEN REINSTALLED ONBOARD VESSEL AND INSTALLATION HAS BEEN INSPECTED BY THE ATTENDING COAST GUARD MARINE INSPECTOR.

Vessel is required to have either permanent draft marks or permanent loading marks displayed on the hull. Vessel's hull is absent either.

Prove proper operation of all emergency lighting.

Remove dead end wiring for removed wiper motors on main deck fwd. and for old camera system in sewage space.

Properly wire in heater blower motor on main deck fwd.

Properly install float water light to lifebuoy.

Provide updated U.S. Coast Pilot [name withheld] list and Charts for area onboard vessel.

Provide better documentation of man overboard, abandon ship and fire drills to include date of drill and training and general description of drill and training topic covered during next annual inspection.

Make passenger safety orientation before getting underway or as soon as practicable every time getting underway.

All penetrations in a watertight bulkhead must be made watertight. the port main engine aft bulkhead exhaust penetration was identified as not being watertight.

Provide secondary hose clamp securing the stbd main raw water suction nonmetallic hose.
