Except for engine exhausts, each inlet or discharge pipe that penetrates the hull below a line drawn parallel to and at least 150 millimeters (6 inches) above the deepest load waterline must have means to prevent water from entering the vessel if the pipe fractures or otherwise fails. Below valves are unsat or being replaced by O/O for convenience. 1. Port M/E Intake Valve (not closing properly) 2. Port Compressor Valve (hard to close) 3. Starboard Bilge Discharge Valve (swapping gate valve with ball valve). Once valves have been installed, schedule inspection with attending marine inspector.

Public address / general alarm not audible in all parts of passenger bunk room. Provide speakers to ensure system is audible in all passenger accessible spaces.

Vessel's three pahse system not isolated properly from shoreside single phase power. When operating using shore power, ensure isolation of three phase loads.

Each life float must be stowed so that in the event of the vessel sinking the life floats are able to float free. Three (03) life floats stowed in a deck box on the port side of the upper deck were found to not be in a float free arrangement. The pilothouse structure prevents the deck box lid from floating free. Prove float free arrangement for port side life floats to an attending marine inspector prior to getting underway.

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. Starboard side main deck super structure has small openings to the engine room that do not have closure devices. Provide closure devices and prove proper operation to the satisfaction of an attending marine inspector prior to getting underway.

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in normally unmanned spaces. Vessels constructed of wood must also provide bilge level alarms in all watertight compartments except small buoyancy chambers. Bilge high water level alarm for the vessel's forepeak was inoperable. Prove proper operation of the bilge high level alarm in the forepeak to the satisfaction of an attending marine inspector prior to getting underway.

Construction and arrangement shall allow free and unobstructed access to the bunks. Each bunk shall be immediately adjacent to an aisle leading to a means of escape from the lounge area. Double bunks do not allow for free and unobstructed escape for the inside occupant. All double bunks currently installed may only be occupied by one (1) person. Overnight passenger capacity has been determined to be thirty-two (32).

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Vessel has multiple styles of life jackets on board. Provide instructions for donning and use of each type of life jacket carried.

A vessel of more than 26 feet in length but not more than 65 feet must carry a life buoy which meets the requirements of 46 CFR 180.70 and 46 CFR 160.050. Life buoy was sun damaged, had dry rotted grab lines and buoyant line, and ring buoy was not marked with vessel name. Provide Serviceable ring buoy appropriately provisioned, marked and installed.

The design, construction, and stowage of portable tanks and related fuel lines and accessories must meet the requirements of ABYC H-25 or other standard specified by the Commandant. Demonstrate that portable gasoline tanks used for the dive tender vessel and stowed on the dive tender vessel and on the aft deck meet ABYC H-25.

Mattresses shall be covered with a material that has been treated to give it fire resistant properties. It was unknown if the mattress covers had been treated with a fire resistant material. Provide proof that mattresses were installed prior to March of 1996 and treat all mattress covers with a material to give them fire resistant properties, or install new mattresses that meet the requirements of 46 CFR 177.405(g)(1) or (g)(2).

All griddles, broilers, and deep fat fryers must be fitted with a grease extraction hood in compliance with 46 CFR 181.425. Vessel is fitted with a heat detection system approved in accordance with G-MOC Policy 4-98, which was discovered to have a disarming switch fitted. Remove ability to disarm the galley heat detection system. - 46 CFR 181.405

Repairs or alterations to the equipment that effect the safety of the vessel must not be made without the approval of the cognizant OCMI, to include electrical wiring and power supply systems. Vessel was found with numerous added electrical loads, including a NITROX generating system in the lazarette, food freezers in the lazarette, electric clothes dryer in the lazarette, a shower/bilge drain pump in the lazarette, a new electric double oven in the galley, additional engine room ventilation fans, a salon ice maker, additional salon refrigerator, hydraulic pump for dive platform, and an electric grill on the main deck. In addition, the shore tie was found without a circuit breaker installed at the switchboard. Provide plans for approval showing electrical generating and distribution system as installed, to include materials used, over current protection details, and load analysis.

Battery chargers must have an ammeter connected in the charging circuit. One battery charger in the engine room was found to be missing an ammeter. Provide ammeter in each battery charging circuit.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons and the vessel from electrical hazards, and minimize accidental personnel contact with energized parts. Motor controller housing for dive air compressors in the engine room were found to have been modified; Controller housing was found holed for additional ventilation, exposing controller. Provide intact motor controllers which provide protection from electrical hazards.

Alterations to the hull, machinery, or equipment that effect the safety of the vessel must not be made without the approval of the cognizant [name withheld] platform installed on transom. Plans are not available demonstrating review of installation. Provide approved plans or submit plans for approval.

Provide current FCC radio station license. If new call sign/MMSI are issued, update VHF/EPIRB and all other impacted documentation.

Provide fixed CO2 fire extinguishing servicing report.

Provide current navigation publications, to include coast pilot, light list, tides and currents.

Repair hydraulic leak on port main engine transmission.

Scrape/clean/treat split port bilge stringer outboard of port main engine.

Grind/treat hull (interior) in way of the stbd sewage valve.

Service the port outboard compressor cooling sea suction valve.

Seal seams of collision bulkhead to ensure watertight integrity.

Repair the forepeak hatch gasket.

Make vessel available for post drydock check ride.

Service stbd main diesel raw water valve.

Remove all welding cable from electrical circuit and replace with approved marine cable.

INSTALL FIXED FIRE EXTINGUISHING SYSTEM IN ACCORDANCE WITH COAST GUARD APPROVAL LETTER DATED 07 JUNE 1999.

CONDUCT POST INSTALLATION INSPECTION OF FIXED FIRE EXTINGUISHING SYSTEM IN THE PRESENCE OF THE ATTENDING COAST GUARD MARINE INSPECTOR PRIOR TO CARRYING PASSENGERS FOR HIRE.

Install retroreflective tape on all five life floats.

Provide updated copy of 46 CFR 166-199.

Refasten starboard rudder post mounting bracket with new fastners.

Reinstall clevis and install cotter pin on stbd. rudder arm.

Replace existing white ring buoy with a Coast Guard approved oragne ring buoy with retroreflective tape.

replace non-approved flexible non-metallic hose on bilge pump with approvedhose.

install close nipple and cap on bleed valve to mask rinse pump.

Repair or replace the lifering prior to carring passengers.

Find the date for the hydro release for the EPIRB, replace if needed.

COMPLETE DRYDOCK INSPECTION.

CONDUCT UNDERWAY FIRE AND LIFESAVING DRILLS IN THE PRESENCE OF A COAST GUARD MARINE INSPECTOR.

REMOVE FROM VESSEL INSTALLED EPIRB WHICH IS UNFIT FOR SERVICE & REPLACE WITH CG APPROVED EPIRB.

PROVIDE & INSTALL LOCKING DEVICE FOR OVBD DISCHARGE VALVE FROM MSD. VALVE TO BE KEPT SECURED WITH THREE MILES OF LAND.

PROVIDE CURRENT LOCAL NOTICE TO MARINERS & UPDATE ALL CHARTS FOR AREA OF OPERATION.

PROVIDE 8 ADDITIONAL ADULT & 2 ADDITIONAL CHILD'S CG APPROVED TYPE I LIFEJACKETS PRIOR TO CARRYING MORE THAN 47 ADULTS & 4 CHILDREN.

Provide vessel with ten (10) additional adult P.F.D.s. Vessel may only carry a total of forty-five (45) persons on board until this requirement issatisfied.

Properly wire galley reefer compressor.

Provide vessel with current nautical publications in accordance with 46 CFR185.20-35.

Provide vessel with current regulations for vessel type.

Provide dive air compressors with rotating machinery guards/covers.

Repair junction box under bridge console and provide cover for compressor reset.

Properly support electrical wiring throughout the vessel in accordance withIEEE-45.

Provide one additional adult life preserver prior to the carriage of more than 54 total persons.
