Vessel is replacing portable bilge pump in the lazarette. Replace prior to credit DDX/ISE.

Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space including fuel tanks, and may require the vessel to be drydocked or taken out of service to assess the extent of the damage, and to affect permanent repairs. Vessel ran aground with unknown damage to underwater hull, appendages, shafts, and rudders. Vessel is required to be taken out of service until a satisfactory damage assessment is conducted by the attending Marine Inspector. 46 CFR 176.600 (d)

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Vessel experienced a failure of the auto-pilot system and subsequent failure causing the vessel to run aground. Effect repairs, provide root cause analysis, and conduct a successful sea trial satisfactory to the attending Marine Inspector prior to the carriage of passengers. 46 CFR 176.840

The marine inspector may require any part or all of the propeller shafting to be drawn for examination of the shafting and stern bearing of a vessel whenever the condition of the shafting and bearings are in question. Vessel ran aground and sustained damage to both port and starboard propellers. Observed missing water lubricated bearing on port shaft strut, with visible damage to propeller shaft. Additionally, both starboard water lubricated bearings shifted from installed position. Both shafts are required to be drawn for inspection, and the replacement of 03 water lubricated bearings is required to satisfaction of the attending Marine Inspector. 46 CFR 176.670 (a)

All vessels except non-self-propelled vessels shall be provided with suitable steering apparatus. Vessel ran aground and sustained damage to starboard rudder, bending the rudder. Submit repair proposal to satisfaction of the attending Marine Inspector. 46 CFR 182.30-1 (OLD - T)

A vessel on an oceans route must be equipped with an electronic position fixing device, capable of providing accurate fixes for the area in which the vessel operates, to the satisfaction of the cognizant OCMI. Vessel reported loss of GPS system and subsequently ran aground. Effect repairs, provide root cause analysis, and conduct a successful sea trial satisfactory to the attending Marine Inspector prior to the carriage of passengers. 46 CFR 184.410

The following vessels must have on board a properly installed, operational Coast Guard type-approved AIS Class A device:(i) A self-propelled vessel of 65 feet or more in length, engaged in commercial service. Vessel reported inoperable AIS. 33 CFR 164.46

Exhaust gas must not leak from the piping or any connections. MIs observed exhaust gas leak from the STBD Side Generator Wet Exhaust in the lazarette space. Submit a repair plan and repair the exhaust leak within 7 days. 46 CFR 182.430 (b)

An interconnected detection and alarm system must: Consist of multiple-station smoke detectors listed by an NRTL, or independent laboratory accepted by the Commandant according to 46 CFR 159.010, as meeting UL 217. Vessel's interconnected detection and alarm system is not certified to UL 217 standard. Owner must submit proposal within 30 days. 46 CFR 181.450 (a)(1)

A battery charger intended for connection to a commericial supply voltage... an ammeter that is readily visible shall be included in the battery charger circuit. Attending inspector noticed no ammeter at batteery charger in engine room. 46CFR 183.05-20 OLD T

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. Vessel's first aid kit on board was expired.

Suitable covers, guards, or rails shall be installed in way of all exposed and hazardous places such as gears, machinery, etc. Vessel had an exposed belt on starboard main engine.

Each life jacket carried on a vessel on oceans or coastwise route, must have a life jacket light. Each life jacket light must be securely attached to the front shoulder area of the life jacket. 46 CFR 180.75(a) During inspection found 18 adult life jackets and 04 Child life jackets with required light inoperable. Restricting vessel to a Limited Coastwise route. Restricted from operating more than 20nm from a harbor of safe refuge.

Each ring life buoy must be marked in clearly legible block capital letters with the vessels name and must be marked with Type II retroreflective material. Ring buoy name markings are faded and there is no retroreflective tape on one of the ring buoys.

A marine employer shall ensure that alcohol testing of each individual involved in a Serious Marine Incident is conducted within 2 hours of the incident occurrence. No unexpired alcohol swabs are onboard.

A self-propelled vessel of 65 feet or more in length engaged in commercial service must have on board a properly installed operational Coast Guard type approved Class A AIS. Vessel AIS is reported as inoperable and is off the vessel for repair.

ALL CABLES AND WIRES MUST FOLLOW STANDARDS SET FORTH IN 46 CFR 183.340. DISCOVERED WIRING FOR POTABLE PRESSURE SWITCH/PUMPS TO BE SOLID CORE WIRING RATHER THAN A STRANDED CORE SET FORTH IN 46 CFR 183.340. 46 CFR 183.340

THE CONSTRUCTION AND ARRANGEMENT OF A VESSEL MUST ALLOW THE SAFE OPERATING OF THE VESSEL IN ACCORDANCE WITH THE TERMS OF ITS CERTIFICATE OF INSPECTION GIVING CONSIDERATION TO PROVISIONS FOR AN UNEXPECTED CASUALTY. DISCOVERED THE VESSEL'S AIR COMPRESSOR TO BE LOOSE AND NOT SECURED CREATING A SAFETY HAZARD. 46 CFR 177.100

ALL DEAD END WIRES SHALL BE PROPERLY CAPPED OFF AND PLACED IN A JUNCTION BOX OR SIMILIAR TO ONE TO PREVENT A POTENTIAL FIRE HAZARD. DISCOVERED PORTIONS OF DEAD END WIRES LOCATED IN THE LAZARETTE SPACE. 46 CFR 183.340

AT EACH INSPECTION FOR CERTIFICATION AND AT EVERY OTHER VESSEL INSPECTION ALL OBSERVED UNSAFE PRACTICES, FIRE HAZARDS, AND OTHER HAZARDOUS SITUATIONS MUST BE CORRECTED AL REQUIRED GUARDS AND PROTECTIVE DEVICES MUST BE IN SATISFACTORY CONDITION. DISCOVERED MISSING GUARDS ON THE MAIN ENGINE/STBD GENERATOR. 46 CFR 176.830 (a)

ON A VESSEL MMORE THAN 65 FEET IN LENGTH, EACH HOSE MUST READILY ASSESIBLE, COMPLY WITH UL19 STANDARDS, BE 50 FEET IN LENGTH/1.5 INCHES IN DIAMTER, HAVE FITTINGS OF BRASS OR OTHER SUITABLE CORROSION-RESISTANT MATERIAL COMPLIED IN NFPA 1963 AND BE FITTED WITH A SPANNER WRENCH AT EACH FIRE HYDRANT/STATION. DISCOVERED THE VESSEL'S FIRE HYDRANT/STATION HAD A QUICK-CONNECT CONNECTION FOR ITS FIRE HOSE WITH NO SPANNER WRENCH AT THE STATION. 46 CFR 181.320

LIFE FLOATS MUST NOT BE STOWED IN TIERS MORE THAN 4 FEET HIGH. WHILE STOWED CORRECTLY IN TIERS, THE SEPARATE UNITS MUST BE KEPT APART BY SPACERS. DISCOVERED THAT THE VESSEL'S LIFE FLOATS WERE NOT SEPARATED BY SPACERS. 46 CFR 180.130 (C)(2)

Replace ringbuoy on main deck

Vessel is limited to a maximum of 150 passengers and 3 crew while on a limited coastwise and lesser routes in warm waters.

STBD nav identified cracked where white light would show through. Repair or replace lense or fixture. ISW standards lister in 33CFR.

Complete permanent repairs to the port side chine in the collision bulkhead including weldment layup and chine below the water line to the unathhorized clad weld patch. Submit a repair proposal prior to commencing any work to the cognizent OCMI via MSD Coram

Crop out and renew in way of old transducer in potable water tank void where carriage bolt was used as a plug. STBD side midships

Complete permanent repairs to the hull plating in way of intregral fuel tanks are, where doubler plates where installed with out USCG knowledge.

Complete permanent repairs to the box keel where doubler plate was installed on the PORT side.

Identified doubler plates in way of intregal fuel tanks on the hull shell plating. Issued requirement to prove that this repair was USCG approved.

Issued requirement to make void 2 available for inspection

Identified excessive clad welding and poultice corrosion in the vessel's shell plating beneath the MSD tanks in the engine room. Issued requirement to effect permanent repairs to the hull plating beneath the MSD tanks

Replace port side fire hose on first deck due to holes.

O/O to provide video survey of running gear prior to carriage of passengers

Clean up frames IVO starboard and port side sewage tank and submit repair proposal.

Provide repair proposal for pitting to inboard side of port engine frame, just forward of the engine.

Make repairs to cracks due to weather damage on the stbd aft railing, vent pipe and stanchion.

Ensure all portable fire extinguishers are mounted in proper locations, with brackets prior to carrying passengers.

Have all portable extinguishers serviced and tagged prior to carrying passengers.

Ensure lifejacket stowage boxes are properly labeled prior to carriage of passengers.

Properly label emergency fuel shutoff valve.

Properly label bildge manifold in engineroom.

Ensure emergency escape hatch is operable from inside engineroom.

Post appropriate lifejacket doning placards where they are visible to passengers.

Post all necessary documents and placards throughout the vessel as discussed.

Label all lifefloat paddles with vessels name, and secure to lifefloats.

Ensure the general cleanliness of vessel and secure all loose misle and trip hazards prior to carrying passengers.

Vessel is limited to no more than 20NM from shore until another VHF or MF radio are operable onboard.

CROP AND RENEW TO GOOD METAL COLLISION BULKHEAD WHERE CRACKED ON PORTSIDE IN WAY OF CHINE.

MAKE PROPER REPAIRS TO BATTERY BOX IN ENGINE ROOM STARBOARD SIDE.

MAKE PROPER REPAIRS AT CRACKS IN COLLISION BULKHEAD BOTH PORT AND STARBOARDSIDE IN WAY OF CHINES.

REPAIR CRACKED WELD IN NUMBER THREE COMPARTMENT AT HEAD OF FOURTH FRAME STARBOARD SIDE IN WAY OF SIDE SHELL.
