Ships shall automatically transmit the identity, position, and date/time of position via LRIT and automatically provide and receive the ship’s identity, type, positions, course, speed, navigational status and exchange information with shore stations and other vessels via AIS. The vessel name and location do not accurately display. This issue has been ongoing since at least 2018 with reports that the vessel’s MMSI has been spoofed by unknown party. USCG NAVCEN recommends contacting Pole Star to resolve. If additional time is needed, contact OCMI for extension. 74 SOLAS (20) V/19-1.5 74 SOLAS (20) V/19.2.4.5

Survival craft, its launching appliances, and the area of water into which it is to be launched shall be adequately illuminated by lighting supplied from the emergency sources of electrical power. Lighting at the forward port liferaft is inadequate. 74 SOLAS (97) III/11.4

Doors fitted in boundary bulkheads of machinery spaces pf category A shall be reasonably gastight and self-closing. The A-60 boundary door to the ‘incinerator room’ is not self-closing and fails to latch. 74 SOLAS (97) II-2/47.1

Adequate lighting shall be provided to illuminate the transfer arrangements overside and the position on deck where a person embarks or disembarks. The lighting at the port and starboard pilot embarkation stations is inadequate. 74 SOLAS (20) V/23.8

Safe and convenient access to and egress from the ship shall be provided by a single length of pilot ladder capable of reaching the water from the point of access to, or egress from, the ship and due allowance is made for all conditions of loading and trim. The port forward pilot ladder is approximately 4 meters above the waterline at present loading condition. 74 SOLAS (20) V/23.3.3

The drencher system used as the means for effectively cooling the designated under-deck cargo space for the carriage of dangerous goods is inoperable. The connection hose assemblies are unable to be connected to deck fittings or cargo hold fittings as quick connect caps on those fittings are unable to be removed. Vessel is prohibited from carrying dangerous goods until proper operation is demonstrated. 74 SOLAS (20) II-2/14.2.1

A vessel at anchor, of more than 100 meters, shall sound a bell in the forepart of the vessel to give warning of her position and of the possibility of collision to an approaching vessel. Bell forward has no clapper and cannot make sound. COLREGS Rule 35(g)

After conclusion of annual servicing in accordance with Resolution MSC.402(96), RO and USCG attended for operational testing of rescue boat davit. The required hand gear provided for recovery of the rescue boat failed to recover the rescue boat. 74 SOLAS (20) III/20.1

The safety management system should ensure compliance with mandatory rules and regulations and applicable codes, guidelines, and standards. Objective evidence indicates a potential failure of the SMS on board the vessel as documented by the below deficiencies. An external ISM audit is required. 74 SOLAS (20) IX/5 ISM Code 1.2.3

The company should ensure that inspections are held at appropriate intervals, that appropriate corrective is taken, and records are maintained. The company conducted an internal "habitability inspection" in March 2020 and an internal audit in October 2020, both of which resulted identification of deficiencies or internal non-conformities. In addition, onboard interviews indicate that the main engine automation and alarm functionality has been an ongoing problem that has become progressively worse since February 2020. Finally, both SSDGs have had ongoing leaks fuel leaks for 2+ years as identified in multiple flag State inspections and RO surveys. Upon further review of Company SMS records, there was no objective evidence that the Company has implemented corrective actions effectively. 74 SOLAS (20) IX/5 ISM Code 10.2

The ship shall be maintained in compliance with the structural requirements of a classification society. The tank tops of WBT #7 and WBT #8 abutting the # 3 cargo hold were gauged and found to over the maximum allowable wastage of 20% in accordance with DNV-GL Rules and NVIC 7-68. 74 SOLAS (20) II-1/3-1 DNVGL-CG-0182 4.1.4 Table 2 (Allowable thickness diminution for hull structure)

The ship and its equipment shall be maintained so it will remain fit to proceed to sea without danger to the ship or persons on board. Unapproved epoxy has been installed by shipyard as repair in lieu of steelwork on hydraulic fluid tank for rescue boat davit and holed bulkhead in the port bunker station which penetrates the CO2 room Class A fire boundary. Make permanent steel repairs. 74 SOLAS (20) I/11(a)

Means shall be provided to control the leak of flammable liquids. Both SSDGs have ongoing fuel leaks from overflow line with pooling in containment. Repair leaks and clean containment as ongoing fuel leaks presents fire hazard. 74 SOLAS (04) II-2/4.1.1

A main electric lighting system shall provide illumination throughout those parts of the ship normally accessible to an used by crew and the emergency source of electrical power shall be capable of supplying emergency lighting for 18 hours. Lights on deck and throughoutship, supplied by both main and/or emergency electrical power, are inoperable or have damaged covers. 74 SOLAS (04) II-1/41.2.1 74 SOLAS (04) II-1/43.2.2

Means shall be provided to control the leak of flammable liquids. Complete following outstanding items from internal audit conducted October 2020: replace oil soaked lagging in way of the OWS in engine room and renew seal/gasket on MDE #2 cylinder. Work orders are in NS5. 74 SOLAS (04) II-1/4.1.1

Before the ship leaves port and at all times during the voyage all lifesaving appliances shall be in working order and ready for immediate use. Vessel will provide a repair plan proposal to address the steel crack on the free fall davit as id by the port engineer and lifeboat hydraulic hose must be replaced.

The owner or managing operator shall make a vessel available for drydock examination & underwater surveys. Vessel will repair ballast tank ladders as id for appropriate access.

It was reported that free fall lifeboat has fiberglass crack IWO P.S Gunnel 2' above waterline and approximately 12" in length. Repair to OEM condition to the satisfaction of RO. SOLAS 74a(14) III/20.2 50a

The ships automation system had a fault which caused both generators to secure, interfering with propulsion and leaving the ship adrift while troubleshooting. The arrangements provided shall be such as to ensure that the safety of the ship in all sailing conditions, including maneuvering, is equivalent to that of a ship having the machinery spaces manned. Increase engine room manning in order to compensate for the unknown reliability of the automation system and allow for human control of the plant while transiting in the Sector Puget Sound Captain of the Port Zone. Provide a report by the classification society detailing the cause of the failure and attesting to the proper operation of the vessel’s automation system

Fire protection equipment shall be kept in good working order and available for immediate use. The #1 cargo hold cooling water spray system automatic control valve was found inoperable during testing with the attending DNV-GL surveyor. 74 SOLAS (97) II-2/39.1 & 74 SOLAS (14) II-2/14.2.1.2

Before the ship leaves port and at all times during the voyage, all life saving appliances shall be in working order and ready for immediate use. The rescue boat davit is not currently operational. 74 SOLAS (14) III/20.2

#2 SSDG Thermostat for the CW system found to be inoperable. Repair and test SSDG #2 to the satisfaction of the RO prior to departure.

Vessel reported that the ventilation dampers that supply air for the emergency generator are inoperable. Effect Repairs and provide class concurrence of adequacy.

Vessel reported 2 losses of propulstion, within 48 hours of each other on the 11th and 13th, due to loss of lube oil pressure.

The arrangements of sea connections, fire pumps, and their source of power shall be as to ensure that if a fire in any one compartment could put all the pumps out of action, there shall be an alternate means consisting of an emergency fire pump. The emergency fire pump was not able to provide consistent water pressure through two hose and was not able to maintain prime.

The minimum fire integrity of bulkheads and decks shall be prescribed in Table 44.1 and 44.2. The penetration in the overhead of the 05 deck fan room is utilizing a rubber hose for a condensate drain to penetrate the deck. This installation does not provide protection as outlined in the tables.

Electrical installations shall be such that the safety of passengers, crew, and ship from electrical hazards will be ensured. The incinerator has been removed from the vessel and the wires that supplied power to the unit have not been removed or properly terminated.

After any survey of the ship under regulation 8 has been completed no change shall be made in the structural arrangements, machinery, equipment and other items covered by the survey, without the sanction of the administration. The starboard side self-closing device to the forward portable berthing is not operational.

The main inlets and outlets of all ventilations systems shall be capable of being closed from outside the space being ventilated. The ventilation closures for the portable berthing spaces are not operational.

Where air pipes to tanks extend above the freeboard deck or superstructure decks a satisfactory means shall be provided for closing the openings of air pipes. The closure device for the air pipe, located on the 05 deck starboard side, is deteriorated due to corrosion; the closure must be replaced.

The machinery and associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended. The above deck piping for fire hydrant #18 is heavily scaled and the flange fasteners are severely wasted.

After any survey of the ship under regulations 7, 8, 9, or 10, has been completed, no change shall be made in the structural arrangements, machinery, equipment and other items covered by the survey, without the sanction of the Administration. Upon arrival to the vessel, a hull insert was being fitted for welding in the port side of the hull IWO the ECR space within the engine room. No notification was made to USCG of hull repairs being made from Class.

The manning requirements for a particular vessel are determined by the OCMI after consultation of the applicable laws, and all other factors involved. Due to class withdrawing the AUT notation (automation), and recommending a fully manned engine room, 1 licensed engineer will be required in the e/r at all times.

Means shall be provided whereby normal operation of propulsion machinery can be sustained or restored even though one of the essential auxiliaries becomes inoperative. The #1 LT cooling water pump motor is inoperative. Repair/replace and prove proper operation of the #1 LT pump/motor prior to departing Aqaba, Jordan. 74 SOLAS (04) II-1/26.3

A general emergency alarm system complying with FSS Code 7.2.1, shall be provided and shall be used for summoning crew to muster stations and to initiate the actions included in the muster list. During operational testing, no sound was emitted from several locations rendering multiple alarm stations on deck and at accommodations inoperable. Repair and prove proper operation. 74 SOLAS (14) III/6.4.2.

Fire detection and alarm system shall be kept in good working order and be readily available for immediate use. The fire detection system is installed and designed so that an unacknowledged alarm sounds the General Alarm after two minutes. During operational testing, the General Alarm did not sound. Repair and prove proper operation. 74 SOLAS (14) II-2 14.2.1.1.2.

Fire detection and alarm systems shall be kept in good working order and be readily available for immediate use. The smoke detector above steering gear is inoperable and is looped out of the system, preventing the alarm to be raised in the event of a fire in the area. Repair smoke detector, reconnect loop to detection system, and prove proper operation. 74 SOLAS (14) II-2 14.2.1.1.2

All life-saving appliances shall be in working order and ready for immediate use. The rescue boat davit has a hand-operated hydraulic pump for manual starting during emergency use. The crew is unfamiliar with its installation and operation. No maintenance is being conducted on the pump and the cheat bar is rusted in place. Prove proper operation. 74 SOLAS (14) III/20.2

The design, materials, and construction of all watertight doors shall be to the satisfaction of the administration. The watertight door to the CO2 room on the main deck fails to close completely and there is at least a ½” gap between the door frame and the door at the bottom right corner. Repair and prove closure. 74 SOLAS (14) II-1/16.1.

To enable pipes of reception facilities to be connected with the ship's discharge pipeline for residues from machinery bilges and from oil residue (sludge) tanks, both lines shall be fitted with a standard discharge connection in accordance with the standard dimensions of flanges for discharge connections table. Sludge standard discharge connection. The standard discharge connection is missing from the port side bunker station and is blanked off. The standard discharge connection has been replaced with a quick connecting coupling on the starboard side. Remove unapproved installations. Install standard discharge connections prior to next discharge. 73/78 MARPOL Annex I/13

Electrical services essential for safety will be ensured under various emergency conditions. The emergency light at the port bunker station has a spotlight connected above the bunker station and the spotlight is filled with water. Remove unapproved installed of spotlight from emergency circuit. 74 SOLAS (04) II-1/40.1.2

Ships shall carry liferafts complying with the requirements of section 4.2 or 4.3 of the LSA Code which requires marking of each liferaft with the name and port of registry of the ship to which it is fitted. The ship carries five liferafts and none are marked with the ship’s name or hailing port. Mark all liferafts with name and hailing port. 74 SOLAS (14) III/31.1.1.2, LSA Code 4.2.7.2

The condition of the ship and its equipment shall be maintained to ensure the ship in all respects will remain fit to proceed to sea without danger to the ship. Containment for port side bunker station has standing water approximately 2” deep. There is a drain in the center deck of containment that is uncovered but not draining. Identify where drain leads, clear drain. Drain in starboard side bunker containment appears to have been welded closed. Identify where drain leads, clear drain prior to using the port bunker station.

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. A failure of the vessel's SMS has been identified as a result of deficiencies issued by USCG and RO-issued PR-17. Specifically, the bilge pump has been inoperable since before last drydock in 2017, with no scheduled maintenance listed in NS5. An external audit is required, which may be of a scope limited to the deficiencies, and related to planned maintenance of equipment, identified during this inspection.

An efficient bilge pumping system shall be provided, capable of pumping from any watertight compartment. The bilge pump in the engine room is inoperable due to inability to gain prime. A diaphragm pump has been installed beside the bilge pump but is not indicated on ships’ drawings nor could the ship provide RO approval for the modifications. Remove temporary installation of diaphragm pump. Repair bilge pump and prove proper operation. 74 SOLAS (04) II-1/21

The condition of the ship and its equipment shall be maintained to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Oil soaked lagging is present in the engine room iwo the lube oil purifier steam piping and valve (approx. 3 feet) and at the heavy fuel oil purifier steam piping and valve (approx.. 4 feet) creating a fire hazard in the engine room. Remove oil soaked lagging, identify source of oil, repair/replace defective parts if necessary, and replace lagging. 74 SOLAS (14) I/11(a)

Where stand-by machines are required for other auxiliary machinery essential to propulsion, automatic change-over devices shall be provided. The #1 MDE lube oil pump motor has damaged windings and technician reports show that one phase runs hot and there is significant vibration. In addition, electrical components are loose and/or corroded. Repair/replace and prove proper operation. 74 SOLAS (04) II-1/53.3

Each emergency generator set arranged to be automatically started shall be equipped with starting devices. A second source of energy shall be provided. The second set of batteries (#2) failed to start the EDG. Replace batteries.

Fire-fighting systems shall be kept in good working order and readily available for immediate use. The water mist system over the main engine lube oil purifier was inoperable during operational testing as no water was available from the nozzles. Identify cause and prove proper operation.

Where stand-by machines are required for other auxiliary machinery essential to propulsion, automatic change-over devices shall be provided. One of two Boiler Feed pumps are inoperable due to a broken coupling and shaft. Repair and prove proper operation and automatic change-over.

Cargo ships shall carry at least one rescue boat. The rescue boat has been sent ashore for FRP repairs. Return rescue boat to vessel.

There are multiple temporary repairs to the Black Water piping in the engine room. During attendance, the smell of sewage was strong iwo the landing on the ladder well between the entrance to the engine room and ECR. Identify new leak(s) or failed temporary installations. Repair.

The machinery and associated piping systems shall be of a design and construction adequate for the service for which they are intended. During operational testing, the seal for the Diesel Oil Supply Pump #2 failed. Replace seal.

The machinery and associated piping systems shall be of a design and construction adequate for the service for which they are intended. The float switch on the sludge tank for the Lube Oil purifier is held together by a ziptie, has an oil soaked rag wrapped around the base where it enters the tank, and electrical tape wrapped around wiring to hold it in place. Repair or replace.

Fire-fighting systems shall be kept in good working order and readily available for use. The cargo hold smoke extraction system indicates a "Smoke Detector Failure" on the Bridge Emergency Alarm Panel but no fault is indicated at the main control panel for the system in the CO2 room.

Every ship shall be provided with a main steering gear and an auxiliary steering gear. Steering gear pump #2 (emergency steering) indicated a Phase Failure alarm that was unable to be cleared. Upon troubleshooting, C/E discovered that the relay for the #2 was different from the #1 and that there were no manufacturer's instructions on board. Repair and demonstrate proper operations.

Cargo ships of 1,000 GT and upwards shall have at least least two independently driven fire pumps and shall be kept in good working order and readily available for use. The fire pump feeds the eductor for the cargo hold bilge suction and was inoperable during testing of the bilge suction system. The fire pump started after troubleshooting but was not operating as designed. Identify cause, repair, and prove proper operation.

An efficient bilge pumping system shall be provided, capable of pumping from and draining any watertight compartment. During operational testing, the bilge pumping system was unable to be operated and failed to gain suction to drain Cargo Hold #3. Identify cause, repair, and demonstrate ability to drain cargo hold.

Where the emergency source of power is a generator, it shall be driven by a suitable prime mover and be started automatically upon failure of the main source of power. The starter relay for the prime mover is not operational and the engine will not start automatically as required.

Main propulsion machinery shall be able to be controlled locally. The local control station for the main engine had multiple extinguished lights, a missing pushbutton, and clear tape holding multiple pushbuttons in place. Repair.

The machinery and associated piping and fittings shall be of a design and construction adequate for the service for which they are intended. The valve labeled "LO to System" is leaking, located at the stbd fwd corned of the main engine on the lowest level of the engine room. MIs witnessed it actively dripping and fresh lube oil is present in the bilges directly below. Repair or replace.

The machinery, associated piping systems and fittings shall be of design and construction adequate for the service for which it is intended. The design shall have regard to the materials used in construction, the purpose for which the equipment is intended, the working conditions to which it will be subjected and the environmental conditions on board. The wiring to the turbo charger, L/O temperature sensor, on the #1 SSDG is broken and the equipment is not operating as designed.

The ship shall have a properly adjusted magnetic compass, or other means independent of any power supply, to determine the ship's heading and display the reading at the main steering position. The ship's magnetic compass is out of calibration by approximately 10 degrees as indicated by the deviation card.

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. The search and rescue locating devices were unable to be demonstrated during operational testing by the Captain and 2/O. Prove proper operation of both devices.

Means shall be provided to control leaks of flammable liquids. The main engine has excess oil in multiple places including: standing pools of oil between the #1 and #2 cylinders, the #4 and #5 cylinders, a leaking fuel injector on cylinder #5, and standing oil on top of the scavenge air box. Clean excess oil and identify source(s).

The main steering gear shall be provided with control in the steering gear compartment. The local steering station is leaking hydraulic fluid. In addition, hydraulic fluid is present in multiple places on the steering gear. Identify leaks and repair.

Fire-fighting systems and appliances shall be kept in good working order. A valve shall be fitted to serve each fire hydrant so that any fire hose may be removed while the fire pumps are in operation. Fire hydrants #2 and #25 were not able to be isolated completely. Repair.

The machinery and associated piping and fittings shall be installed to reduce to a minimum any danger to persons on board. The containment for the purifiers in the purifier space is leaking where the containment meets the deck and water and oil is running onto the deck creating a slip hazard. It appears a weld has failed and caulking installed and painted to control leakage from containment. Repair.

Electrical installations shall ensure the safety of the crew and ship. Identify system and function red box on port bridge wing.

All electrical apparatus shall be so constructed and so installed as not to cause injury when touched in the normal manner. The light fixture in the #5 cargo hold aft access trunk is disassembled and lying on the deck with dead end wires mad off to the ladder.

All electrical apparatus shall be so constructed and so installed as not to cause injury when touched in the normal manner. Two lights are missing covers and one does not work in the void spaces directly above the emergency fire pump.

The machinery and associated piping and fittings shall be of a design and construction adequate for the service for which they are intended. The heat exchanger (plate type) at the purifier in the purifier room is leaking where the steam pipe enters at the top corner. Repair.

The machinery and associated piping and fittings shall be of a design and construction adequate for the service for which they are intended. The heat exchanger (plate type) on the lowest level in the engine room, stbd fwd corner by the fire pump is leaking from a valve on the "SW IN" line. Repair.

All electrical apparatus shall be so constructed and so installed as not to cause injury when toucehed in the normal manner. The light switch in the doorway by the entrance to the engine room is held on by duct tape.

Oil fuel pipes, which if damaged, would allow oil to escape from a storage, settling, or daily service tank having a capacity of 500l and above situated above the double bottom, shall be fitted with a valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of a fire occurring in the space in which such tanks are situated. Hydraulic actuator for the HFO tank to the Booster pumps is leaking hydraulic oil into containment and the valve is covered in oil soaked lagging. Remove oil soaked lagging, repair actuator, and replace lagging. Remove hydraulic oil from containment.

Where the emergency source of electrical power is a generator, it shall be driven by a suitable prime mover and be started automatically upon failure of the main source of power. The Emergency Diesel Generator (EDG) is inoperable due to a broken piston connecting rod and resulting damage to the cylinder liner and crankshaft requiring the EDG prime mover be rebuilt. Prove satisfactory operation and testing.

A main source of electrical power of sufficient capacity shall consist of at least two generating sets. Electrical power is supplied by two ships service diesel generators (SSDG). Both SSDGs have excessive oil leaks, including from the main seal, that requires ship’s crew to add 30-50 liters of oil to one of the prime movers per day according to the Engineer’s Noon Log. This issue has been persistent over a period of time and has been identified through multiple deficiencies issued by the USCG since September 2018. A deficiency was issued on 06Nov18 to provide a repair proposal for both generating set prime movers and vessel failed to do so. SSDG #1 is in the process of being overhauled and as such, is inoperable. Complete overhaul of SSDG #1 and prove satisfactory operation and testing.

A main source of electrical power of sufficient capacity shall consist of at least two generating sets. Electrical power is supplied by two ships service diesel generators (SSDG). Both SSDGs have excessive oil leaks, including from the main seal, that requires ship’s crew to add 30-50 liters of oil to one of the prime movers per day according to the Engineer’s Noon Log. This issue has been persistent over a period of time and has been identified through multiple deficiencies issued by the USCG since September 2018. A deficiency was issued on 06Nov18 to provide a repair proposal for both generating set prime movers and vessel failed to do so. SSDG #2 is leaking a significant amount of oil, creating a fire hazard in the engine room as well as presenting the potential for the SSDG #2 to fail due to loss of oil. Complete repairs to remove excess oil leakage. Prove satisfactory operation and testing. Ensure required load capacity can be demonstrated.

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. The vessel is equipped with 5 liferafts. Four of 5 life rafts had hydrostatic release units (HRU) improperly maintained – two had incorrect expiry dates indicted (two years indicated) and two had the painter attached to the frame of the cradle instead of the weak link, meaning the the life raft would inflate but not separate from the ship at the designated depth of painter and would sink with the ship. Where “A” class divisions are penetrated, such penetrations shall be tested and shall be suitably insulated by extension of the insulation at the same level of division. Numerous penetrations thru “A” divisions were identified including: the wire way penetrating the deck in the A/C room is missing fire stop material; two PVC pipes penetrating the overhead “A” division in the A/C room; and, pipe penetration in the overhead “A” division of the ECR behind the switchboard that has been discontinued and has a rag stuffed inside. These penetrations render the “A” division unserviceable and allow the passage of heat and smoke negating the “A” division rating.

Amend PSTPs to accurately demonstrate the proper operation of automated equipment for: M/E 2.3, 4.0, 4.7, remote valve shut offs – properly identify valves actuated by levers 3, 5, & 6. Update PSTP Section 18 - Emergency shutoffs for Remote Oil Valves Shutoffs. Update to accurately reflect labeling, location, and operation of each valve and lever. The current configuration will not lower unassisted with no personnel in the rescue boat. With 360 pounds (2 persons) the boat will free spool down with no mechanical assistance. The steering gear compartment shall include gratings or other nonslip surfaces to ensure suitable working conditions in the event of a hydraulic fluid leak. There is only a small section of grating that is not adequate. Install nonslip surface.

Hydraulic power operated steering gear shall have a fixed storage tank having sufficient capacity to recharge at least one system. The fixed storage tank was below the sight class. After removing the filler screen, the resevoir is low to the point it does not have enough fluid to fill one system.

All windows seperating accomodations, service spaces, and control stations from the weather shall be constructed with frames of steel or other suitable material. The glass shall be retained by a metal glazing bead of angle. The frame around the starboard forward bridge window is deteriorated and the fasteners are sheared. Make repairs as necessary.

The vessel's NS5 maintenance program does not accurately reflect the equipment onboard and has numerous overdue items. Maintenance is instead being tracked on spreadsheets developed by the previous Chief Engineer. The vessel must demonstrate to the satisfaction of the attending Coast Guard Marine Inspector that the NS5 is completely accurate and up-to-date and that the Chief Engineer can competently maintain the system, prior to obtaining authorization to operate a Periodically Unattended Machinery Space (PUMS).

CH I/REG 11

Rescue Boat release hook is not USCG Approved. The original hook from re-flag, was deemed unserviceable due to over 5 yrs old. Replacement hook is different type and only Class approved.

HFO Tanks 32/33 to remain closed until remote shut off valves operate properly. Class surveyour may witness test in order to clear def.

PTSP's for MDE to be completed and certified complete by CHENG prior to vessel assuming PUMS status. MDE testing to be done u/w IAW PTSPs. 1 Lic Engineer req per watch until complete.

Resubmit updated PSTPs from previous revision D 7/2017 1.2, to MSC for approval. Specific line items for review: 10.1, 10.4.2, 10.11, 12.0

New CG-835 issued due to MSC letters E2-1300528 and E2-1202729 requiring PSTPs to be witnessed by a cognizant OCMI or ACS surveyor. Following appropriate testing, any changes must be resubmitted for final approval to MSC. Completion is due at next COI renewal/drydock period, which ever comes first. See MSC Letters in Documents.

Lower boat, test and run in the water IAW Life saving code and SOLAS.

Provide an approved BWM plan with correct company name.

The compartment aboard the rescue boat containing rescue boat equipment (flash light, knife, heaving line, etc) was damaged. The weather tight integrity was compromised and some of the required equipment was wet. Replace in kind or repair. SOLAS (2014) III/20.11.2.1

#2 SSDG fell of line while transferring power from #1 to #2 SSDG.

Complete repair of #6 cylinder on main engine and all affected components before departing port of Shuaiba, Kuwait. 835 extended due to lack of resources in Kuwait. Vessel will make permanent repairs in Abu Dhabi.

While making temp repairs to pin hole leak in fire pump casing, damage was sustained to mechanical pump seal causing excessive water intrusion when pump is running. Repair/replace damaged seal to the satisfaction of Class and US Coast Guard.

Vessel's fire pump has a pin hole leak in the pump casing. Temp repair has been completed and pump was run providing adequate pressure to the hoses. Vessel is to make permanent repairs to the holed casing to the satisfaction of Class and US Coast Guard.

Vessel was unable to start emergency diesel generator (EDG) due to insufficient starting source. Demonstrate proper operation to the attending Coast Guard marine inspector.

Vessel was unable to demonstrate emergency fire pump due to sea chest being out of the water. Vessel is to demonstrate proper operation of emergency fire pump to the satisfaction of the attending marine inspector.
