The following locations in the weather are Class I, Division 1 (Zone 1) locations and may have only approved intrinsically safe, explosionproof, or purged and pressurized electrical equipment, and through runs of marine shipboard cable if the location is: Within 5 meters (16 ft) of cargo pressure/vacuum valves with an unlimited height. Vessel's lights and A/C unit aft of cargo tank #5 are not intrinsically safe/explosionproof. Vessel is prohibited from carrying flammable or combustible cargo that has a flashpoint below 140°F. 46 CFR 111.105-31(m)(3)

Piping systems used where the fluid temperature exceeds 150 °F must be suitably insulated as necessary to preclude injury to personnel. Vessel's exhaust piping does not have insulation. 46 CFR 56.50-1(k)

The requirements of § 58.10-5 (a), (c), and (d) apply to diesel engine installations. Horizontal dry exhaust pipes are permitted only if they are so arranged as to prevent entry of cold water from rough or boarding seas. The starboard generator did not have a cover on the exhaust pipe. The starboard exhaust pipe for the prime mover of the cargo pump was corroded, leaving a large hole on the pipe. The port exhaust pipe for the prime mover of the cargo pump has a exhaust flap that stays open. These arrangements allow ingress of water in both exhaust pipes. Repair to the satisfaction of the USCG Marine Inspector. 46 CFR 58.10-10(a) 46 CFR 58.10-5(d)(ii)

An electric apparatus must be designed, constructed, and installed to prevent any person from accidentally contacting energized parts. Engine room breaker panel was missing a plate cover, exposing wiring. Bolt on the back of the engine room breaker panel was missing causing the panel to loosen and expose electrical wiring. Repair to the satisfaction of the USCG Marine Inspector. 46 CFR 111.05-3(a)

Under or around each loading manifold and each transfer connection point, a fixed container or enclosed deck area that meets with requirements of 33 CFR 155.310. Multiple holes were found in the port and starboard manifold containment. Repair to the satisfaction of the USCG Marine Inspector. 33 CFR 155.310(a)(1)

Each nameplate for a circuit breaker must show the electrical load served and the setting of the circuit breaker. The labeling on the breaker panel inside the house did not correctly show the electrical load it served. Label to the satisfaction of the USCG Marine Inspector. 46 CFR 111.30-15(b)

Each device must have a nameplate showing the device's function. There was no labeling on the switches/controls on the port side of the house, showing what function it served. Label to the satisfaction of the USCG Marine Inspector. 46 CFR 111.30-15(a)

Sidelights mean a green light on the starboard side and a red light on the port side each showing an unbroken light over an arc of the horizon of 112.5 degrees and so fixed as to show the light from right ahead to 22.5 degrees abaft the beam on its respective side. Port navigation light was inoperable. Repair to the satisfaction of the USCG Marine Inspector. 46 CFR 83.21(b)

Remote control for the means of stopping machinery driving forced and induced draft fans, fuel oil transfer pumps, fuel oil unit pumps, and fans in the ventilation systems serving machinery and cargo spaces shall be tested at each regular inspection for certification and periodic inspection. Generator providing power to fuel oil pumps and machinery space ventilation had no means of remote stopping. Install ESD to the satisfaction of the USCG marine inspector. 46 CFR 61.20-3(b)

A sign shall be displayed to warn persons approaching the gangway... The sign shall state in letters not less than 2 inches high substantially as follows. Sign posted by gangway was weathered and illegible. Re-letter sign to the satisfaction of the USCG marine inspector. 46 CFR 35.30-1(b)

Barges described in paragraph (h) of this Rule shall carry two unobstructed all-round white lights. FWD all around light was bent causing it to be obstructed and was not illuminated. Stern light was also inoperable. Fix to the satisfaction of USCG marine inspector 33 CFR 83.30

The boundary bulkheads and decks separating the accommodations and control stations from cargo shall be of “A” Class Construction. Electrical gland on the house FWD bulkhead was packed with silicone. Repack gland to the satisfaction of USCG marine inspector. 46 CFR 32.57-10(c)

Each lifejacket must have a lifejacket light approved under approval series 161.112 securely attached to the front shoulder area of the lifejacket. 13 lifejackets were fitted with expired lights. Replace expired lights to the satisfaction of USCG marine inspector. 46 CFR 31.36 46 CFR 199.70 (b)(4)(i)

The boundary bulkheads and decks separating the accommodations and control stations from cargo shall be of “A” Class Construction. Escape hatch in the office space and ventilation hatch in galley was not sealed properly exposing daylight through the knife edge. Rectify to the satisfaction of USCG marine inspector. 46 CFR 32.57-10(c)

At least one-half the total number of lifebuoys on the vessel must each be fitted with a self-igniting light approved under approval series 161.010. AFT STBD lifebuoy light was fitted with incorrect batteries and was not operational. Rectify to the satisfaction of USCG marine inspector. 46 CFR 199.70 (a)(3)(ii)

If your vessel's Certificate of Inspection is renewed less often than annually, the marine inspector must check the settings of the safety or relief valves on all pressure vessels, except cargo tanks, twice every 5 years. No more than 3 years may elapse between any check and its immediate predecessor. UPV and compressor relief valve has not been tested due to the system not being able to provide enough pressure. Pressure relief valves need to be bench tested to the satisfaction of the USCG marine inspector. 46 CFR 61.10-5(i)(2)

Electric installations and electric equipment must be inspected at the inspection for certification and periodic inspection to determine mechanical and electrical condition and performance. Particular note must be made of circuits added or modified after the original issuance of the Certificate of Inspection. 03 ESD on port bulkhead of house was not labeled and it is unknown what they serve. Label to the satisfaction of the USCG marine inspector. 46 CFR 110.30-5

Electric cables constructed of stranded copper conductors, thermoplastic, elastomeric or other insulation, moisture-resistant jackets, and, where applicable, armoring and outer-sheathing must meet all the requirements of IEC 60092–350:2014, 60092–352:2005, 60092–353:2016, 60092–354:2014, 60092–360:2014, IEC/TR 60092–370:2009, 60092–376:2017, IEEE 1580, ANSI/UL 1072, ANSI/UL 1309, or MIL–DTL–24640C or MIL–DTL–24643C. Coast Guard Marine Inspectors observed wire sheathing entering into the blower stop and where the sheathing makes a 90 degree bend running from port to go amidships to be cracked or damaged, causing the internal wire sheathing to be exposed. Repair to the satisfaction of the attending Marine Inspector.

Each cable entering a box or fitting must be protected from abrasion and must meet the following: (a) Each opening through which a conductor enters must be closed. (b) Cable armor must be secured to the box or fitting. (c) Each cable entrance in a damp or wet location must b Coast Guard Marine Inspectors observed the insulated cap where the wire enters in the forward light and into a junction box backed out. Repair to the satisfaction of the attending Marine Inspector.

Each lifejacket must have a lifejacket light approved under approval series 161.112 securely attached to the front shoulder area of the lifejacket. Coast Guard Marine inspector noted 08 lifejackets with waterlights non operational. Repair to the satisfaction of the attending Marine Inspector.

A tank vessel with a capacity of 250 or more barrels that is carrying oil or hazardous material as cargo must have A mechanical means of closing each drain and scupper in the container or enclosed deck area required by this section. Coast Guard Marine inspector observed vessel was missing closure devices. Obtain closure devices to the satisfaction of the attending Marine Inspector.

Any tank vessel which is equipped with an internal combustion engine driven cargo pump on the weather deck shall be provided with a minimum of one remote manual shutdown station, conspicuously marked, and located at the midpoint of such vessel, or 100 feet from the engine, whichever is the more practical. The remote quick acting manual shutdown shall be installed on the engine so as to provide a quick and effective means of stopping the engine (such as by cutting off the intake air). Vessel's ESD was non operation. Vessel corrected on the spot.

All vessels except unmanned barges shall be provided with a satisfactory bilge pumping plant capable of pumping from and draining any watertight compartment except for ballast, oil and waters tanks which have acceptable means for filling and emptying independent of the bilge system. Bilge pump was not operational.

Each lifejacket must have a lifejacket light approved under approval series 161.112 securely attached to the front shoulder area of the jacket. 5 PFD lights were not operationals; replace.

In a vessel of 50m or more in length: (iv) a towing light, 3 miles. Tow light was dimmed and not visible; replace.

Hydraulic line for anchor system controls leaked at connection point on to deck upon testing.

Wiring and cords leading to loud hailer are secured with plastic zip-ties. Need to be metal.
