At each inspection for certification, the owner or operator must have vessel ready for inspection of fire protection equipment to include operation of the fire main system. Fire main pump had a steady drip (>1 drip per second unpressurized and valves closed) and a steady stream when operational from the packing and was not in fully operational condition upon inspection.

Rails must be 36 inches high. Rail courses shall be installed so that no open space exists more than 12 inches. Vessels did not have proper installation of rail courses for removable rails on aft deck. Vessel shall install proper railing prior to departing shipyard.

On a vessel of more than 65 feet in length, each hose must be lined commercial hose that conforms to UL 19 or hose that is listed and labeled by an independent laboratory recognized by the Commandant of being equivalent performance. While testing the vessels fire fighting system, the port side main deck fire was discovered to have a puncture approximately 15 mm in size. Vessel rectified deficiency.

Portable and semi-portable extinguishers must be inspeted and maintained in accordance with NFPA 10. The portable extinguisher located in the forward machinery space was found to be outside of proper operational pressure requirements. Vessel shall service extinguisher to meet standards outlined in NFPA 10 as amended by 176.800(b). Vessel had extinguisher re-serviced by a quilified servicing facility.

During inspection it was discovered that the emergency fuel shut valve for the vessels main fuel tank piping was installed on a secondary valve, approximately 5' from the valve installed at the fuel tank. Vessel shall alter placement of emergency fuel shut off valve to be located at the closest valve to the fuel tanks at next drydock.
