Placards containing instructions for the donning and use of the lifejackets aboard the vessel must be posted. All type Lifejackets must have posted instructions.

Each rescue boat and ring life buoy must be marked with Type II retro reflective material approved IAW 164.018 in subchapter Q of this chapter, or other standard specified by the commandant. The arrangement of the retro reflective material must be as specified by IMO Resolution A.658(16). Life buoy on bridge has peeling retro reflective material, repair/replace to satisfaction of CG Marine Inspector.

Remote fuel shut off stations must be marked in clearly legible letters at least 25mm (1 inch) in height indicating the purpose of the valve and the direction of operation. No markings, replace to satisfaction of CG Marine Inspector.

Vessels contracted for on or after July 1, 1961, which carry more than 150 passengers shall meet the requirements of subpart 72.05 of Sub-chapter H of this chapter. The minimum requirements for the bulkheads between the various spaces, where such bulkheads do not form the boundaries of main vertical zones, shall be as noted in table 72.05-10(e). * Category 5 space adjacent to category 8 space require a B-0 boundary. Bulkheads of the “B” Class shall be constructed with approved incombustible materials and made intact from deck to deck (or to ceiling as provided in paragraph (h) of this section) and to shell or other boundaries. The following bulkheads were found to be constructed of combustible materials and do not meet fire boundary requirements: - Aft bulkhead separating main deck dining area and the heads/freezer space. - Forward bulkhead separating main deck dining area and the kitchen space *Correct prior to next DD (30Sep2026).

Bulkheads, linings, and ceilings may have a combustible veneer within a room not to exceed 2/28 of an inch in thickness. Bulkhead in front of forward bar found to have combustible framing and veneer attached. *Correct prior to next DD (30Sep2026).

All case furniture such as desks, wardrobes, dressing tables, bureaus, dressers, etc., shall be constructed entirely of approved incombustible materials; except that a combustible veneer not exceeding 1/8 inch may be used on the top surface of such articles. The following case furniture was found to be constructed of combustible material: - Bar located next to forward stairwell. - Life jacket storage lockers surrounding aft stairwells. *Correct prior to next DD (30Sep2026).

A vessel must be operated in accordance with applicable laws and regulations in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons to be transported. The below deficiencies provide objective evidence that the vessel is substandard and not proactively managing technical and operational requirements. Vessel must make correction to the below deficiencies and get underway with marine inspectors to perform satisfactory operational test and drills prior to any movement.

The owner, charterer, master, or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency. - Crew was unable to get bilge suction to work for 20 minutes. - When testing fire pump, crew energized system with all fire hoses valves open and hoses laid out and unattended causing a wild hose incident.

Junction boxes, connection boxes, and outlet boxes, for use in damp or wet locations shall be of watertight construction. The following locations were found to have unapproved outlets: - Aft junction box with hard wired surge protectors for outlets - Mast where decorative lights extension cord is installed - Surge protector installed on stern of third deck 46 CFR 183.10-20(k) (old T)

Except as provided otherwise in this subpart, all electrical equipment, including controllers and distribution panels, shall be dripproof protected. Meaning that it is so constructed that uninsulated parts cannot be inadvertently touched. - Electrical panel going down to engine room missing breaker or blank allowing uninsulated parts to be touched. - Missing electric cover on pressure switch to fresh water system in forward engine room - CORRECTED ON THE SPOT - Electrical cover missing starboard engine room - Electrical cover on pump by sewage tank missing 46 CFR 182.25-5(b) (old T)

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. The following electrical shock hazards were identified: - Open electrical jbox in bridge above the forward window at centerline - Daisey chain of extension cords, surge protectors and decorative lighting on exterior of vessel - Dead end wire above port engine - CORRECTED ON THE SPOT - Open electrical jbox found on second deck in passenger space.

Permanently installed extension cords found in the following locations: - on bridge powering air conditioner - permanently installed decorative lighting on exterior of 3rd deck - permanently installed fans on 2nd deck 46 CFR 183.10-20(a)(2) (Old T)

Bilge suctions shall be fitted with suitable strainers having an open area not less than three times the area of the bilge pipe. The following discrepancies were identified for bilge strainers: - Steering gear bilge suction strainer clogged - Engine room bilge suction strainer broken - Void next to bar bilge suction missing strainer 46 CFR 182.25-5(b) (Old T)

All vessels shall be provided with not less than two avenues of escape from all general areas accessible to the passengers or where the crew may be quartered or normally employed, so, located that if one is not available the other may be. Means of escape from engine room blocked by tables and chairs. - CORRECTED ON THE SPOT 46 CFR 177.15-1(a) (Old T)

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. Vessel is equipped with two indicators for separate spaces. Neither indicator is labeled.

For portable fire extinguishers, the inspections, maintenance procedures and hydrostatic pressure tests required by Chapter 4 of NFPA 10, with the frequency specified by NFPA 10.The following deficiencies were identified with portable fire extinguishers: - two fire extinguishers were found without an annual inspection approval by a qualified servicing facility. - CORRECTED ON THE SPOT - Owner/Managing operator could not provide monthly inspection records required by NFPA10.

An adult life jacket must be provided for each person carried on board a vessel. 18 Life jackets missing from required passenger count.

Each life jacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel's name. Vessel's name worn on ring life buoys

Each rescue boat and ring life buoy must be marked with Type II retroreflective material approved in accordance with § 164.018 in subchapter Q of this chapter. Retroreflective tape on ring buoys were found wasted.

Connections must be made to battery terminals with permanent type connectors. Spring clips or other temporary type clamps are prohibited. Battery terminal connections are found to be made with wing nuts.

A vessel must be fitted with ground tackle and mooring lines necessary for the vessel to be safely anchored or moored. Anchor was found disassembled and not ready.

A durable placard must be posted next to all radiotelephone installations with the emergency broadcast instructions and information, specific to the individual vessel. Emergency Broadcast Plaquard was found to be unreadable.

Dry pipe vertical exhausts are permissible if suitably insulated. Propulsion engine's vertical exhaust piping leaving the manifold not properly isolated. 46 CFR 182.15-15(a)(1) (Old T)

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. The following deficiencies were identified in the vessels bilges: - Oil absorbent rags and trash found in engine room bilge - Lazarette found to have debris from shipyard construction - Ladder in aft void wasted

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. The following unsafe practices were identified: - Flammable liquids found stored in machinery spaces. - Zip tie found sticking out of engine valve cover - Bilge pump manifold valves labels not legible

Independent motors should be mounted on foundations as high as practicable above bilges. Raw water pump for air conditioning system found unmounted.

When operating a vessel on a body of water where the discharge of untreated sewage is prohibited by the Environmental Protection Agency under 40 CFR 140.3, the operator must secure each Type III device in a manner which prevents discharge of sewage. Sewage overboard discharge valve in stern space not equipped with securing device.

Vessels contracted for on or after July 1, 1961, which carry more than 150 passengers shall meet the requirements of subpart 72.05 of Sub-chapter H (Passenger Vessels) of this chapter. Staterooms and public spaces of 500 square feet or less with combustible furnishings, and isolated storerooms adjacent to Machinery Spaces must have an A-60 structural fire protection boundry. The following deficiencies were identified regarding structural fire protection: - Engine room has no structural fire protection boundry and has public spaces and storage rooms adjacent to the machinery space. - Cable trunk located in air handler space not protected

An adult life jacket must be provided for each person carried on board a vessel. 18 Life jackets missing from required passenger count. Passenger count for vessel has been reduced to 382 passengers until deficiency is resolved.

Bilge suctions shall be fitted with suitable strainers having an open area not less than three times the area of the bilge pipe. Void next to bar's bilge suction missing strainer. 46 CFR 182.25-5(b) (Old T)

Whenever damage or deterioration to hull plating that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space and may require the vessel to be drydocked or taken out of service to assess the extent of the damage, and to effect permanent repairs. The OCMI may also decrease the drydock examination intervals to monitor the vessel's structural condition. Starting January 5th, 2025 In accordance with the letter provided by the vessel owner agreeing that the vessel will not be allowed to carry passengers until proper repairs are made to the side shell identified on the provided UT report and areas around the rub rail identified by the attending marine inspector.

46 CFR 176.25-10(b): At all inspections special attention shall be paid to insure that no fire hazards exist. Engine room had excessive oil, fuel, jacket water, rags, used oil buckets, new oil buckets extension cords, hoses, pumps and garbage throughout. Clean all flammable and combustible materials to CG Marine Inspector satisfaction.

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspection. MIs discovered that the ground fault detection system is inoperable. Vessel must repair ground fault detection system. 46 CFR 183.01-15(a)

Vessels shall be equipped with a suitable number of portable battery lights. Place portable battery light at operating station. 46 CFR 184.30-1(a)

All electrical installations shall be to the satisfaction of the [name withheld] missing covers for battery installations. 46 CFR 183.05-20(a)

Generators and motors shall be mounted as high as practicable above the bilges to avoid damage by splash or contact with low lying vapors. MI observed a starboard generator pumps were not mounted. Vessel shall mount/secure both pumps. 46 CFR 183.10-5(c)

Portable fire extinguishers shall be installed and located to the satisfaction of the [name withheld] all fire extinguishers with appropriate brackets. 46 CFR 181.30-10(b)

All vessels shall be provided with a satisfactory arrangement for draining any watertight compartment. Remove all unnecessary submersible power bilge pumps to minimalize the potential for oily water being inadvertently pumped directly overboard. 46 CFR 182.25-1(a)

Provide a minimum standard... of electrical installations and equipment that will result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment and reasonable safe from electrical shock of personnel. Renew cover for fuse box in machinery space. 46 CFR 183.01-5

Gages to indicate engine cooling water discharge temperature... shall be provided for all propulsion engines and located so as to be in view of the operator. Renew temperature gage at operating station for the starboard engine. 46 CFR 182.20-5(b)

At each initial and subsequent inspection for certification and reinspection the marine inspector shall examine and test the main and auxiliary steering apparatus to the extent necessary to determine that its condition is satisfactory and that it is fit for the service intended. MI noticed chaffing on the hydraulic lines leading to the steering ram. Vessel needs to replace the chaffing gear and extend it to wholly cover the hydraulic lines. 46 CFR 176.25-35(a)

At each inspection... the marine inspector shall examine the bilges and other spaces to see that there is no accumulation of oil or other matter which may create a fire hazard. Remove oily water and clean bilges in machinery space. 46 CFR 176.30-1

ANY VESSEL OF AT LEAST FIVE NET TONS WHICH ENGAGES IN COASTWISE TRADE ON THE NAVIGABLE WATERS OF THE UNITED STATES MUST HAVE A CETIFICATE OF DOCUMENTATION (COD) BEARING A VALID ENDORSEMENT APPROPRIATE FOR THE ACTIVITY WHICH ENGAGED. THE VESSELS COD EXPIRED 28 FEB 2023. PROVIDE A VALID CERTIFICATE OF DOCUMENTATION. 46 CFR 67.7

A CERTIFICATE OF INSPECTION (COI) MAY BE ... REVOKED BY THE COGNIZANT OCMI AT ANY TIME FOR NON-COMPLIANCE. THE VESSEL'S COI IS HEARBY REVOKED DUE TO THE COD, WHICH IS REQUIRED FOR COASTWIDE TRADE, BEING EXPIRED. ONCE THE VESSEL OBTAINS A VALID COD AND PASSES A COI RENEWAL INSPECTION THEY MAY RESUME OPERATIONS. 46 CFR 115.107, 46 CFR 67.7

Nothing in this subpart limits the marine inspector from conducting such tests or inspections he or she deems necessary to be assured of the vessel's seaworthiness. Vessel is required to conduct a full marine survey with UT shots and provide report attesting to the structural integrity of the vessel's hull. 46 CFR 176.500 (b)(1)(iv)

Each life jacket must be marked with Type I retro-reflective material approved I.A.W. 46 CFR 164.018. Many life jackets were found with deteriorated retro reflective material. Owner/operator must replace all deteriorated retro reflective materaial with Type I retro reflective tape.

During testing of bilge pump and bilge piping system, found broken bilge pipe in engine room preventing satisfactory test of bilge pump an piping system. Make proper repairs.

The fire hose station on 2nd deck is missing a nozzle. Provide a serviceable nozzle for fire hose station.

Did not observe man overboard, fire, and security drills. Vessel must conduct satisfactory drills in the presence of USCG Marine Inspectors prior to the carriage of passengers.

In storage void: PVC pipe for hydraulic steering going through aft bulk head starboard side is not outfitted with proper bulkhead penetration, missing metal sleeve. Install proper metal fittings in way of bulkhead penetrations.

Found numerous plastic trash cans on vessel. Either provide proof existing plastic trash cans are non combustible or replace plastic trash cans with non combustible trash recepticles.

The fire pump outlet shall be fitted with a pressure gage. Repair or install pressure gage on fire pump outlet side showing at least 60 psi of pressure when running.

Observed water leaking from fresh water line in way of both forward and aft bulkheads in Steering gear room causing rust on botom plates. Make proper repairs to fresh water lines leading into heads from steering gear room to prevent rust.

Found loose faceplates of electrical outlet taped up with duct tape located on 2nd deck in front of the DJ stand. Make proper repairs to electrical outlet.

Replace retro reflective tape and 3-6 foot lanyard on ring buoy.

Each vessel must be equipped with a public address system. The Public Address system is not operational on the first deck of the vessel.

Propulsion machinery shall be suitable in type and design for propulsion requirements of the hull in which it is installed. The vessel must complete repairs to the shaft saver to the satisfaction of the Coast Guard and complete a satisfactory propulsion test prior to sailing with passengers.

All wiring and cables shall be suitable for marine service and for the particular installation used. Household extension cord is being used to power semi-submersible pump used for nuisance water. Remove extension cord and make permanent repairs. 46 CFR 183.10-20(a)

Each Vessel that carries more than 49 passengers must comply with watertight subdivision. During inspection marine inspectors found epoxy patches on forward and aft bulkhead in the engine room. Provide repair plan to the CG and do not begin repairs until plan has been approved. Top of watertight bulkhead between storage room and engine room from sewage tank to overboard on starboard side is missing weld seam.

The bilge suctions shall be fitted with suitable strainers having an open area not less than three times the area of the bilge pipe. Bilge piping in Void # 1, # 2 and Lazzarete missing strainer. Install appropriate strainers. 46cfr182.25-5(b)

At all inspections special attention shall be paid to ensure no fire hazards exist. Clean up oil residue from engine room bilge and clean and inspect the port fuel filter housing. 46CFR176.25-10(b)

All electrical installations shall be to the satisfaction of the [name withheld] lens covers on starboard bathroom light fixtures. 46 CFR 183.01-15(a)

The marine inspector shall examine all accessible parts of the vessel’s hull, machinery, and equipment to be assured that they are in satisfactory condition. Salt water supply pump to flush toilets is excessively leaking. 46CFR176.10-5(b)

Each life preserver carried must have at least 200sq. cm of retroreflective material attached on its front side and 200sq cm on its backside. Replaced all retroreflective tape on faded and damaged life jackets. 46CFR180.25-25

…each battery without an expiration date indicated on it or for which the owner or managing operator does not have a record of the expiration date, used in an item of lifesaving equipment, must be replaced during the annual inspection. Replace batteries on life buoy lights. (46CFR122.726(e)

Abandon ship and man overboard drills and training shall be logged. Produce record of previous drills conducted. (46CFR122.520(g)

Each vessel must meet the applicable requirements in Subchapter S of this chapter. Submit repair proposal and make permanent repairs to the port side shell in the aft compartment. 46CFR177.13-1

The number and identification of the items stowed inside, and their size, must be marked in clearly legible letters and numbers on each container for life jackets. Mark all containers containing life jackets with quantity and type. (46CFR180.25-15(b)

Observed corrosion of deck in way of port side main entrance, male head on port side aft main deck, and deck where deep coolers are located on aft main deck. Crop and renew to good materiall.

Observed evidence of deck corrosion along perimeter of main deck under carpet. Remove carpet and decorative side panels along perimeter of main deck for further Coast Guard inspection.

The master shall conduct drills and give instructions as necessary to ensure that all crew members are familiar with their duties. There is no record that Man-Overboard and Fire drills were conducted within the past year. Conduct drills prior to carrying passengers. - 46CFR185.25-10 "Old TL"

Batteries shall be secured against shifting with the motion of the boat. Battery for the (#2) generator located in the centerline of the engine room is not secured and can freely slide across the deck. Secure battery to the satisfaction of the Marine Inspector. - 46CFR183.05-20(b) "Old TL"

An ammeter shall be provided that can be used for measuring the current of each generator that is in operation. Ammeters on board do not properly measure the current of each generator. 2nd ammeter appears to measure both generators. - 46CFR183.10-5(g) "Old TL"

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. First-aid kit had multiple expired items. - 46CFR121.710

All self-propelled vessels must be fitted with a Federal Communications Commission type accepted general marine radar system for surface navigation with a radar screen mounted at the primary operating station. Vessel's marine radar is not properly connected and is inoperable. Vessel is limited to a Lakes, Bays, and Sounds route until radar is operable. - 46CFR121.404

Each vessel certificated to operate on a limited coastwise route within three miles of land in warm water, and is equipped with an FCC type accepted Category 1 406 MHz EPIRB installed to float free and automatically activate, is not required to carry survival craft. Vessel removed EPIRB and does not have any survival craft. Vessel is limited to a Lakes, Bays, and Sounds route within one mile of shore until an EPIRB is properly installed to the satisfaction of the Marine Inspector. - 46CFR117.205(d)

Exhaust gas must not leak from the piping or any connection. Observed evidence of an exhaust leak on the forward end of the starboard main exhaust silencer. Closer inspection revealed scale rust, severe deterioration of, and holes in the exhaust piping where it meets the silencer. 46 CFR 119.430 - 30c

Connections must be made to battery terminals with permanent type connectors. Spring clips or other temporary type clamps are prohibited. Replace the battery within the 32vdc system that has a melted post; replace the 32vdc battery jumpers and ensure proper connections are made at every battery connection onboard. 46 CFR 120.350 - 30c

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Permanently mount the 73kw generator battery switch; repair or replace voltmeters 1 and 2 on 32 volt DC system; repair or replace AC voltmeter cover. 46 CFR 120.350 - 16c

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Secure the lamp base in the stern light housing prior to operating between sunset and sunrise. 46 CFR 120.420 - 705c

An adult life jacket must be provided for each person carried on board a vessel and in addition, a number of child-size life jackets equal to at least 10% of the number of persons permitted on board must be provided, or such greater number as necessary to provide a life jacket for each person being carried that is smaller than the lower size limit of the adult life jackets provided to meet this section. Replace 30 adult life jackets and 3 child life jackets. 46 CFR 117.71 - 701c

A vessel on an oceans, coastwise, limited coastwise, or Great Lakes route must carry six hand red flare distress signals approved in accordance with §160.021 in subchapter Q of this chapter and six hand orange smoke distress signals approved in accordance with §160.037 in subchapter Q of this chapter. Provide 3 additional orange and 2 additional red signal flares. 46 CFR 117.68 - 701c

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Mark stowage location for child life jackets. 46 CFR 117.78 - 701c

Renew vacuum gauge on port side main diesel engine fuel strainers.

Renew port sea strainer for 73Kw generator.

Blank off opened pop-out on starboard side distribution panel in engine room.

Make permanent installation for water tank in engine room in way of pumps.

Crop and renew holes on port side of sewage holding tank.

Starboard side engine is shifting by 1/4 inch. Adjust engine mounts to prevent shifting.

Blank off old sewage discharge piping, as close to bulkhead as practicable.

Side shell port side aft kitchen in way of midwell, make temporary repair. Correct at next dry dock.

Seal port side aft kitchen window.

Wasted frame in storage room at keel. Itemis to be corrected at next dry dock. Vessel is limited to lakes, bays, and sounds route until repair is made.
