Each portable fire extinguisher on a vessel must be of an approved type. The minimum number of portable fire extinguishers required on a vessel must be acceptable to the cognizant Officer in Charge, Marine Inspection, but must not be fewer than the minimum number required by table 1 to § 181.500(b) and other provisions of this section. Vessel is missing servicing reports for multiple portable fire extinguishers.

Each life float and bouyant apparatus must be secured to the vessel by a painter and a float-free link. Starboard life raft was not properly secured in it's bracket due to no line being attached on the breaking side of the float free link. Vessel capacity is reduced by 50 passengers prior to rectification.

Dry pipe vertical exhausts are permissible if suitably insulated. Exhaust pipe in starboard enginge room missing lagging. 46 CFR 182.15-15(a)(1)

Propulsion machinery shall be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. MI identified significant leak coming from starboard propulsion shaft. 46 CFR 180.20-5(a)

All life floats and buoyant apparatus shall be conspicuously marked with the vessel's name. In addition the number of persons allowed on each life float or buoyant apparatus shall be conspicuously marked or panted thereon in letters and numbers at least 1 1/2 inches high. Life floats had worn painting. 46 CFR 180.20-5(a)

Electrical equipment on a vessel must be installed and maintained to provide services necessary for safety under normal and emergency conditions, protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock, minimize accidental personnel contact with energized parts and prevent electrical ignition of flammable vapors. MI observed extension cord spliced and used as a permanent wiring to the bilge pump in the stbd engine room. Also, another extension cord is being ran through the aft bulkhead of the engine room to power and electric bilge pump.

If an opening in a location specified in paragraph (d) of this section is provided with a class 1 watertight door, the height of the watertight coaming need only by sufficient to acommodate the door. Watertight coaming is less than 6 inches. Coaming is to be raised at next dry dock.

This part applies to tank vessels that use a vapor control system (VCS) to collect vapors emitted to or from a vessel's cargo tanks while operating in the navigable waters of the United States. Vessel has a a pinhole leak in a weld seam located at the junction of the drainpipe and vapor pipeline. Vessel is to offload loaded cargo. Vessel is not to load with any cargo that is required the use of the VCS. Vessel is to enact permanent repairs to the satisfaction of the Coast Guard.

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment, other than small buoyancy compartments, under all practicable conditions. MI observed a leak on the discharge pipe of the port hull bilge pump. Repair pipe to the satisfaction of USCG.

Each ship transporting more than six passengers for hire equipped with a radiotelephone station in accordance with Part III of Title III of the Communications Act must carry a radio operator who meets the following requirements: (1) Where the station power does not exceed 250 watts carrier power or 1500 watts peak envelope power, the radio operator must hold a marine radio operator permit or higher class license. Provide evidence to USCG that the vessel is manned with a radio operator who holds a marine radio operators permit. 47 CFR 80.159(e)(1)

A hydrostatic release unit when used in a float-free arrangement must be approved under approval series 160.062 or 160.162 or other standard specified by the Commandant. Vessels liferaft hydrostatic releases did not have and expiration date. Provide evidence of installation date or renew hydrostatic releases and provide evidence to USCG.

Each buoyant apparatus must have Type II retroreflective material meeting subpart 164.018 of this chapter on each side and end. MI observed damaged retroreflective material on all 3 life floats. Replace all damaged/delaminated retroreflective material on all lifefloats to the satisfaction of USCG MI. 46 CFR 160.010-4(p)

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. MI's observed the interconnected fire detection system improperly installed prohibiting the interconnected function and alerting at the pilothouse. Properly wire the interconnected fire detection system to the satisfaction of attending MI. Also, install one of the fire detectors within the galley space.

A vessel on an oceans route must carry - (1) Six hand red flare distress signals approved in accordance with § 160.021 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.121; and (2) Six hand orange smoke distress signals approved in accordance with § 160.037 in subchapter Q of this chapter, or other standard specified by the Commandant. MI's observed all flare and smoke signals expired. Provide required amount of pyrotechnics.

Each life float must be fitted with a lifeline, pendants, two paddles, a painter, and a light. MI's observed one life float light inoperable. Provide operable life float light to the satisfaction of attending MI.

An adult life jacket must be provided for each person carried on board a vessel. MI's condemned 15 life jackets. Provide serviceable life jackets to amount the number required on the COI. Vessels adult passenger count is reduced to 107.

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Post life jacket donning instructions in a visible area on the upper level.

Electrical equipment on a vessel must be installed and maintained to Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. MI observed an open junction box in the port side engine room entryway as well as the fresh water start control box in the stbd side engine room. Install proper enclosures and repair start control box to prevent accidental electrical shock.

Whenever any vessel is to be hauled out to carry out major repairs or alterations affecting the safety or seaworthiness of the vessel, it shall be the duty of the person in charge of the vessel or the owner to report the same, together with the nature of any repairs or alterations contemplated, as far in advance as practicable to the Officer in Charge, Marine inspection, in order that an examination may be made by a marine inspector. Vessel was observed hauled out at Peninsula Marine Service Bolivar, TX with a new insert into the hull with no notification made to the local [name withheld] attended on 08FEB2023 and observed a insert approximately 4'x6' centerline on the hull as well as a through hull insert on the starboard hull inboard. MI's at permission from the vessel owner conducted a visual weld inspection and NDT (water test) of the insert. MI's visually observed substandard welds on the interior. The water test showed a pinhole leak approximately halfway along the starboard weld. MI also observed an unauthorized doubler approximately 6' outboard of the new insert on the starboard side that had water seeping from underneath it during the water test. Vessel is to submit a repair proposal for approval to encompass the doubler plate with a new insert. Installation of the new doubler will encompass the initial pinhole leak. Vessel is also required to inspect interior welds for pitting and blowouts of the weld and to properly repair those welds to the satisfaction of attending MI. Vessel shall notify OCMI via photographs of all back gouged welds prior to completing final weld. Conduct NDT to the satisfaction of attending MI.

Plastic tie wrap found being used as support, the use of plastic ties must be limited to the use of bundling or retention 46 CFR 183.340(4)

Cable and wiring found in engine room with dead ends. Electrical equipment must be installed to minimize accidental personnel contact with energized parts 46 CFR 183.200(c)

Life floats provided could only support 132 of 136 endorsed on COI, Life floats provided could not support 100% of the total number of persons permitted on board. **Reduced total POB endorsed on COI** 46 CFR 180.202(b)(2)

Fire hazard, found excessive build up on galley vent hood, at each inspection all fire hazards and other hazardous conditions must be corrected 46 CFR 176.830(a)

Marpol V placard not on board, applicable to vessels greater than 26' in length and must be displayed in prominent location 33 CFR 151.59

Station License not on board, must be made readily available 46 CFR 184.502

Ship log not being properly maintained, log must show training at least once every three months 46 CFR 185.420
