A visual and audible alarm must be provided at the operating station to indicate high water level in a machinery space bilge. Bilge alarm in engine room did not operate. 46 CFR 182.530

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. 46 CFR 181.405 Visual alarm for interconnected fire detection system did not operate in the engine room.

Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck from all passenger and crew accommodations spaces located below the main deck. Emergency lighting did not properly operate in the aft passageway. 46 CFR 183.432

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are not familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons who have fallen overboard. Vessel has not conducted any drills since being in service from dry dock completion in March. Unable to get underway to complete drills on day of inspection. 46 CFR 185.520

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. MI identified an excess of oily water beneath center MDE. Vessel is to remove water to satisfaction of the Coast Guard prior to the carriage of passengers.

For each watertight door which is in a required subdivision bulkhead, an indicator light must be installed in the pilothouse and at each other vessel operating station from which the door is not visible. The indicator must show whether the door is open or closed. MI identified vessel having a mixed audible and visual alarm for engine room watertight door that continued to sound when door was closed. Vessel is instructed to correct both audible and visual prior to carriage of passengers.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. MI identified multiple loose wirings in the engineering space. Vessel is to identify any and all loose/peeling wires within engine room, then remove or replace in accordance with CFR requirements.

The exhaust piping must be so arranged as to prevent backflow of water from reaching engine exhaust ports under normal conditions. MI identified leaking of water from exhaust port on starboard engine. Vessel is to enact repairs to satisfaction of the CG.

Inspection and operation of all watertight closures in the hull, decks, and bulkheads including through hull fittings and sea valves. MI identified a leak on the starboard shaft seal. Vessel is to enact repairs to the satisfaction of the CG

An interconnected detection and alarm system must: Be installed such that the actuation of alarm in one area results in both audible and visual alarms in all areas required by 46 CFR 181.405(c) or 118.400(d) to be protected by the interconnected detection and alarm system. MI discovered the alarm system in main engine space and steering gear without visual indicators.

Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck from all passenger and crew accommodation spaces located below the main deck. MI discovered an aft berthing fwd emergency light inoperable.

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. MI found a compressor in the main engine space without a guard.

A penetration in a watertight bulkhead must be as high and as far inboard in the bulkhead as practicable, and made watertight. MI discovered two holes in a machinery space bulkhead.

Where flexibility is necessary, a section of flexible metallic hose may be used. Nonmetallic hose may be used for wet exhaust systems provided it is especially adapted to resist the action of oil, acid, and heat, has a wall thickness sufficient to prevent collapsing or panting, and is double clamped where practicable. MI discovered a missing clamp on the starboard engine exhaust cooling water hose.

Vessels described by 46 CFR 175.110(d) must ensure that the two means of escape required in paragraph (b) of this section are unobstructed and the door, hatch, or scuttle is not located directly above, or dependent on, a berth. The interior pilothouse door is designed so that there is potential for the door to obstruct the aft vertical means of escape from the forward accommodation space. Remove or reconfigure the door so there is no potential to obstruct the means of escape. Affect repairs/alterations prior to carriage of passengers in the forward accommodation space to the satisfaction of USCG.

On vessels where the certificate of inspection is renewed less often than annually, a marine inspector must check the settings of the safety relief valves on all pressure vessels twice every five years. No more than 3 years may elapse between checks. MI could not verify settings during COI. Prove proper operation of pressure relief valve to the satisfaction of attending marine inspector. 46 CFR 61.10-5(i)2

46 CFR 182.510(a), 46 CFR 182.520 Table A vessel must be provided with a satisfactory arrangement for draining any watertight compartment, other than small buoyancy compartments, under all practicable conditions.A vessel more than 19.8m (65 ft) is required 2 fixed power pumps with a minimum capacity rating of 190 LPM (50 GPM). The bilge system is inoperable and will not pump out any space.

46 CFR 179.360 Each hatch exposed to the weather must be watertight. The engine room emergency exit hatch will not fully seat, allowing for the chance for down flooding.

46 CFR 183.200 Electrical equipment on a vessel must be installed and maintained to: (b) Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment and electrical shock. Shore tie is repaired with electrical tape. Make proper repairs to the damage shore tie cable.

46 CFR 183.432 Each vessel must have adequate emergency lighting fitted along the escape to the main deck from all passenger and crew accommodation spaces. A source of emergency lighting is required in the aft portion of the galley space in the vicinity of the exit.

46 CFR 183.310(a)(1) Each vessel that relies on electricity to power the following loads must be arranged so that the loads can be energized from two sources of electricity: (ii)Interior lighting except for decorative lighting. (iii) Communication systems including a public address system. Emergency lighting and radio is inoperable on the back-up battery when the power to the vessel is secured.

Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Vessel must provide training to crew for monthly inspections/ monthly inspection logs to the satisfaction of the MI.

The sidelights of vessels of 20 meters or more in length shall be fitted with matt black inboard screens and meet the requirements of §84.15. On vessels of less than 20 meters in length, the sidelights, if necessary to meet the requirements of §84.15, shall be fitted with matt black inboard screens. With a combined lantern, using a single vertical filament and a very narrow division between the green and red sections, external screens need not be fitted. Port and Starboard navigation light screens are wasted and must be replaced to the satisfaction of the attending MI.

Materials used in piping systems must meet the requirements of this subpart and be otherwise acceptable to the cognizant OCMI. Vessel must replace piping to the satisfaction of the attending MI. Must be completed at next DD.

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. If the handle to the shutoff valve at the tank is located inside the machinery space, it must be located so that the operator does not have to reach more than 300 millimeters (12 inches) into the machinery space and the valve handle must be shielded from flames by the same material the hull is constructed of, or some noncombustible material. Electric solenoid valves must not be used, unless used in addition to the manual valve. MI discovered that the reach rod used to shut off the fuel supply was secured by a cotter pin that was not properly installed. Vessel rectified deficiency to the satisfaction of the MI.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: Charts of large enough scale to make safe navigation possible, U.S. Coast Pilot or similar publication, Coast Guard Light List, Tide tables, and Current tables. Vessel must obtain current publications to the satisfaction of the attending MI.

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. MI's discovered that EPIRB Hydrostatic Release expired in June of 2019. Vessel is required to install an approved Hydrostatic Release to the satisfaction of the attending MI.

A vessel with more than one passenger deck and a vessel with overnight accommodations must have the public address system operable from the operating station. MI's discovered that PA system was non operational. Vessel is required to rectify system to the satisfaction of the attending MI.

A fire hose with a nozzle must be attached to each fire hydrant at all times. For fire hydrants located on open decks or cargo decks, where no protection is provided, hoses may be temporarily removed during heavy weather or cargo handling operations, respectively. Hoses so removed must be stored in nearby accessible locations. One of the two fire hoses on board did not have a brass fitting to attach to the fire hydrant. Vessel must replace fire hose to the satisfaction of the attending MI.

Electrical equipment exposed to the weather must be watertight. The junction box on the port aft side of the deck must be replaced to the satisfaction of the attending MI.

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Abandon ship drills and man overboard drills were not properly logged. Vessel must provide training to personnel on the proper format of the drills log to the satisfaction of the MI.

Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. The installation and location must be to the satisfaction of the Officer in Charge, Marine Inspection. E/R portable extinguisher bracket is wasted and must be replaced to the satisfaction of the attending MI.

Each ungrounded system must be provided with a suitably sensitive ground detection system located at the respective switchboard that provides continuous indication of circuit status to ground with a provision to momentarily remove the indicating device from the reference ground. MI's discovered that continuous indication status was not able to be removed. Vessel must meet the intent of the regulation to the satisfaction of the attending MI.

A vessel's hull must not carry current as a conductor. The switchboard displayed a ground. Vessel must rectify the ground to the satisfaction of the attending MI.

Fire hazards to be minimized. The general construction of the vessel must be such as to minimize fire hazards insofar as it is reasonable and practicable. Excessive amount of oil/oily water found in engine room bilges creating fire/pollution hazard. Remove oil/oily water from bilges.

Excessive oily dirt/grime found caked on the engine room exhaust fan and fan housing creating a fire hazard. Clean fan and fan housing of excessive caked on oil. Recommended to vessel operator to properly re-install missing crank case exhaust filters as originally provided by MDE manufacture.

Emergency escape hatch from the machinery and auxiliary Mech space were found seized, requiring mechanical force to open. Repair/Service all escape hatches and prove proper operation to the satisfaction of the attending marine inspector

Found dive tanks, boxes and gear adrift in the rudder compartment in close proximity to the rudder steering arms and linkage rod. This condition could create an unsafe operational condition. Remove all loose/adrift gear and properly secure away from steering equipment.

Port side steering ram has a hydraulic hose that is dry rotted and is showing signs of excessive deterioration. Remove and replace hose.

Class III WTD separating the machinery space from the Aux machinery space has several hydraulic oil leaks coming from the hydraulic piping associated with the actuator used for opening and closing of the door. Effect repairs and prove proper operation of the installed WTD.

The fuel shut of valve, to the port main diesel engine, could not be secured with the use of the reach rod. Repair and prove proper operation of the shut off valve with the use of the reach rod.

Approx 15ft of fire main was found to be wasted in several location.

Port SSDG has high pressure fuel hoses with unapproved fittings. Fuel hoses have metal hose clamps on each side.

Vsl was unable to get underway to conduct abandon ship drill or fire drill due to unsafe condition of fuel system and fire main system.

04 of the 06 required red hand flares onboard are not CG approved.

Lifejacket stowage was not properly labled with number of lifejackets stowed inside.

03 lifebuoys found to have markings worn off.

Port side steering gear hoses are wasted.

Improperly installed battery charger for winch on back deck, zip tied wires and power strip to bulkhead.

44 life jackets had its internal flotation material deteriorate to the point of non-serviceability. Replace all prior to carrying passengers.

EPIRB has missing registration sticker, expired battery and expired hydro static release device. No operations beyond the 3 miles allowed.

Class II water tight door did not have a functioning open door indicator light. Repair indicator light to indicate when water tight door is open at any position.

Port side remote fuel shutoff valve not operable.

Control air system could not be utilized to transfer main control to the remote operating station.
