Engine exhaust gas must not leak from the piping or any connections. Port side generator exhaust muffler has an approx. 3 inch rupture allowing exhaust gas and cooling water to freely exit the muffler into the lazerette which is located under the main passenger accommodation space. 46 CFR 182.430(b)

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. The marine inspector identified a leaking salt water exhaust pipe and evidence of hull plate corrosion in the lazarette compartment located underneath the lead ballast. The inspector was unable to view the area under the ballast to determine the extent of the corrosion to the hull plate due to limited space between the ballast and hull plate. Remove the ballast and clean all debris under the lead ballast in order for the inspector to determine the amount of corrosion under the lead ballast. 46 CFR 176.840

Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. OSHA, UL and NFPA 70 NEC specific to the use and application of cord connected electrical power strips (also referred to as re-locatable power taps or RPT) prohibit RPT daisy chain connections. The main hazard associated with multiple power strips (also known as re-locatable power taps – RPT as defined by Underwriters’ Laboratories) is fire. Electrical fires can result from overloading the circuit or damage to the insulation of the cord or device. The UL listing for re-locatable power taps (RPT) states “cord-connected RPT is not intended to be connected to another cord-connected RPT”. UL states "A cord-connected RPT is not intended to be connected to another cord-connected RPT." Outlet devices and the number of receptacles per circuit are detailed in NFPA 70 National Electric Code for commercial use 210.21 and 220.3. Multiple RPT connections identified by the marine inspector at the "DJ" booth located on the 2nd deck creating a fire hazard. Install a new outlet or multiple outlets to serve the electrical connections IAW the professional guidance found in NFPA 70 as needed. 46 CFR 183.200(b)

Electrical installations shall be installed to prevent fires. Battery chargers are installed on gen and main engine batteries without temperature sensors to prevent overcharging and overheating of batteries. 46 CFR 183.200(b)

The two required fixed power bilge pumps shall be self-priming and shall each be driven by different sources of power. MI observed inoperable self-priming bilge system from the lazarette and engine room. Repair inoperable bilge system. 46CFR182.115(a) 46CFR182.25-10(c)

An adult life jacket must be provided for each person carried on board a vessel. Eight adult life jackets were missing from the requirements on the COI. Eight new Type I life jackets were replaced on the spot. Deficiency rectified. 46CFR180.71(a)

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. MI observed donning instructions for life jackets were only applicable to the "extended" life jackets on board. Add additional donning instruction placards for each type of life jacket on board. 46CFR185.516

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, as indicated by the vessel’s stability letter, the maximum number of passengers allowed on an upper deck must be indicated by a durable marking of at least 2 mm (1 inch) numbers and letters at the entranceway to that deck. MI observed that the entrance of the third deck, which passengers are permitted, is not labeled in accordance with the stability letter's criteria. Label third deck maximum allowable passenger limitations. 46 CFR 185.602 (g)

Each life ring buoy on a vessel must be approved in accordance with 160.050 subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.150. MI observed the third deck ring buoy plug was missing. Repair/Replace ring buoy. 46CFR180.70(b)(1)

A vessel must comply with the applicable requirements for any radio installation, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Radio installation does not have GPS input. Vessel's route restricted to not more than 1,000 feet from shore. IAW: 46 CFR 184.502

Vsl is broadcasting: Greater/smaller than expected vessel length, no dimension information (AIS Antenna Position ABCD values equals 0), and not broadcasting an AIS Ship Type.

Fire hose burst, replace with UL19 approved fire hose to the satisfaction of attending marine inspector 46 CFR 181.320

Emergency fuel shutoff valve is not closing all the way, prove proper action shutoff to the satisfaction of the attending marine inspector 46 CFR 182.455

High water alarm bilge system at operating station not working properly, show prove of proper action of bilge system to the satisfaction of the marine inspector. 46 CFR 182.530

Fuel ball valves handle rotates 360, replace with adequate valves to the satisfaction of the attending marine inspector 46 CFR 182.455

Upper deck passenger capacity is limited to not more than 119 passengers, until MSC reviews/approves submitted changes to lighships characteristics.

REPAIR LEAKS IN RAW WATER SYSTEM FOR TOILETS IWO FIRE PUMP.

REPAIR/REPLACE BOW THRUSTER HIGH BILGE LEVEL ALARM.

Cap off open-ended new hydraulic pipes penetrating the aft bulkhead of the bow thruster space.

Insure all watertight bulkhead penetrations are properly sealed.

Insure all exhaust coolling hoses are double clamped.

Insure all battery connections are tight and have lock nuts or lock washers.

Cap-off dead end pipe in the fwd bulkhead of the engine room, port side.

Install a handle on the sea chest valve.

Repair and replace the exhaust lagging in the engine room as needed.

Replace the fuel crossover hose on the #2 main engine.

Install a handle on the fuel supply valve for the #1 generator prime mover.

Provide a non-conductive mat in front of the switchboard.

Properly label the main switchboard as needed.

Prove proper operation of the overspeeds on the prime mover for the generators.

Replace the non-metalic (hose) fitting in the fire pump overboard discharge piping.

Label the valve manifold.

There are cracked welds in the forepeak, starboard side, where frames meet bulkhead. Cracks are on frames 5, 6, 7, and 8 numbered down from deck. V/weld cracked welds.

Air receiver is past due date for hydrostatic pressure test. Have air receiver tested in the presence of USCG Marine Inspector.

Properly log man overboard and fire drills.

There are cracks on the starboard railing near the bow. Make permanent repairs.

Properly label ciruits in 03 panel boards (#3, #4, & #5)

General housekeeping in Galley (Clean Vents, Dispose of Grease Vats on Deck, Remove Broken Glass in Bus Pans, & Remove Standing water in Serving Pans).

Procure Storm Rails Around top of Stove.

Repair Oil Pressure Gauges in Pilot House on Centerline/Starboard Side MDE's.

Repair water leak on port generator.

Install guard for belt on MDE #2.

Install locking devices for oven doors.

Install sea rails for stove.

Replace block heater wire.

IRT area noted in NYO8, C/R area wasted in excess of 25% of original scantling w/ min plate of 18x18. Insert to good aluminum. Fit-up and final die pen test must be witnessed by CGMI.
