At least one ring life buoy must be fitted with a lifeline. Neither of the two ring life buoys had a lifeline attached. Attach a lifeline to one ring life buoy.

Each distribution panel or switchboard must be totally enclosed and of the dead front type. The cover for the vessel's distribution panel was missing. Replace distribution panel cover.

There must be an individual holding an appropriate license or a valid MMC with endorsement as master in command of every inspected small passenger vessel. Master presented an expired MMC. Vessel had a secondary master onboard at time of inspection.

Scantlings, materials, fastening, workmanship, and arrangements of each vessel are such as to provide for the safe operation in accordance with the terms of its certificate of inspection. Primary consideration shall be given to the provision of a seaworthy hull. Visible deterioration and damage was noted on the starboard quarter of the lazarette where the side shell meets the transom. Damage resulted in visible holes/pitting in the hull which is impairing its watertight envelope. O/O is to submit a repair action plan to the OCMI for approval and effect proper permanent repairs. 46CFR177.01-5 (OLD T)

Flexible hose shall be fabricated with an inner tube and a cover of synthetic rubber or other suitable material reinforced with wire braid. The hose cover shall be adjudged self-extinguishing per ASTM D1692. Port main engine fuel oil return hose was heavily deteriorated and rotted through the outer and inner sheathing. O/O is to submit a repair plan and renew the port engine fuel return hose to original construction standard or greater to the satisfaction of attending marine inspector. 182.20-40(a)(2)(ii) (Old T) 46CFR182.720(e) (New T)

Where flexibility is necessary a section of flexible metallic hose may be used. Non metallic hose may be used provided it is especially adapted to resist the action of oil, acid, and heat and has a wall thickness sufficient to prevent collapsing or panting. Port main engine exhaust hose flex joint was found to be heavily deteriorated and beyond its' service life. O/O to renew port main diesel engine exhaust flexible hose to satisfaction of attending marine inspector. 46CFR182.15-20(c) (Old T)

Deck rails must be designed and constructed to withstand a point load of 91 kilograms (200 pounds) applied at any point in any direction, and a uniform load of 74 kilograms per meter (50 pounds per foot) applied to the top rail in any direction. The point and uniform loads do not need to be applied simultaneously. Rails on passenger decks of a ferry or a vessel engaged in excursion trips, including but not limited to sightseeing trips, dinner and party cruises, and overnight cruises, must be at least 39.5 inches high. Port side forward deck rail weld at the gunwale was found broken and weld on top rail adjoining two rail sections was found cracked. Exhaust gas must not leak from the piping or any connections. The piping must be properly supported by noncombustible hangers or blocks. Ship service generator aluminum exhaust pipe on the starboard outboard side of system was found to have a hole compromising the integrity of the pipe. O/O is to make permanent repairs to the exhaust pipe to the satisfaction of attending marine inspector. 46CFR182.430(b) (New T)

A submersible electric bilge pump conforming with this part may be used as a power bilge pump required by Table 182.520(a). Electrical submersible pump in engine room was found to not be permanently mounted and the suction hose assembly was made of collapsible hose. O/O is to permanently mount pump and install non-collapsible suction line to satisfaction on attending marine inspector. 46CFR182.520(e) New T

Propulsion machinery shall be installed and maintained in such manner to afford adequate protection against machinery failure. Port main diesel engine was found to have a significant raw water cooling leak on pump flange housing. O/O is to effect proper repairs to raw water pump assembly. 46CFR182.01-5 (Old T)

Each ring life buoy on a vessel must be approved in accordance with § 160.050 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.150. Port side ring life buoy was found to be beyond its' service life due to grab line deterioration and dry rot of buoy. O/O is to renew ring life buoy to satisfaction of attending marine inspector. 46CFR180.70(b)(1) (New T)

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. Marine Inspector attended a damage survey and noted two hull punctures on starboard side on the vessel. Fwd puncture located on stbd quarter measuring approx 1/2" x 1/2". Second puncture located just fwd of amidship approx 1" x 3/4". Submit temporary repair proposal to this office for OCMI approval prior to start of repair.

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspection. Exposed wire in the third void space. Find the source for the wire, remove or have wire connected in accordance with regulations.

Each ring life buoy must be marked with Type II retroreflective material arranged in accordance with IMO Resolution A.658(16). No retroreflective material was on the life buoys. Have ring life buoys meet the applicable regulations.

A submersible electric bilge pump may be used as a power bilge pump required by Table 182.520(a) only on a vessel of not more than 19.8 meters (65 feet) in length carrying not more than 49 passengers, other than a ferry, provided that a positive shutoff valve is installed at the hull penetration. No valve installed at engine room electric submersible bilge pump hull penetration. Install valve at hull penetration.

Be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. Wiring on port and stbd side of void #2 and engine room not completely supported and resting on metal frames creating a potential chaffing hazard. Provide wire supports as required.

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. Posted procedures do not match equipment used (rescue ladder) and crew actions during man overboard drill. Updated emergency instructions to match equipment used and procedures followed by crew.

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew members are familiar with their duties during emergencies that necessitate abandoning ship or the recovery of persons who have fallen overboard. Drill not logged in previous 3 months due to vessel inactivity. Crew demonstrated and logged drill during inspection.

At least one ring life buoy must be fitted with a lifeline. If more than one ring life buoy is carried, at least one must not have a lifeline attached. No lifeline attached to life ring. Vessel rep installed lifeline prior to completion of inspection.

Each bilge suction line shall be provided with a check valve at some accessible point in the bilge line. Forward bilge suction line not fitted with a check valve.

Crop and renew affected area on the forward starboard bow to good metal, to the satisfaction of the attending marine inspector.

Prove proper operation of LAZ bilge alarm.

Instal bonding wire to fuel tank fill piping.

Relocate fuel vent pipe to exterior of ventillation housing for engineroom.

Port and STBD engine exhaust cooling hoses and port engine inlet cooling hose are dry rotted. Replace in kind.

Rudder tubes were wasted, need to provide repair proposal to address the issue.

Support frame needs to be repaired in forward most void, repair support frame.

Coupling corrosion in stbd keel cooler valve, Prove operation of keel cooler valves and address corrosion issue.

Provide adequate strainers for all through hull valves.

Port through hull cutlass bearing needs to be replaced.

CHANGE THE VESSEL'S NAME ON THE HULL, DOCUMENTS, LICENSES, LIFESAVING EQUIPMENT, AIS, LOGS, PLANS, ETC. TO CORRESPOND TO RECENTLY ISSUED CERTIFICATE OF DOCUMENTATION.

Retroreflective tape was wasted on 26 PFD's

Fuel lines running to generator were under heavy vibration and were not properly clamped in place with two hose clamps and fuel hoses were not of approved type for service.

Marine Sanitation Device overboard discharge was not secured. The MSD is not currently in operation or being used, It is however, piped for use. Vessel rep is going to secure the MSD by disconnecting the piping to the holding tank.

Repair inoperative bilge high leval alarms system.

Replace missing door from breaker box in pilot house.

Replace 8 type 1 adult life jackets and 1 type 1 child size life jacket.

Make permanent repairs to port MDE circ pump

Replace the foward and aft hatch covers.
