Each vessel must be designed, constructed and maintained to minimize fire hazards. -Repair/replace oil leak from turbo drain line on port main propulsion engine. -Replace 02 chaffed injector supply lines on starboard generator. 46 CFR 127.220

The owner of an inspected offshore supply vessel must be equipped with a master key control system for physical, manual, or electronic keys or access systems, which provides controlled access to all copies of the vessel’s master key. Only crewmembers identified by position in the master key control system are allowed access and use of a master key. The list of crewmembers with master key access must be kept on the vessel. Vessel must record in a logbook information on all access and use of the vessel’s master key, including dates and times of access, the room or location accessed, and the name and rank of the crew member who used the master key Vessel does not have master key control requirements fully implemented. 46 USC 3106 CVC Policy 23-06

Each OSV carrying NLSs in bulk in integral tanks or fixed independent tanks must have the Cargo Record Book. Vessel has not been using Cargo Record Book.

Records of daily hours of rest must be maintained onboard the vessel. Each seafer must receive a copy of the records pertaining to them, which will be endorsed by the master and by the seafarer. Vessel is logging working hours in bridge log but have not given seafarers the opportunity to endorse the data in writing.

Fire and abandon-ship drills must be held on each vessel in alternate weeks. Drills are being conducted inconsistently more than three weeks apart.

Each vessel must have marine charts of the area to be transited that are currently corrected. Vessel had been operating with nautical charts that had not been updated since April 2024 (two months old).

Each entrance to a space storing C02 cylinders must be marked in accordance with CG regulations. CO2 room doors were missing required markings. 46 CFR 131.817

Each vessel must be arranged so that the navigation lights can be energized from at least two sources of electricity. Nav lights couldn't be turned on upon loss of main source of power.

Vessel must undergo an annual inspection within 3 months before or after each anniversary date. Vessel was due to complete annual inspection by completed inspection by 04 July 2020. Inspection was completed on 12 Aug 2020. Vessel reported overdue inspection when scheduling. 46 CFR 126.510(a).

At least one abandon-ship drill each 3 months must be held at night, unless the master determines it unsafe. Vessel was completing required drills, but were being done during daylight hours. 46 CFR 131.530(b)(9)

Each OSV must carry lifebuoys approved under approval series 160.150 or 160.050. Lifebuoy on the port bridge wing had worn line attached around buoy. Crew renewed line. 46 CFR 133.70(a)
