Pre-lube oil pump for stbd engine was not isolated when emergency shutdown device was activated. Cooling water expansion tanks for port/stbd z-drives, fwd/aft bow thrusters, SSDG's 1-3, and the crane generator were found missing mechanical damage protective devices and valve wheels. Electrical components, wiring and penetrations found in disrepair in following locations: stbd Z-drive room doors, overhead of the upper stairwell landing for entry into E/R from main deck, cable gland at bottom of hydraulic motor controller located above shore connection box stbd side of E/R, emergency steering station had exposed electrical components, shore power box on stbd side of E/R cover was missing and improper wire connections, stbd main pyrometer junction box was missing covering, knife switches for the battery banks A and B, located on the port side of the aux. machinery space were tied closed, and both the SSDG's and crane generator battery boxes had damaged ventilation tubes so as to allow ingress of water/fluids. 74SOLAS(04) Ch. The termination of sounding pipes are to be fitted with self-closing blanking devices and with self-closing control cock located below the blanking device for the purpose of ascertaining before the blanking device is opened so that oil fuel is not present. Provisions shall be made so as to ensure that any spillage of oil fuel through the control cock involves no ignition hazard. Sounding tubes, located in the engine room just fwd of the SSDG's, were found to have their self-closing devices tied open and missing plugs.
