The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: Smoke Detector in Bow [name withheld] indicated that fire location was in Engine Room when tested on ECR Fire Detection Panel. 74SOLAS(20)II-2/14.2.1

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. Emergency Fire Pump mechanical seal & housing were leaking during operation.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea. Manual operation of the watertight door in the emergency steer gear compartment is inoperable. Repair to the satisfaction of class. 74SOLAS(20) I/11(a)

All ships irrespective of size, shall have a properly adjusted magnetic compass. Bridge magnetic compass reads 135M. Steering room magnetic compass reads 229M. Gyro compass reads 100T. 74 SOLAS (14) V/19.2.1.1

Fire-fighting systems and appliances shall be kept in good working order. Fixed CO2 system in paint locker was not serviced since 2019.

Each Cargo vessel must carry liferafts with an aggregate capacity on each side sufficient to accomodate the total number of persons on board. Vessels' liferafts not on board.

Each Cargo vessel must carry an immersion suit approved under approval series 160.171. 19 immersion suits are not CG approved. Total persons allowed restricted to 37. 46CFR 199.273

Each cargo vessel must carry at least one rescue boat. Rescue boat gas tank degraded.

An inspection either general or partial depending upon the circumstances shall be made whenever any important repairs are undertaken. Vessel self-reported fuel holding tank-top 4-Center has pitting and wastage. An approx 4"X4" section has been temporarily repaired with "splash-zone" epoxy. Vessel will transit to Charleston SC on or about 09Nov to offload equipment and gear, then proceed to Tampa Ship LLC Tampa Fl. OOA 12Nov to conduct permanent repair. Vessel will arrive at Tampa Ship between 17-21 Nov. Vessel is to contact Sector Hampton Roads if this time-line changes. Vessel is not to carry any cargo or fuel in affected tank, or conduct any operations en-route. Vessel is to contact CG Sector St. Petersburg to schedule repair oversight. 46CFR91.45

Insulation in A-60 overhead in EDG space above fuel tank had been removed during yard period. Replace A-60 insulation to manufacture recomendations.

1 (EACH) fire hose failed during pressure test.Replace new-in-kind.

Found 3/8" Main Deck Plate area between FR 50 & 86 (approx) to be wasted and holed. Provide repair proposal to the attending Marine Inspector.

Complete repairs of the Main Deck Plate area FR 50 to 86 in accordance with the approved repair proposal.

Found several deck and bulkhead penetration transit devices with openings, mismatched modules (different manufactures in same transit device). Inspect all transit devices, utilize single approved system or method (in accordance with manufacturers instructions) to ensure transit devices in fire boundaries maintain the fire-tight/watertight integrity required, to the satisfaction of the attending Marine Inspector.

Vessel continued to operate beyond the Internal Structural Exam date of 11MAR2016, as required by 46 CFR 126.140 (c). Schedule and complete ISE with USCG before returning to service.

Replace turbo seals in both the Bow Thruster and Stern Thruster which are leaking oil.

Remove hold back devices in openings in Class A machinery spaces.

02 Lifebuoys found with bad retro.

Found panel ELP-203 on the 03 level electronics room missing 3 blanks.

Bolts painted and not serviceable.
