A vessel over 500 GT in Great Lakes service must be fitted with an efficient electronic sounding device. Captain reported intermittent operability of the depth sounder. The depth read out on the display was intermittent. Company is going to replace the transducer. Vessel authorized to continue to make shuttle runs in the Cuyahoga River based on Captain's assessment and statement that he can safely navigate. Once the transducer is replaced, MI will attend the vessel to inspect repair and ensure proper operation of the depth sounder.

Each lifeboat must have a lifeline properly secured to both sides and of sufficient length and strength. Both life lines were frayed and nearly parted.

Each lifeboat must be lowered into the water and operate with its assigned crew at least once every 3 months. Due to weather conditions, a drill could not be conducted at the dock. Captain must provide evidence of lifeboat drill within 30 days of departure from Ashtabula.

An inspection is required after important repairs are made. Vessel has multiple repairs required to the hull and structure of the vessel. These repairs must be inspected prior to departure from port.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. Vessel sustained hull damage to starboard exterior watertight door and frames. Make ballast tank #6 starboard available for inspection prior to departure from winter layup.

A survey shall be made every time a defect is discovered which affects the safety of the vessel or whenever any important repairs or renewals are made. While conducting ballast tank cleaning in preparation for winter-layup period, the vessel discovered multiple pinholes and fractures as listed below: - Crack in bulkhead between #1 and #2 Port Ballast Tank (repaired ABS report received 20 NOV 25 rpt no. 7323422-A) - Crack in bulk head between #1 and #2 Starboard Ballast Tank (repaired ABS report received 20 NOV 25 rpt no. 7323422-A) - Crack in bulkhead between the #4 and #5 port ballast tank - Crack discovered in the slope plate between the #5 starboard ballast tank and #5 cargo hold. Conduct repairs to the satisfaction of the attending Marine Inspector prior to departure from MSU Cleveland's AOR.

In general, compliance with structural standards by American Bureau of Shipping will be considered satisfactory evidence of the structural efficiency of the vessel. Vessel reported a leak in the #3 STBD Ballast tank suction pipe. Make repairs to the satisfaction to the attending Coast Guard Marine Inspector, or ABS Class Surveyor. Notify the local OCMI of any changes.

When a Load Line regulatory authority notes any unsatisfactory condition the vessel shall repair the condition to the satisfaction of the attending inspector. As 6" crack was noted between the #5 starboard ballast tank and the #5 cargo hold that must be repair prior to departing the Port of Cleveland. Temporary repair of the crack is approved and must be reevaluated for additional work during the 2026 winter maintenance period but no later than 31MAR2026.

A survey shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel. Cargo hatch #7 was bent during removal, preventing closure. Repairs shall be made to the satisfaction of the Coast Guard prior to departure from [name withheld]. 46 CFR 9.30-1(a)

A survey either general or partial shall be made every time an accident occurs or defect is discovered. The port ballast pipe ruptured between cargo gates 36 and 37 causing flooding into the cargo tunnel. Ballast pipe must be repaired in accordance with repair proposal. Company must also provide UT report for sections identified by the attending Marine Inspector prior to departure from Cleveland.

Sector Detroit received report that subject vessel had, according to the Master: a crew member disembarked the vessel at the port of Milwaukee due to unforeseen circumstances, specifically impending birth of a child. The vessel departed Milwaukee at 1919 EST on April 29th, 2025 and arrived in Port Gypsum, MI at 0005 EST on May 01 st, 2025. A replacement has embarked upon arrival in Port Gypsum. Sector Detroit MI notified Captain that the circumstances of the discharge were consensual and therefore required a temporary manning deviation request prior to disembarkation per D9 policy and CFR, not a after-discharge compulsory notification typical of a non-consent based discharge. Informed Master of requirements and provided policy. Sector Detroit to discuss requirements with company representative to prevent re-occurrence.

Periodic testing and inspection of low pressure heating boiler (Serial # CHI74-64) requires the following inspections/tests, after repair of boiler fitting: hydrostatic pressure test. These inspections and tests must be completed to the satisfaction of the attending Marine Inspector prior to boiler operation.

Periodic Safety Test Procedures must be in a step-by-step or check off format. Each test instruction must specify the equipment status, apparatus necessary to perform the tests, safety precautions, and safety control and alarm set points, the procedure to be followed, and the expected result. Unable to complete PSTPs for ship service generators. Tests to be performed by the crew did not match the test prescribed in the procedure. Crew did not want to perform prescribed tests due to the test procedure possibly causing catastrophic engine failure. Certified engine tech must verify proper test procedure, carry out tests and provide report to OCMI. Should the test procedure be different from approved PSTP, amendments must be submitted to MSC for approval.

Cargo vessels must meet the requirements in this subpart. Per Coast Guard letter dated May 1, 2024, the vessel was approved an alternative lifesaving arrangement with the caveat that an approved rescue boat and davit under 46 CFR 160.156 also be installed in place of the open top lifeboat. Plans for installation of a Coast Guard approved rescue boat shall be submitted to the OCMI within 30 days, and installation shall be completed prior to completion of winter layup 2026.

All general alarm bells shall be identified by red lettering at least 1/2 inch high. Various general alarms need to be re-stenciled.

Remote fuel oil shutoffs must be suitably marked. Fuel oil shutoffs on main deck need to be re-stenciled.

Each lifeboat must be launched with its assigned operating crew aboard and maneuvered in the water at least once every 3 months. Could not complete launch of lifeboat due to lifeboat being on the dockside of the vessel. Provide evidence of launch, release, and maneuvering of lifeboat within 30 days.

General arrangement of fire extinguishing appliances must be in accordance with the approved fire control plan. There was no fire blanket in the galley.

Master reported a fracture and leak in the #05 stbd ballast tank adjacent to the #05 cargo hold. Vessel is to make repairs within 14 days to the satisfaction of class and/or USCG.

M/V SAM LAUD reported a 1 foot chip in its propeller due to unknown reason. Vessel to load gypsum in Sector Detroit AOR then transit to Waukegan, IL to offload, then transit to Cleveland, OH where it will be assessed and repaired. ABS to attend and submit report to Sector Detroit.

Vessel Master reported an issue with the CPP system, after further investigation a defect was discovered in the servo pitch control connecting shaft which had sheared at the coupler, causing the CPP to be pitch to full ahead with no way to adjust except declutch. Temporary repairs sighted, thorough operational testing was witnessed pier-side, and tech report for temp repair was reviewed. ** Permanent repairs are required to the servo pitch control connecting shaft along with all associated appurtenances. Repairs and operational testing shall be completed to the satisfaction of the attending Coast Guard Marine Inspector.

Vessel Master notified Sector Detroit OCMI of consensual crew shortage of a third engineer. Master shall ensure a 3-watch system is maintained and work/rest requirements are met. Master attests to be sufficiently manned, that a 3-watch system will be maintained, and that work/rest requirements will be met. Prior to 5/30/2024 with a tentative port schedule of Waukegan, IL, a replacement with the same or greater credential of a 3rd engineer shall be found for the vacancy. If no replacement is found prior, the master must provide notification to the cognizant OCMI and resubmit the District 09, Temporary Manning Request Checklist: D9-WI-DOM-009(01)-F(01). 46 CFR 15.515(a)

While bringing the third generator on parallel, vessel suffered an energy spike causing the clutches on both MDE's to disengage. Vessel temporarily lost propulsion for two to three minutes until all systems were brought back online and into operation. Provide attestation from C/E to the proper operation of generators and main propulsion drives. 46 CFR 91.30-1(a)

Each vessel must have the following: an illuminated magnetic steering compass, mounted in a binnacle, that can be read at the vessel's main steering stand. The difference between the vessel's charted heading and the magnetic compass heading is 82 degrees West. Vessel is to make permanent repairs to the magnetic compass to ensure proper operation to the satisfaction of the CG within 14 days.

Each vessel must have the following: A current magnetic compass deviation table or graph or compass comparison record for the steering compass, in the wheelhouse. Vessel will need to swing new compass and develop a deviation table or graph withing 7 days to the satisfaction of the Coast Guard.

The lifeboat's center seat is deteriorated and needs to be repaired. Repaired to the satisfaction of Coast Guard within 30 days.

Those items which are found to be in unsatisfactory condition by a surveyor shall be repaired or renewed in order to place the vessl in a satisfactory condition. A pin-hole leak was noted in the #3 port ballast tank, (Sector Detroit Activity #7849337) and welded closed, as a temporary repair until an approved permanent repair may be completed, no later than the next scheduled dry-docking of the vessel.

Those items which are found to be in unsatisfactory condition by a surveyor shall be repaired or renewed in order to place the vessl in a satisfactory condition. A pin-hole leak was noted in the forepeak space, and welded closed, as a temporary repair until an approved permanent repair may be completed, no later than the next scheduled dry-docking of the vessel.

THOSE ITEMS WHICH ARE FOUND TO BE IN UNSATISFACTORY CONDITION BY A SURVEYOR SHALL BE REPAIRED OR RENEWED IN ORDER TO PLACE THE VESSEL IN A SATISFACTORY CONDITION. CROP AND RENEW DAMAGED AREA AT THE FIRST BILGE ROLL, AFT OF FRAME 72, LOCATED JUST BELOW THE SEWAGE TANK IN PORT BALLAST TANK #6. THE REPAIR MUST BE COMPLETED TO THE SATISFACTION OF THE COAST GUARD OR CLASS, BY THE COMPLETION OF THE NEXT SCHEDULED DRY-DOCK.

EACH VERITCAL LADDER MUST HAVE RUNGS THAT ARE UNIFORM FOR THE LENGTH OF THE LADDER. THE VERTICAL LADDERS AT EITHER END OF CARGO HOLD #3 HAVE RUNGS THAT ARE SUBSTANTIALLY ERODED AND MUST BE REPLACED TO THE SATISFACTION OF THE ATTENDING COAST GUARD INSPECTOR, PRIOR TO DEPARTURE.

Except as provided by 46 CFR 15.725 of this part, no vessel may be navigated unless it has in its service and onboard the crew complement required by the COI. Vessel made notification to MSU Cleveland of pending crew shortage due to the consensual discharge of the 2nd Mate. Master shall ensure a 3-watch system is maintained, work/rest requirements are met, and all maintenance performed in accordance with SMS. Master and Chief Engineer shall conduct a risk assessment prior to each cargo operation, underway transit, and special evolutions for the duration of the 2nd Mate's absence. If no replacement is found, the master/operator must provide notification to the cognizant OCMI at each port of call with the following documentation: 1) Work/rest logs for the previous period of sailing with crew shortage; 2) documentation that substantial effort was made to find a replacement; and 3) statements from the vessel's Master and Chief Engineer attesting that the vessel is sufficiently manned.

Delegated functions performed by, and statutory certificates issued by, an authorized classification society will be accepted as functions performed by the Coast Guard, provided that the classification society maintains compliance with all provisions of its agreement with the Commandant. ABS issued the vessel a Condition of Class for the fractured propeller, requiring the vessel to affect permanent repairs within 18 days. The vessel is permitted to transit the Cuyahoga River to conduct shuttle runs, at reduced RPMs. Vessel shall make permanent repairs to the propeller in accordance with the issued Condition of Class to the satisfaction of ABS.

Vital systems are those systems that are vital to a vessel's survivability and safety. For the purpose of this subchapter, the ballast systems are vital systems. The vessel's starboard ballast pipe was found leaking near the #3 forward portion of the ballast tank and the #1 ballast tank. The leaks from the ballast pipe are flooding the vessel's tunnel. Vessel is to affect permanent repairs to the satisfaction of Class prior to departure.

Vessel's starboard side fire main has a pin hole leak that was reported forward of Hatch 20 on the underside of the pipe. Vessel has scheduled repair team upon arrival in the Cleveland zone on 7Nov23 and ABS to survey. 46 CFR 91.30-1; 705 - Prior to Departing Cleveland, OH; ac.

Vessel's bow thruster was reported to be leaking hydraulic oil when in use. Vessel has tagged out equipment to prevent further discharge and will transit to discharge cargo and make appropriate repairs to bow thruster.

Fracture was found at the bottom of the bulkhead allowing ballast water to weep from No. 3 to No. 4 Starboard Ballast tank during cargo operations in Waukegan, Il. Effect repairs to the satisfaction of class (ABS) or Coast Guard Marine Inspector prior to departure Marine City, Michigan and submit class report to Sector Detroit and MSU Chicago.

Burst firemain pipe was discovered while offloading cargo in Waukegan, IL. Between cargo hatch No. 5 and 6. Crop and renew firemain section and hydro to 1.5 MAWP not less than 150 psi IAW 46 CFR 56.97-40(a)(7) Prior to departure from Calcite, Michigan. Notify Sector Sault Sainte Marie upon completion of repair.

Vessel reported a pinhole leak in the starboard fire main system piping. Vessel will make permanent repair to the system at NPOC - Calcite, [name withheld] repair shall be made to the satisfaction of the attending surveyor or marine inspector.

All piping, valves, and fittings shall meet the applicable requirements of subchapter F (marine Engineering) of 46 CFR. Fire main has leak on deck, starboard side. Make permanent repairs and test to the satisfaction of Class/Coast Guard within 7 days.

Vessel reported a weep from the #6 port ballast tank leaking into the E/R.

46CFR50.20-35 Marine Inspector decisions. ...Whenever it is necessary to make decisions in matters not specifically covered by regulationsin this subchapter or by referenced requirements, the marine inspector shall clearly state the reasons which caused him to arrive at such decisions. On January 1, 2023 the vessel reported that on their propeller, two of the four blades were missing approx. 2' of the blade tip. ABS Surveyor attended the ship on the evening of January 7, 2023. After reviewing the results of a vibration test, interviewing crew and inspecting components of the propulsion system (MDE, Redusction Gear, Shaft and propellor blades) the Surveyor noted that the vibrations were within acceptable limits, while operating at reduced RPMs. This gives both Clas, and the Coast Guard confidence to allow limited operations, at reduced speeds, while making "shuttle runs" to and from [name withheld] Steel Plant and [name withheld] Terminal, until 16JAN2023, when the ship will go into a maintenance period, and shall be required to make permanent repairs to the propeller, to the satisfaction of class, before 31MAR2023.

The Surveyor also found substantial cracks in a third blade on the propeller. In response to this situation, the vessel must replace this blade to the satisfaction of Class, prior to departure.

Vessel was unable to get lifeboat underway during inspection due to its location at the dock. Vessel is required to lower the lifeboat and have the crew maneuver it to meet the requirements of 46CFR199.180(d). Lifeboat lowering and maneuvering should be conducted in the presence of a CG MI.

Prior to operation of AUX Boilers, Automation Test Procedures and safety-valve testing is required to be conducted per 46CFR63.15-9 and 46CFR61.05-10.

First Aid Kit required onboard lifeboat and throughout vessel should be maintained in a operationally ready status. All first aid kits onboard had expired medication. Replace with first aid kits meeting Coast Guard Approval 160.41.

Vessels forward Masthead light is improperly plugged into an electrical outlet with an extension cord, bypassing vertical cable run and secured to outlet with electrical tape and paper tag reading "Masthead light - do not unplug". Other wire in same cable rack is being held to rack with plastic zip-ties. Make proper repairs to forward mast electrical components per 46 CFR 111.75-17.

Failure of the starboard main diesel engine clutch.

Vessel was unable to get lifeboat underway during the inspection due to its location at the dock. Vessel is required to lower the lifeboat and have the crew maneuver it to meet the requirements of 46CFR 199.80 (d).

Prior to operation of AUX boilers Automation test procedures are required to be conducted per 46 CFR 63.15-9.

Weight test in accordance with 46CFR199.45(b)(1) not conducted in order to maintain social distancing practices due to COVID-19.

Safety Valve on the Out Board AUX Boiler lifted at 54 psi which is above the 2% allowable standard in accordance with ASME Section one PG-72. Outboard boiler MAWP is 50 PSI. Vessel owner is required to repair/ replace safety valve and retest prior to the outboard boiler being placed back into service.

This modifies the requirement imposed by this office on 12 SEP 19. One of two steering gear pumps required to meet the auxiliary steering requirement is inoperable. The vessel maintains full steering capability with no loss of maneuverability, however the vessel now lacks the required redundancy for steering systems. The vessel is authorized to make transits within the MSU Cleveland area of responsibility (AOR) only. When transiting into or out of ports, harbors, rivers or other restricted waterways, the vessel shall be escorted by at least one (1) towing vessel capable of rendering adequate assistance should the vessel lose steering; more than 1 towing vessel is not required unless considered necessary by the vessel's master. The vessel is not required a tug escort while transiting on Lake Erie, outside of restricted waters, unless considered necessary by the vessel's master.

Prior to making a transit outside of MSU Cleveland's AOR while this CG-835V is in force, a vessel representative shall submit in writing to this office a detailed explanation of the vessel's status and the proposed transit for consideration by cognizant OCMIs on the transit route. Should the OCMIs on the transit route approve the proposed voyage this CG-835V must be further modified to authorize the proposed transit including any additional restrictions imposed by those OCMI's impacted by the transit, prior to departing the MSU [name withheld].

#2 steering gear is inoperable.

Prove proper operation of the steering gear system including meeting the requirements for an auxiliary steering capability as required in 46 CFR sub-chapter F, to the sanctification of the attending marine inspector within 14 days of the date on this CG-835V.

All restrictions imposed by this CG-835 shall be in force unless modified or cleared by MSU Cleveland marine inspector upon satisfaction that the vessel meets the requirements of 46 CFR sub-chapter F. Requests to modify or clear this CG-835V should be made to the point of contact listed below.

Make temporary repairs, to the satisfaction of Class, to stop the ingress of water noted in the #3 center void.

Function that freezes the rudder whenever the rudder doesn't match the helm command within 5 seconds, was extended to 7 seconds before it freezes the rudder. Ship is to track all alarms and failures for the next 14 days and report same to OCMI, for further evaluation.

Vessel report a loss of steering, and are awaiting a service tec arrival.

Steering gear alarms and controls do not operate as designed. Vessel may operate under temporary repairs until manufacturer's reps can attend the ship, but no longer than April 4, 2019.

Conduct lifeboat drill in the presence of Coast Guard Marine Inspector, IAW 46CFR199.190.

Nuisance alarms are causing the steering system to freeze if both pumps are not running. DVTP's need to be approved and tested.

The broken steering gear linkage arm must be permanently repaired to the satisfaction of the attending Coast Guard Marine Inspector from the cognizant Officer in Charge, Marine Inspection zone, prior to departure from the next port of call or by 03Dec2016.

Make permanent repairs to the port fuel line.

The lifeboat davit falls parted at a swage fitting while the lifeboat was in the secured position. The davit falls must be replaced prior to sailing.

Compass deviatiion table is not current. Provide updated deviation table within 07 days.

Vessel could not conduct lifeboat weight testing as required due to mooring arrangment. Conduct light weight and full weight test within 14 days.
