A hatch exposed to the weather must be watertight. MI observed Lazarette & Forward Peak hatch gaskets missing, compromising watertight integrity.

A bilge pipe piercing the collision bulkhead must be fitted with a screw-down valve located on the forward side of the collision bulkhead and operable from the weather deck. MI observed reach rod snapped below the deck level.

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who could be directly involved in a serious marine incident. MI observed no alcohol testing devices were currency on board the vessel.

Each fixed power bilge pump must be self priming. It may be driven off the main engine or other source of power. It must be permanently connected to the bilge manifold and may also be connected to the fire main. If of sufficient capacity, a power bilge pump may also serve as a fire pump. The impeller on the bilge pump was inoperable and unable to provide suction. 46 CFR 182.520

Examine and test by operation all watertight closures in the hull, decks and bulkheads. Marine Inspector observed forepeak deck hatch seized and inoperable. Repair/replace hatch. 46CFR176.25-5.2

Each ring life buoy must be of an approved type, constructed in accordance with subpart 160.050 of this chapter; except a ring life buoy that was approved under former subpart; 160.009 of this chapter may be used if it is in good and serviceable condition. Marine Inspector observed broken plug on ring buoy. Replace buoy.46CFR180.30-1(a)

At all inspections special attention shall be paid to insure that no fire- hazards exist and-that guards- or protective devices are provided in all hazardous places. Marine Inspector observed cracked weld on STBD side exhaust piping in steering lazarette. Repair cracked weld. 46CFR176.25-10(b)

All vessels shall be provided with bilge pumps in accordance, with Table 182.25-lo(&). Marine Inspector observed inop electric bilge pump. Repair/replace electric bilge pump.46CFR 182.25-10 (a)

The bilge pipe piercing the collision bulkhead shall be fltted with a screw-down valve located on the forward side of the collision bulkhead and operable from above the weather deck. In lieu thereof, a screw-down valve without a reach rod may be fitted to the bilge line on the after side of tho collision bulkhead if it is readily accessible under service conditions. Marine Inspector observed inop reach rod for bilge system in collision bulkhead. Repair/replace reach rod.46CFR182.25-5(1)(d)

The intent of this part is to insure that each vessel carry sufficient lifesaving equipment to provide for timely rescue ind survival of the personnel on board. Marine Inspector observed rescue platform missing securing mechanism. Install/replace corroded inop securing chain.46CFR 180.01-5(a)

All vessels shall be provided with bilge pumps in accordance, with Table 182.25-lo(&). Marine Inspector observed bilge/FF pump in engine room inop. Crew able to make repairs to the impeller and MI observed satisfactory operation of pump during inspection.46CFR 182.25-10 (a)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts of large enough scale to make safe navigation possible. The Marine Inspector (MI) observed the current chart was out-of-date.

Generators and motors designed for 40 degrees Celsius may be used in the 50 degrees Celsius ambient locations provided the machines are derated to 80% of full load rating, and the rating or setting of the overcurrent devices is reduced accordingly. An additional nameplate specifying the derated capacity shall be provided for each such motor and generator. The MI observed the generator is rated for 40 degrees Celsius, but does not have a nameplate specifying the derated capacity. Obtain a nameplate with the derated capacity or a nameplate showing the generator is rated for 50 degrees Celsius.

All life preservers and ring life buoys shall be marked with the vessel's name. The MI observed the ring life buoy needs to be updated with the new vessel name.

Remote fuel shut-off stations shall be marked in at least 1-inch letters indicating purpose of the valve and direction of operation. The MI observed the fuel shutoff valve on the back deck was not labeled properly.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. The MI observe the protective guards for the upper belts on the main engines were not installed.

Provisions shall be made by means of plugs, covers, etc. to prevent the admission of air into the space protected. The MI observed two (2) holes in the bulkhead of the steering gear compartment shared with the machinery space on the upper port side that were not sealed properly to prevent the passage of air.

In general, compliance with the standards established by a recognized classification society will be considered satisfactory evidence of structural adequacy of a vessel. The MI observed a crack in horizontal member below the port side main engine forward inboard engine mount.

Each hatch cover must have securing devices. The MI observed the machinery space hatches do not have a securing mechanism installed.

No vessel subject to inspection under 46 USC 3301 will be navigated unless it is under the direction and control of an individual who holds an appropriate license. The masters merchant mariner credential onboard the vessel was only endorsed for operating on inland waters. Vessel records and statements by the vessel master indicated operations beyond the demarcation line. Vessel shall not operate until a proper credentialed mariner is onboard, but may continue to operate on inland waters with current master. Vessel shall provide evidence to OCMI of a master with proper endorsement prior to operating outside of inland waters. 46 CFR 15.515

EPIRB is required to be tested monthly and logged per 46 CFR 185.728. Vessel did not provide any record of this being conducted beyond April 2018.

Manufacturer's instructions for onboard maintenance of survival craft and logs for such maintenance shall be available for inspection by the Coast Guard per 46 CFR 185.702. Neither were available for inspection.

Emergency instructions required to be carried onboard and be available to the crew per 46 CFR 185.510 were missing.
