Connections must be made to battery terminals with permanent type connectors. Spring clips or other temporary type clamps are prohibited. Five battery terminals (four on port and one on starboard engine batteries) were found with wing nuts installed to secure connections. Install permanent type connectors to the satisfaction of the attending marine inspector.

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. On board alcohol testing kits were expired. Replace kits to the satisfaction of the attending marine inspector.

A vessel must carry a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10). Vessel did not have 48 doses of analgesic medication on board.

A vessel must carry one floating water-light. Floating water light was inoperable. 705- Day light operations only

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. Engine room ventilation was unable to be closed off. 46 CFR 182.465(h)

Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. The installation and location must be to the satisfaction of the Officer in Charge, Marine Inspection. Fire extinguishers are not installed in designated locations. 46 CFR 181.520

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (21/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. Fire buckets were not on board. 46 CFR 181.610

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces. The indicating panel light was inoperable and the panel was not labeled. 46 CFR 182.530(a)

A submersible electric bilge pump may be used as a power bilge pump required by Table 182.520(a) ... provided that:(1) The pump is listed by an independent laboratory as meeting the requirements in UL 1113. Installed bilge pumps were not UL rated. 46 CFR 182.520(e)(1)

A pre-engineered fixed gas fire extinguishing system must be capable of manual actuation from outside the space. The manual pull cable for the pre-engineered system has a significant spot of rust and wear affecting its serviceability. Replace cable to the satisfaction of the attending Marine Inspector. 46 CFR 181.420(a)(2)

Shutoff valves, installed to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. Remote fuel shut off cable snapped. Repair or replace vessel’s means of fuel shutoffs for both engines. 46 CFR 182.455(b)(4)

A suitable metal marine type strainer, meeting the requirements of the engine manufacturer, must be fitted in the fuel supply line in the engine compartment. Strainers must be leak free. Strainers in engine compartment are missing bottom plugs. Install plugs onto the bottom of each strainer. 46 CFR 182.455(b)(6)

Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018. Sixteen adult and two child sized life jackets had deteriorated retroreflective material. Renew retroreflective material to the satisfaction of MI. 46 CFR 185.604

Each watertight compartment must be permanently fitted with a means to dewater that space. The provided independent, electric, submersible bilge pump in the engine room was not capable of dewatering the space due to excessive debris in the bilge. Remove the debris and prove proper operation of the bilge pump. 46 CFR 176.830

Normally unmanned spaces with through-hull fittings or non-watertight closures must be fitted with a bilge high level alarm that activates an audible and visual alarm at the operating station. The lazarette and the forward crew compartment bilge high level alarms were not operational. One additional crew member, in addition to those already required on the COI, shall be required when passengers have access to the vessel to conduct rounds of the lazarette and forward crew compartment bilges. Rounds shall be conducted every 15 minutes when the vessel is away from the dock. Prove proper operation of the bilge high level alarms in those spaces. 46 CFR 182.530

Vessel not required to have a power driven fire pump must have at least 3 - 2.5 gallon fire buckets. Only 2 fire buckets were provided. Provide 1 additional fire bucket. 46 CFR 181.610

Each survival craft must be secured to the vessel by a painter with a float-free link permanently attached to the vessel. The provided float-free link was not attached to the vessel. Vessel is restricted to a limited coastwise route no more than 3 NM from shore. 46 CFR 180.130(a)(1)

Each survival craft must be stowed so that when the vessel sinks the survival craft floats free. The provided buoyant apparatus is secured to the vessel with a strap and an expired hydrostatic release. Vessel is restricted to a limited coastwise route no more than 3 NM from shore. 46 CFR 180.130(a)(2)

All machinery and equipment on board the vessel must be installed and maintained in such a manner as to afford adequate protection from machinery failure. The provided hose end fitting on for the steering system in the rudder room are severely corroded. Renew hose end fittings. 46 CFR 182.100

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10 (incorporated by reference, see § 175.600 of this chapter) as amended and the owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. The owner or managing operator failed to provide servicing records for the portable fire extinguishers. Provide servicing records in accordance with NFPA 10. 46 CFR 176.810(b)(iv)

The marine employer shall ensure that all persons directly involved in a serious marine incident are chemically tested for evidence of dangerous drugs and alcohol in accordance with the requirements of 46 CFR 4.06. The marine employer did not have a procedure in place for testing crew members following marine incidents. Provide written procedures for post-incident chemical testing. 46 CFR 16.240

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel. Vessel does not have any means to test for presence of alcohol. Provide valid alcohol test devices. 46 CFR 4.06-15 (a)

Remote fuel shutoff stations must be marked in clearly legible letters at least 1 inch high indicating the purpose of the valve and direction of operation. The vessels fuel shutoff valves are not labeled. Properly label fuel shutoff valves. 46 CFR 185.608

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. The hydrostatic release for the EPIRB is expired and shall be replaced. Vessel shall not operate more than 3 nautical miles from land and not more than 20 miles from a harbor of safe refuge until the hydrostatic release is replaced. 46 CFR 185.740(b)

Equipment and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. The vessel did not have oxygen onboard as required by Work Instruction 31. The vessel’s crew is prohibited from entering the water until the provisions of Work Instruction 31 are met.46 CFR 176.800 (a)

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Vessel suffered damaged to the port hull following an allision with the pier while mooring. Vessel is restricted from operating until deemed safe by Marine Inspector, to include inspection of the hull and repairs. 46 CFR 176.600

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Child size life jackets were not stowed separately from adult size life jackets. Separate child life jackets from adult life jackets. 46 CFR 180.78

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines at the tank connection. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. Three of the four fuel shut off valves did not close properly. Ensure proper closure of manually operated fuel shut off valves. 46 CFR 182.435

Valves for removing water or impurities in fuel strainers must be provided with caps or plugs to prevent fuel leakage. Installed valves did not have plugs. Provide plugs for drain valves for fuel strainers. 46 CFR 182.455(b)(9)

The fuel shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. The starboard aft fuel tank emergency shut off valve was inoperable. Provide proper operation of the fuel shutoff valve prior to carriage of passengers for hire.

Vapor barriers must be provided where flammable and combustible liquids or vapors are present, such as machinery spaces. The forward bulkhead of the engine room was fitted with a vent leading to an unsealed compartment. Seal the forward engine room bulkhead to ensure it is vapor tight prior to carrying passengers for hire.

A vessel must carry one floating waterlight, unless it is limited to daytime operation, in which case no floating waterlight is required. The vessel’s floating waterlight was found to be inoperable. Vessel is restricted to daylight operations only until proper operation of the floating waterlight is proven.

The cognizant OCMI may require that a vessel and its equipment undergo any additional inspection deemed reasonable and necessary to determine that the vessel is suitable for the service in which it is employed. The vessel’s AED is expired, and no longer meets the requirements of Work Instruction 31. Vessel must be manned with full complement listed on COI until a new AED is provided onboard.

All cable and wire must be installed with metal supports spaced not more than 24 inches apart in such a manner to avoid chafing. The electrical wiring under the forward starboard cabinet was not supported. Provide supports IAW 46CFR183.340.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, and the vessel from electrical hazards. The oil pump wiring in the engine room was unconnected and exposed. Remove, properly terminate, or reinstall the oil pump wiring to the satisfaction of the Marine Inspector.

Flexible nonmetallic hose used in fuel oil systems must meet SAE J-1942. The vessel had an unauthorized priming bulb in the port fuel line. Remove the priming bulb and reinstall the fuel line.

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. The port and starboard battery banks have unmounted, exposed switches. Mount the switch in appropriate enclosures to prevent accidental contact.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts of large enough scale to make safe navigation possible. The vessel’s chart was expired. Provide an up-to-date chart onboard to the satisfaction of the Marine Inspector.

A hatch exposed to the weather must be watertight. The forward anchor locker hatch on the bow was removed. Reinstall the forward anchor locker hatch.

For the purposes of flexibility only, flexible hose installed in lengths of not more than 30 inches and subject to a pressure of not more than 5 psi may meet the following requirements: USCG type A1, A2, B1, or B2 may be accepted in accordance with 33 CFR 183, subpart J. Vessels fuel hose from tank worn and cracking. Hose must be USCG A1 in accordance with 183.558 (b)(2). 43 CFR 182.720 (e)(3)(v)

The owner or operator shall make a vessel available for drydock examinations. In accordance with U.S. Coast Guard Marine Safety Information Bulletin 09-20 "Vessel Inspections, Exams, and Documentation", the vessel's required drydock inspection has been deferred 90 days based on objective evidence provided by the owner and/or operator attesting to the vessel being fit for its intended route and service. The vessel's owner or operator shall ensure that the vessel completes a USCG Marine Inspector drydock inspection within 90 days of the drydock due date listed on the its COI. Failure to complete a USCG Marine Inspector attended drydock inspection may result in the vessel being restricted from carrying passengers.

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules, except that a vessel of more than 19.8 meters (65 feet) in length must also have navigation lights that meet UL 1104, “Standards for Marine Navigation Lights,” or other standard specified by the Commandant. The port running light has 3 out of the 5 LEDs inoperable. It no longer meets the standard.

Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of §80.1101(c)(2). The vessel's VHF-DSC is not properly configured to show/ broadcast the vessel's GPS coordinates and MMSI.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Found the fuel shut off covers require to be screwed off using a crank handle. Establish a more readily accessible method to quickly utilize the fuel shut off valves in the event of a fire.

Each life jacket must be marked with Type I retro-reflective material approved in 46 CFR 164.018 or other standard specified by the Commandant. The arrangement of the retro-reflective material applied after March 11, 1996, must be as specified by IMO Resolution A.658(16) (incorporated by reference; see 46 CFR 175.600). Half of the life jackets have retro-reflective tape that is wore and unsat.

On a vessel of at least 26 ft in length, a visual and audible alarm must be provided at the operating statoin to indicate high water level; vessel's midships and fwd visual indicators are not functioning. Prove proper operation of visual indicators.

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating; vessel's fwd bilge pump indicator light is not working. Prove proper operation of visual indicator.

A hydrostatic release unit when used in a float free arrangement must be approved under approval series 160.062 or 160.162 or other standard specified by the commandant; vessel hydrostatic release unit for life float is expired. Replace HRU. Vessel route restricted to not more than 3 NM from shore
