Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. When repairs are made during an emergency, the owner, managing operator, or master shall notify the OCMI as soon as practicable after such repairs or alterations are made. Repairs or alterations that affect the safety of the vessel include, but are not limited to, the: replacement, repair, or refastening of deck or hull planking, plating, and structural members; repair of plate or frame cracks; damage repair or replacement, other than replacement in kind, of electrical wiring, fuel lines, tanks, boilers and other pressure vessels, and steering, propulsion and power supply systems; alterations affecting stability; and repair or alteration of livesaving, fire detecting, or fire extinguishing equipment. Owner/Operator must notify local [name withheld] once repairs have been made in accordance with 46 CFR 119.220 and request Marine Inspector attendance to verify repairs and conduct operational testing.

Each vessel that relies on electricity to power the following loads must be arranged so that the loads can be energized from two sources of electricity: (i) The vital systems listed in § 119.710 of this chapter; (ii) Interior lighting except for decorative lights; (iii) Communication systems including a public address system required under § 121.610 of this chapter; and (iv) Navigation equipment and lights. (2) Except as provided in § 120.312 of this part, a vessel with batteries of adequate capacity to supply the loads specified in paragraph (a)(1) of this section for three hours, and a generator or alternator driven by a propulsion engine, complies with the requirement in paragraph (a)(1) of this section.

Electrical equipment on a vessel must be installed and maintained to: (b) Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical Vessel has unsafe electrical equipment consisting of exposed wires, open junction boxes, and wires pulled from stuffing tubes etc. Receptacles panels and various portable electrical equipment pose significant safety issues with exposed wiring and improper installation.

System design and installation must be in accordance with the Marine Design, Installation, Operation, and Maintenance Manual approved for the system by the Commandant. Identified 03 main co2 hoses leaking at the fittings. Additionally CO2 did not discharge in bow thruster room.

A device must be provided to automatically shut down power ventilation serving the protected space and engines that draw intake air from the protected space prior to release of the extinguishing agent into the space. Ventilation was not secured as designed when testing the protected space.

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. Valve handle did not fully close when exercised. valve condition is rusted and unreliable.

A vessel must be operated in accordance with applicable and regulations and in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. Vessel failed a fire drill twice showing insufficient knowledge to fight a fire in the engine room. Training was conducted and concepts were still not grasped.

The design of all exhaust systems must ensure minimum risk of injury to personnel. Protection must be provided in compliance with § 116.970 of this chapter at such locations where persons or equipment might come in contact with an exhaust pipe. Vessel's main engine exhaust shows signs of a leak allowing exhaust air to enter the steering compartment.

At each inspection for certification and at every other vessel inspection, quarters, toilet and washing spaces, galleys, serving pantries, lockers, and similar spaces may be examined to determine that they are serviceable and in a sanitary condition.

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes.

46 CFR 114.400 (b) Accommodation space (5, 6, or 7 depending on size, fire load, and furnishings) means a space that does not contain any cooking appliance other than a microwave oven or other low heat (maximum heating element temperature less than 121 °C (250 °F)) appliance used as a— (1) Public space; (2) Hall; (3) Dining room and messroom; (4) Lounge or cafe; (5) Public sales room; (6) Overnight accommodation space; (7) Barber shop or beauty parlor; (8) Office or conference room; (9) Medical treatment room or dispensary; or (10) Game or hobby room. Please refer to both tables 46 CFR 116.415 (b) and 46 CFR 116.415 (c) as defined by definitions located in 46 CFR 114.

In response to your requested extension to repairs: You have been granted a 45-day extension to conduct repairs by Diversified Fabricators and Erectors in Brunswick, GA. A repair proposal must be submitted to MSU Savannah for review. After approval, coordinate with the local marine inspector during critical repair stages, including crop out and renewal, fit-up, and final weld, along with any other necessary oversight. All other dry dock items should be managed with Sector Charleston. A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural examination at least once every two years

46 CFR § 161.002-6 (b) Each fire detection device must comply with the following standards (incorporated by reference, see § 161.002–1) as appropriate: (c) All devices must be tested by an accepted independent laboratory, as defined in § 159.010 of this subchapter, to meet the marine environment testing requirements in Table 161.002–6(c) of this section. The test parameters are found in IEC 60092–504 (incorporated by reference, see § 161.002–1). Ships fire detection panel is not operating. IDS came on board and determined the system needs a new panel. Vessel rep instructed to replace in kind/ with USCG approved panel.

46 CFR § 119.600 A self-propelled vessel must meet the applicable requirements for main and auxiliary steering apparatus in subchapters F (Marine Engineering) and J (Electrical Engineering) of this chapter. Vessels rudder angle indicator is not reflecting accurate due to a malfuntioning piece in the steering gear compartment. Vessel has ordered replacement part to ensure accurate rudder angle positions.

46 CFR 116.415 (i) A-Class bulkheads or decks must be composed of steel or equivalent material, suitably stiffened and made intact with the main structure of the vessel, such as the shell, structural bulkheads, and decks. They must be so constructed that, if subjected to the standard fire test, they are capable of preventing the passage of smoke and flame for 1 hour. In addition, they must be so insulated with approved structural insulation, bulkhead panels, or deck covering so that, if subjected to the standard fire test for the applicable time period listed below, the average temperature on the unexposed side does not rise more than 139 °C (250 °F) above the original temperature, nor does the temperature at any one point, including any joint, rise more than 181 °C (325 °F) above the original temperature: Repair missing and warn insulation in engine room under ladder well on starboard side.

Vessel needs to complete underway MOB Drill and sea trials prior to operating.

46CFR115.700; Permission for repairs and alterations.(a) Repairs or alterations to the hull,machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI.

46CFR121.300;A vessel must be fitted with ground tackle and mooring lines necessary for the vessel to be safely anchored or moored. The ground tackle and mooring lines provided must be satisfactory for the size of the vessel, the waters on which the vessel operates, subject to the approval of the cognizant OCMI.

Electrical equipment on a vessel must be installed and maintained to: Prevent electrical ignition of flammable vapors. Lazarette port aft light is missing the seal where the electrical cabling enters the light assembly. Complete repairs within 14-days.

Each lighting fixture globe, lens, or diffuser must have a guard or be made of high strength material. Four (04) lights in lazarette are missing the guards. Complete repairs within 14-days.

At each initial and subsequent inspection for certification the owner or managing operator shall be prepared to test the steering systems of the vessel and make them available for inspection to the extent necessary to determine that they are in suitable condition and fit for the service intended. Stbd steering gear motor fan inoperable; port steering gear and auxiliary steering gear operable. Complete repairs within 14-days.

Each outlet or junction box must have a cover unless a fixture canopy, switch cover, receptacle cover, or other cover is used. The cover for junction box in lazarette space was missing post-dry dock, new junction box to be ordered. Complete repair within 14-days.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of electrical equipment and systems, including the following: Operational test of all emergency electrical systems. Switchboard indicated a hard ground, diagnosis and repair within 14-days.

On vessels with no overnight accommodation; public spaces that may be assumed to be occupied by a large number of persons when passengers are on board need only be served by a manual alarm system that meets the requirements in part 76 in subchapter H of this chapter. The alarm boxes must be located in the vicinity of each required exit, and easily seen in case of need. Engine room manual pull alarm box unable to latch, secured with tape. System operational, replaced pull box within 14-days.

The construction and arrangement of the vessel must allow for safe operation of the vessel in accordance with the terms of its COI. Vessel is safe to transit from Sector Charleston AOR to Savannah in order to complete all final repairs and checks. Vessel needs an in-service exam by MSU Savannah prior to carrying passengers.

Each survival craft must be stowed in a position that is readily accessible to crew members for launching and be ready for immediate use, so that crew members can carry out preparations for embarkation and launching in less than 5 minutes. While securing the rescue boat back in the cradle, after the man overboard drill, the wire rope on the winch snapped rendering the rescue boat to be launched in the event of a man overboard. Make appropriate repairs and demonstrate satisfactory operation to the satisfaction of the attending MI prior to carrying passengers.

Unless otherwise specified, vessel owners must pay the fees required by this subpart before inspection or examination services are provided. There is no evidence that vessel inspection user fees were paid. Pay all user fees and provide documentary proof that payment was submitted before any further inspection or examination services are provided to the satisfaction of the attending MI.

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. Enclosure for smoke detector number 48 in engine room had a broken cover plate. Make appropriate repairs to the satisfaction of the attending MI prior to October 13, 2022, otherwise the vessel will be restricted from the carriage of passengers.

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. Light fixture was broken behind generator switchboard. Make appropriate repairs demonstrate satisfactory operation to the satisfaction of the attending MI prior to October 13, 2022, otherwise the vessel will be restricted from the carriage of passengers.

Electrical equipment exposed to the weather must be watertight. Electrical outlet by fire station number 9 is not watertight. Make appropriate repairs to the satisfaction of the attending MI prior to October 13, 2022, otherwise the vessel will be restricted from the carriage of passengers.

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. Port side main electrical panel was exposed. Make appropriate repairs demonstrate satisfactory operation to the satisfaction of the attending MI prior to October 13, 2022, otherwise the vessel will be restricted from the carriage of passengers.

Nonconducting mats or grating must be provided on the deck in front of each switchboard and, if accessible from the rear, on the deck in the rear of the switchboard. Nonconducting mats were missing from switchboards in engine room. Make appropriate repairs to the satisfaction of the attending MI prior to October 13, 2022, otherwise the vessel will be restricted from the carriage of passengers.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Deck plates in the engine room were not secured creating a trip hazard. Make appropriate to the satisfaction of the attending MI prior to October 13, 2022, otherwise the vessel will be restricted from the carriage of passengers.

Gauges to indicate engine revolutions per minute (RPM), jacket water discharge temperature, and lubricating oil pressure must be provided for all propulsion engines installed in the vessel. The gauges must be readily visible at the operating station. The high jacket water temperature and low oil pressure gauges for the starboard main engine were inoperable. Make appropriate repairs demonstrate satisfactory operation to the satisfaction of the attending MI prior to October 13, 2022, otherwise the vessel will be restricted from the carriage of passengers.

Nonmetallic piping materials, including nonmetallic flexible hose assemblies, must meet the requirements of § 56.60–25 in subchapter F. 3 inch flex hose on Portside forward bow thruster engine is degraded and needs to be replaced. Make appropriate repairs demonstrate satisfactory operation to the satisfaction of the attending MI prior to October 13, 2022, otherwise the vessel will be restricted from the carriage of passengers.

Nonmetallic piping materials, including nonmetallic flexible hose assemblies, must meet the requirements of § 56.60–25 in subchapter F. Portside steering gear ram flex hose is degraded and needs to be replaced. Make appropriate repairs demonstrate satisfactory operation to the satisfaction of the attending MI prior to October 13, 2022, otherwise the vessel will be restricted from the carriage of passengers.

Remote fuel shutoff stations must be marked in clearly legible letters at least 1 inch high indicating purpose of the valve and direction of operations. Bow thruster shut off not marked. Make appropriate repairs demonstrate satisfactory operation to the satisfaction of the attending MI prior to October 13, 2022, otherwise the vessel will be restricted from the carriage of passengers.

Auxiliary machinery of the internal combustion piston type must comply with the provisions of this part. Bow thruster engine is discharging exhaust in to the space, ensure engine is arranged in accordance with manufacture. Make appropriate repairs demonstrate satisfactory operation to the satisfaction of the attending MI prior to October 13, 2022, otherwise the vessel will be restricted from the carriage of passengers.

The master or person in charge of a ship to which this section applies shall ensure that the ship is not operated unless a garbage management plan is on the ship and that each person handling garbage follows the plan. The master was unable to produce a waste management plan during the inspection. Develop a garbage management plan to the satisfaction of the attending MI prior to October 13, 2022, otherwise the vessel will be restricted from the carriage of passengers.

Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck from all passenger and crew accommodation spaces located below the main deck. During a test of the emergency lighting, the exit sign and emergency light on the aft stairwell on the main deck was inoperable. Crew replaced the light and it was found to be fully operational – corrected on the spot.

If during the inspection of a vessel, the vessel or its equipment is found not to conform to the requirements of law or the regulations in this subchapter, the marine inspector will point out deficiencies observed and discuss all requirements with the owner, managing operator, or a representative thereof. Attending MI identified a fault for the "first deck bathroom head pull-station" on the fire alarm detection panel. Require third-party technician report of functionality.

Immediately after addressing resultant safety concerns, the owner or master shall notify the nearest OCMI whenever a vessel is involved in a marine casualty consisting of an occurrence adversely affecting the vessel's seaworthiness or fitness for service... marine casualty includes: fire, flooding, [or] damage to fixed fire extinguishing systems, lifesaving equipment, power-generating equipment, or bilge pumping systems. On 04/30/2022, the vessel had an unintended ingress of water into the engine room due to a failed plug on the port generator heat exchanger. This failure caused flooding of the engine room to a level exceeding the high water level alarm in the bilge. The vessel used the bilge discharge to de-water the space, discharging into the Brunswick River, a navigable waterway of the U.S. The cognizant OCMI was not notified of the marine casualty.

Each vessel must be provided with a visual and audible alarm at the operating station to indicate a high water level in each normally unmanned engine space. Attending MI tested the high water level alarm in the engine room bilge and observed a only visual alarm at the operating station, the audible alarm was inoperable.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel may not be made without the approval of the OCMI, except during an emergency. When repairs are made during an emergency, the owner, operator, or master shall notify the OCMI as soon as practicable. Attending MI identified holes on the aft deck which had been patched with an unapproved epoxy repair. The aft deck is above the lazarette space and is normally only accessible to crew to transit to the engine room. In an abandon ship situation, passengers would egress from the vessel into liferafts from the aft deck. The OCMI had not been notified of these repairs. Provide repair plan to cognizant OCMI for approval prior to beginning repair work. Repairs affected will be of a temporary nature until vessel enters drydock on 11/01/2022 to conduct crop and renew of aft deck metal plating.

Piping used in a vital system must meet the requirements of Subchapter F. Attending MI identified a soft patch on the raw water discharge pipe for the port main engine. Additionally, the raw water discharge pipe for the port generator had two welds wrapped with flex-tape. The OCMI had not been notified of these repairs.

Exhaust gas must not leak from the piping or any connections. Attending MI identified an exhaust leak into the lazarette space. The following defects were found: port generator exhaust muffler - 2 cracked welds; port main engine exhaust muffler - 3 cracked welds and a hole on top of the muffler under the outboard bracket; and stbd generator exhaust muffler - 01 cracked weld. Provide repair plan to cognizant OCMI for approval prior to start of work. Repairs must be verified complete by attending MI.

The person in charge of an oceangoing ship that cannot discharge oily mixtures into the sea in compliance with paragraphs (a), (b), (c), or (d) of this section, must ensure that those oily mixtures are: (1) retained on board; or (2) discharged to a reception facility. Attending MI identified oily-water mixture in the engine room bilge at a level that required discharge to reception facility. Provide evidence of discharge of oily-water mixture to authorized reception facility prior to movement. In addition, Owner or Master shall provide a plan that describes the retention and disposal of oily-water mixture created by vessel, to the satisfaction of the OCMI.

No marine employer shall engage or employ any individual to serve as a crew member unless the individual passes a chemical test for dangerous drugs for that employer. During a review of the marine employers drug and alcohol program, it was discovered that eleven (11) persons with safety sensitive positions had no proof of preemployment testing/ or in random program . Provide preemployment and Random enrollment for all persons assigned to safety sensitive duties to the satisfaction of an attending Coast Guard Marine Inspector prior to October 13, 2021.

Vessels to which sub chapter K applies must meet the applicable provisions in sub chapter S (Subdivision and Stability) of this chapter. The company made several changes to the second deck as documented in the vessel's stability letter and stability information. Vessel was calculated for stability indicating 11 tables on the second deck. These tables were replaced by 43 slot machines. Provide updated calculations with the weight changes by the addition of 43 slot machines on the second deck and provide an amended stability letter to the satisfaction of an attending Coast Guard Marine Inspector prior to October 13, 2021.

The hull, structural bulkheads, columns and stanchions, superstructures, and deck houses on a sub chapter K vessel must be composed of steel or equivalent material to provide adequate structural fire protection. There was a small hole identified inside the forward overhead, starboard side leading into the steering gear compartment as pointed out to the vessel representative. Make necessary and adequate repairs to the small hole as identified to the satisfaction of an attending Coast Guard Marine Inspector prior to October 13, 2021.

The hull, structural bulkheads, columns and stanchions, superstructures, and deck houses on a sub chapter K vessel must be composed of steel or equivalent material to provide adequate structural fire protection. There was a small hole identified on the back deck on the open deck as pointed out to the vessel representative. Make necessary and adequate repairs to the small hole as identified to the satisfaction of an attending Coast Guard Marine Inspector prior to October 13, 2021.

Windows and other openings at the operating station must be of sufficient size and properly located to provide an adequate view for safe navigation in all operating conditions. The windows on the navigation bridge did not provide adequate protection from the elements and showed signs of leaking. Address all the leaks and ensure all windows on the navigation bridge are weather tight to the satisfaction of an attending Coast Guard Marine Inspector prior to October 13, 2021.

Each vessel that relies on electricity to power the electrical loads must be arranged so that the loads can be energized from two sources of electricity. None of the gauges on the standby generator were operating properly while under load. Make repairs to the standby generator and demonstrate proper operation to the satisfaction of an attending Coast Guard Marine Inspector prior to October 13, 2021.

A door in a stair tower, stairway, and main vertical zone bulkhead of a sub chapter K Vessel must have a hold back mechanism for a door that will allow the door to be released locally, on loss of power or upon signal from the control space. The hold back devices on doors in the main vertical zone on two doors were manual release only. Remove all hold back devices and test all fire screen doors to the satisfaction of an attending Coast Guard Marine Inspector prior to October 13, 2021.

A door in a stair tower, stairway, and main vertical zone bulkhead of a sub chapter K Vessel must have a hold back mechanism for a door that will allow the door to be released locally, on loss of power or upon signal from the control space. The hold back device on FSD #3 on the third deck was not releasing upon control from the operating station. Make repairs to the controls for the fire screen door releases and demonstrate proper operation of all fire screen doors to the satisfaction of an attending Coast Guard Marine Inspector prior to October 13, 2021.

Materials used in piping systems on a sub chapter K vessel must meet the requirements of this subpart and be otherwise acceptable to the cognizant [name withheld] an operational test of the port side ship service generator, a leak was observed at the raw water intake of the generator. Arrest the leak in way of the raw water intake and make necessary repairs to the satisfaction of an attending Coast Guard Marine Inspector prior to October 13, 2021.

Materials used in piping systems on a sub chapter K vessel must meet the requirements of this subpart and be otherwise acceptable to the cognizant [name withheld] an operational tests in the machinery space, a leak was observed in both the port and starboard side main heat exchangers. Arrest the leak in way of the raw water intake and make necessary repairs to the satisfaction of an attending Coast Guard Marine Inspector prior to October 13, 2021.

At each inspection for certification and at every other vessel inspection of a sub chapter K vessel, the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. During a walk through of the machinery space there was a significant accumulation of oil in the engine room bilge. Remove all excess oil and properly dispose any oil or oily rads and pads. Ensure bilges have been cleaned to the satisfaction of an attending Coast Guard Marine Inspector prior to October 13, 2021.

Indication of the rudder angle on a sub chapter K vessel must be provided both at the main steering station in the pilothouse and in the steering-gear compartment. The rudder-angle indicator must be independent of control systems for steering gear. The rudder angle indicator on the bridge was not properly mounted or secured. Properly mount the rudder angle indicator in a position where the person on the helm can visually see the degree of rudder applied to the satisfaction of an attending Coast Guard Marine Inspector prior to October 13, 2021.

Materials used in piping systems on a sub chapter K vessel must meet the requirements of this subpart and be otherwise acceptable to the cognizant [name withheld] a walk through of the steering compartment, a small raw water leak was observed on the port side, outboard main engine exhaust. Arrest the leak in way of the main engine exhaust and make necessary repairs to the satisfaction of an attending Coast Guard Marine Inspector prior to October 13, 2021.

Fire detection panel was displaying a trouble alarm, smoke detector on main deck near cash window was faulty.

Emergency escape lighting over STBD generator was inoperative.

STBD generator fuel filters are missing the heat shield on the fuel bowls.

Each vessel operating in warm water may be provided with life floats of an aggregate capacity that will accommodate at least 50% of the total number of persons permitted on board. 9 out of 18 - 22 Person life floats were found in serviceable. Vessel is restricted to 396 total people to include passengers and crew.

A weathertight door must be provided for each opening located in a deck house or companionway. Both starboard and port weathertight doors on the main were found to not be weathertight as light could be seen from the outside. Also, gasket material was missing.

All cable and wire must be protected from the weather. Vessel's shore tie receptacle is missing its weathertight caps and enclosures to ensure protection from the elements.

Equipment mounted on a hinged door of an enclosure must be constructed or shielded so that a person will not accidentally contact energized parts of the door mounted equipment when the door is open and the circuit is energized. Panelboards are missing blanks and/or breaker

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. Electrical fan in the galley port side wss found to have exposed wiring and capacitors.

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. Freshwater system in the engine room pressure switch was missing a cover plate.

Be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. The use of plastic tie wraps must be limited to bundling or retention of multiple cable installations, and not used as a means of support. On the main deck aft overhead, wire cable was found hanging by its wire and the conduit was not properly mounted.

The certificate of inspection (COI) issued by an Officer in Charge, Marine Inspection (OCMI), to a vessel required to be inspected under 46 U.S.C. 3301 specifies the minimum complement of officers and crew necessary for the safe operation of the vessel. After reviewing the logbook, it was determined the crew was working beyond the 12 hour within 24 hour work/rest requirement.

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. The electrical fan over the waterheater on the second deck next to the lifejackets had exposed wires.

Users must always maintain their Automatic Identification System in an effective operating condition and ensure the accuracy of all broadcasted dynamic, static and voyage information in accordance with 33 CFR 164.46(d). This vessel currently is transmitting a wrong value for the vessel’s IMO number and all the Antenna Distances (ANT A, ANT B, ANT C, and ANT D) are showing zero values. Load the correct data for the vessel into the AIS to the satisfaction of an attending Coast Guard Marine Inspector prior to February 6, 2021.

Annual inspection. Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel is overdue for an Annual Inspection. Vessel is restricted from carrying passengers until an Annual Inspection is completed.

Each vessel must be provided with a visual and audible alarm at the operating station to indicate a high water level in each normally unmanned space. Bilge high level alarm failed to activate in the Bow Thruster Room, Main Engine Room, Aux Engine Room (AC Room) and the Steering Gear Room and does not meet the minimum requirements as required by 46CFR119.530(a)

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection. Vessel's main engine exhaust has a leak in the lazarette/steering compartment that allows seawater to enter the space from the stern/transom of the vessel. The exhaust piping penetrates the stern of the vessel roughly 6 inches above the waterline. The hole in the exhaust piping allows water into one of the vessel's watertight compartments. The hole is roughly 1/4" by 6" long

Fuel Piping shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. Vessels Fuel Oil Shut off in engine room was not operating properly and does not meet the minimum requirements as required by 46CFR119. 455(b)(3).

Piping used in a vital system must meet 46CFR56.60 in subchapter F of this chapter. Vessels vital raw water discharge metallic piping was modified to include PVC (Plastic) piping and does not meet the minimum requirements as required by 46CFR119.710(c).

Nonmetallic piping materials, including nonmetallic flexible hose assemblies, must meet the requirements of 46CFR56.60–25 in subchapter F of this chapter. Vessel’s raw water discharge piping for generator in the engine room was modified to include nonmetallic piping materials and does not meet the minimum requirements as required by 46CFR119.720. The vessel will be restricted from departure and carrying passengers after January 11, 2020 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

A hatch exposed to the weather must be watertight. Vessels steering gear escape hatch coaming is wasted and not providing watertight integrity as required by 46CFR 116.1160 (a).

Electrical equipment used in the machinery space must be dripproof. Vessel has failed drip proof and abrasion protection wiring leading in to top aft engine in the bow thruster space and does not meet the minimum requirements as required by 46CFR120.210.

An exposed hazard, such as gears or rotating machinery, must be protected by a cover, cover, or rail. Vessels turbo air inlet for bow thruster engine is exposed does not meet the minimum requirements as required by 46CFR116.960.

Each window must be capable of withstanding the maximum load from wave and wind conditions expected due to its location on the vessel and the authorized route of the vessel. Vessels window on the main deck starboard side forward is broken and does not meet the minimum requirements as required by 46CFR116.1020.

Electrical equipment exposed to the weather must be watertight. Electrical cable leading in to anchor winch enclosure on the forecastle is not watertight and does not meet the minimum requirements as required by 46CFR120.210(b).

The use of twist-on type wire nuts is permitted as long as it is made within an enclosure and the insulated cap of the connector must be secured to prevent loosening due to vibration. Vessel is utilizing twist-on type wire nuts for light fixtures but are not in an enclosure and further does not meet minimum standard as required by 46CFR120.340(i).

Vessel’s means of escape pathways are limited in the AC Room due to storage of boxes and general lack of cleanliness (Refrigerant bottles that are not secure) and does not meet the minimum requirements as required by 46CFR116.500(e).

Electrical equipment used in a machinery space must be dripproof. Vessel has failed dripproof on number 49 heat/smoke detector in the engine room that does not meet the minimum requirements of electrical equipment used in machinery spaces as required by 46CFR120.210. The vessel will be restricted from departure and carrying passengers after January 11, 2020 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

The dimensions of a means of escape must be such as to allow easy movement of persons when wearing life jackets. There must be no protrusions in means of escape that could cause injury, ensnare clothing, or damage life jackets. Vessel’s means of escape pathways are limited in the Engine Room due to storage of un secured drums and general lack of cleanliness and does not meet the minimum requirements as required by 46CFR116.500.

Each pressure type wire connector and lug must meet UL 486A, “Wire Connectors and Soldering Lugs for Use With Copper Conductors,” or other standard specified by the Commandant. Vessel is utilizing unapproved pressure wire connectors aft port side generator battery box and the wire is not protected by sheathing and does not meet minimum standard of splices shall be made by a pressure wire connector (UL 486A) as required by 46CFR120.340(i). The vessel will be restricted from departure and carrying passengers after January 11, 2020 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Tubular gauge glasses, if fitted to diesel fuel tanks, must be of heat resistant materials, adequately protected from mechanical damage, and provided at the tank connections with devices that will automatically close in the event of rupture of the gauge or gauge lines. Vessel’s Tubular gauge glass is not adequately protected from mechanical damage due to loose fittings as required by 46CFR119.440(a)(7).

A hatch exposed to the weather must be watertight. Vessel’s engine room escape hatch coaming is wasted and a securing dog is missing and not providing watertight closure as required by 46CFR 116.1160. The vessel will be restricted from departure and carrying passengers after January 11, 2020 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Electrical equipment used in a machinery space must be dripproof. Vessel has failed drip proof and missing a control switch for the potable water system and does not meet the minimum requirements of electrical equipment used in machinery spaces as required by 46CFR120.210.

Vessels electrical shore tie penetrations (2 locations) in steering gear room do not prevent the passage of flame and smoke for one hour as required by 46CFR116.415. The vessel will be restricted from departure and carrying passengers after January 11, 2020 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Electrical equipment used in a machinery space must be dripproof. Vessel has failed drip proof on port side steering control box and does not meet the minimum requirements of electrical equipment used in machinery spaces as required by 46CFR120.210.

The vessel will be restricted from departure and carrying passengers after January 11, 2020 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

When life floats are stowed in tiers, the separate units must be kept apart by spacers. Vessel’s life floats are not properly spaced with a spacer by 46CFR117.137(f).

Each lifejacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel’s name. Lifejackets located on the 2nd deck aft located on the weatherdeck need to be relabeled with the vessel’s name as the name of vessel is not clearly legible as required by 46CFR122.604(b).

Each battery used in an item of lifesaving equipment, except inflatable survival craft equipment, must be replaced during the annual inspection if the expiration date of the battery has passed. The expiration date of the battery may be marked on the battery in clearly legible letters or the owner or managing operator may have a record of the expiration date from the manufacturer of a battery marked with a serial number. The vessel’s lifefloat batteries were found expired as required by 46CFR122.726(d).

Each battery used in an item of lifesaving equipment, except inflatable survival craft equipment, must be replaced during the annual inspection if the expiration date of the battery has passed. The expiration date of the battery may be marked on the battery in clearly legible letters or the owner or managing operator may have a record of the expiration date from the manufacturer of a battery marked with a serial number. The vessel’s aft lifering waterlight battery was found expired with the the incorrect type of battery as determined by the waterlight manufacturer as required by 46CFR122.726(d)

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment. Vessel has a bag covering the bow thruster bilge suction rendering the system ineffective and does not meet the minimum requirements as required by 46CFR119.500(a)

Update/Evaluate the approved Fire Control Plan for the vessel to reflect the space designation of the small galley space on the third deck, port side. This space was initially designated as a low risk service space (08) but has an oven in the space that has capability to produce heat above 250 degrees Fahrenheit. According to the definitions in 46 CFR 114.400 this space should be designated as a high risk service space (09). Upon determination/clarification of this space, ensure the suitability of the fire doors within the space. This deficiency must be corrected no later than January 9, 2019 to the satisfaction of an attending Coast Guard Marine Inspector. If it is not corrected by this date, the vessel will not be authorized to carry passengers.

The coaming in the doorway to the machinery space from the main deck aft is wasted. Make permanent repairs to the coaming to ensure the watertight integrity of the vessel in accordance with 46 CFR 116.116 (d)(1). This deficiency must be corrected no later than January 9, 2019 to the satisfaction of an attending Coast Guard Marine Inspector. If it is not corrected by this date, the vessel will not be authorized to carry passengers.

Identify and secure the ground fault observed on the vessel as indicated on the generator switchboard in accordance with 46 CFR 120.378 This deficiency must be corrected no later than January 9, 2019 to the satisfaction of an attending Coast Guard Marine Inspector. If it is not corrected by this date, the vessel will not be authorized to carry passengers.

Repair or replace four emergency escape lighting in accordance with 46 CFR120.432. This deficiency must be corrected no later than January 9, 2019 to the satisfaction of an attending Coast Guard Marine Inspector. If it is not corrected by this date, the vessel will not be authorized to carry passengers.

Properly secure the cover in way of the electrical panel located in the third deck small galley in accordance with 46 CFR 120.330. In addition, provide a suitable door that closes and is capable of being secured. This deficiency must be corrected no later than January 9, 2019 to the satisfaction of an attending Coast Guard Marine Inspector. If it is not corrected by this date, the vessel will not be authorized to carry passengers.

Remove all dead end wiring in the galley located on the third deck, port side aft in accordance with 46 CFR 120.200(b). This deficiency must be corrected no later than January 9, 2019 to the satisfaction of an attending Coast Guard Marine Inspector. If it is not corrected by this date, the vessel will not be authorized to carry passengers.

Properly secure the cover in way of the electrical panel located in the Engine Room in accordance with 46 CFR 120.330. This deficiency must be corrected no later than January 9, 2019 to the satisfaction of an attending Coast Guard Marine Inspector. If it is not corrected by this date, the vessel will not be authorized to carry passengers.

Provide suitable lagging around the dry exhaust piping in way of the bow thruster in accordance with 46 CFR 116.970. This deficiency must be corrected no later than January 9, 2019 to the satisfaction of an attending Coast Guard Marine Inspector. If it is not corrected by this date, the vessel will not be authorized to carry passengers.

Identify and eliminate water leaking on top of generator battery box in accordance with 46 CFR 120.200. This deficiency must be corrected no later than January 9, 2019 to the satisfaction of an attending Coast Guard Marine Inspector. If it is not corrected by this date, the vessel will not be authorized to carry passengers

Demonstrate proper operation of the mechanical ventilation damper located in the machinery space, port side in accordance with 46 CFR 119.465(h). The dampers were not closing to ensure an airtight seal in the event the fixed gas fire extinguishing system is engaged. This deficiency must be corrected no later than January 9, 2019 to the satisfaction of an attending Coast Guard Marine Inspector. If it is not corrected by this date, the vessel will not be authorized to carry passengers.

Demonstrate proper operation of the mechanical ventilation damper located in the galley on the third deck, port side, aft in accordance with 46 CFR 116.610. This deficiency must be corrected no later than January 9, 2019 to the satisfaction of an attending Coast Guard Marine Inspector. If it is not corrected by this date, the vessel will not be authorized to carry passengers.

Make repairs and demonstrate proper operation of the ventilation fan located in the CO2 cylinder storage space in accordance with 46 CFR 116.600. This deficiency must be corrected no later than January 9, 2019 to the satisfaction of an attending Coast Guard Marine Inspector. If it is not corrected by this date, the vessel will not be authorized to carry passengers.

Provide Spanner Wrenches for each fire hydrant in accordance with 46 CFR 118.310(e). This deficiency must be corrected no later than January 9, 2019 to the satisfaction of an attending Coast Guard Marine Inspector. If it is not corrected by this date, the vessel will not be authorized to carry passengers.

Properly mount in an approved bracket for the portable fire extinguisher located at the exit to the machinery space in accordance with 46 CFR 118.500(d). This deficiency must be corrected no later than January 9, 2019 to the satisfaction of an attending Coast Guard Marine Inspector. If it is not corrected by this date, the vessel will not be authorized to carry passengers

Provide a life line in way of one (01) life ring buoy that is at least 60 feet in accordance with 46 CFR 117.70(c) This deficiency must be corrected no later than January 9, 2019 to the satisfaction of an attending Coast Guard Marine Inspector. If it is not corrected by this date, the vessel will not be authorized to carry passengers.

Properly mark the life jacket storage locker located on the second deck, aft weather deck with the total number of adult lifejackets stowed inside in accordance with 46 CFR 122.604(f) This deficiency must be corrected no later than January 9, 2019 to the satisfaction of an attending Coast Guard Marine Inspector. If it is not corrected by this date, the vessel will not be authorized to carry passengers.

Properly demonstrate the deployment of a sampling of the life floats located on the upper deck of the vessel to ensure the operational readiness and current arrangement and to ensure the familiarity of the vessel's crew in accordance with 46 CFR 117.130(a) This deficiency must be corrected no later than January 9, 2019 to the satisfaction of an attending Coast Guard Marine Inspector. If it is not corrected by this date, the vessel will not be authorized to carry passengers.

Provide a corrosion resistant tag with permanently marked on it the date the new fall was installed and If the fall has been turned end for end, the date it was turned in accordance with 46 CFR 122.704(c). This deficiency must be corrected no later than January 9, 2019 to the satisfaction of an attending Coast Guard Marine Inspector. If it is not corrected by this date, the vessel will not be authorized to carry passengers

Make necessary repairs to the broken t-handle in way of the emergency fuel shut down servicing the tank in the bow thruster space and demonstrate proper operation of the shut down in accordance with 46 CFR 119.455(b)(3) This deficiency must be corrected no later than January 9, 2019 to the satisfaction of an attending Coast Guard Marine Inspector. If it is not corrected by this date, the vessel will not be authorized to carry passengers.

During a walk around of the vessel spaces, there was no fire detection system as indicated on the vessels fire control plan in the galley space on the third deck port side. Provide an approved fire detection system in accordance with the Fire Control Plan as required in 46 CFR 116.530 or provide documentation that fire detection is not needed in this space. This deficiency must be corrected no later than January 9, 2019 to the satisfaction of an attending Coast Guard Marine Inspector. If it is not corrected by this date, the vessel will not be authorized to carry passengers.

Provide an additional portable fire extinguisher classified as a 40 B:C in the vicinity of the exit to the bow thruster space in accordance with the required portable fire extinguisher table in 46 CFR 118.500(a). This space is classified as as a machinery space as defined in 46 CFR 114.400 and is required to be protected by a portable 40 B:C fire extinguisher near the exit. This deficiency must be corrected no later than January 9, 2019 to the satisfaction of an attending Coast Guard Marine Inspector. If it is not corrected by this date, the vessel will not be authorized to carry passengers.

All vessels except unmanned barges shall be provided with a satisfactory bilge pumping plant capable of pumping from and draining any water tight compartment Vessel bilge piping was missing multiple strainer boxes and had piping alterations. Vessel must prove system operation in each watertight space.

The installation of fuel lines, including pipe, tube, and hose, must comply with the requirements of this paragraph. There is a fuel leak in bow thruster fuel piping by the tank. Epoxy installed has to be removed and fuel leak corrected.

Fire control boundaries must be so insulated with approved structural insulation, bulkhead panels, or deck coverings. Bow thruster space the A-60 insulation by the hatch has improper repair with unapproved materials E/R port side underside of stair case by the vent the A-60 insulation is missing.

Cable and wiring be installed with metal supports spaced not more than 24inch apart, in such a manner to avoid chafing and other damage. Through out vessel wire supports are missing, worn or improperly installed. Plastic wire tie wraps are being used in lieu of the metal supports. Main areas of focus should be Thruster room and E/R.

Provide a VHF Marine Radio that is properly Programmed with the vessel’s MMSI number and interfaces with the vessel’s GPS to provide an accurate position in accordance with 47 CFR 80.905(a)(1) prior to January 19, 2018 and to the satisfaction of an attending Coast Guard Marine Inspector.

During a test of the vessel’s bilge high level alarms, the crew had to manipulate the alarm to get it to sound. Some of the alarms continue to sound after the crew reset the. Prior to January 19, 2018, demonstrate the proper operation and dependability of the bilge high level alarms in all spaces in accordance with 46 CFR 119.530 and to the satisfaction of an attending Coast Guard Marine Inspector.

Observed a fuel leak on the fuel supply line leading to the bow thruster. Prior to carrying passengers, make permanent repairs to the leaking pipe in accordance with 46 CFR 119.455 to the satisfaction of an attending Coast Guard Marine Inspector.

Observed a leak and a temporary repair soft patch on the raw water intake pipe from the sea valve to the strainer in way of the bow thruster. Prior to carrying passengers, make permanent repairs to the leaking pipe in accordance with 46 CFR 119.700 to the satisfaction of an attending Coast Guard Marine Inspector.

The emergency fuel shutoff valve in way of the fuel tank in the bow thruster space was broken. Prior to carrying passengers, make necessary repairs to the remote fuel shut off in accordance with 46 CFR 119.455(b)(3) to the satisfaction of an attending Coast Guard Marine Inspector.

The high level alarm IWO the bow thruster space failed to properly operate. Prove proper operation of the bilge high level alarm to the satisfaction of the attending CG Marine Inspector. 46CFR119.530

The exhaust pipe IWO the lazarette space has approx. 5 in. crack at the weld seam causing a leak of exhaust gas into the space. Prove proper repair of exhaust piping to the satisfaction of the the attending CG Marine Inspector. 46CFR119.430(b)

The bilge suction IWO of the HVAC compartment does not have a strainer fitted (flex hose connected to the end of the pipe). Provide proper installation of the a strainer to the satisfaction of the attending CG Marine Inspector. 46CFR119.510; 46CFR56.50-50(g)

Excessive oil was found IWO the lazarette and engine room space that prevented the testing of the bilge suctions. Remove excessive oil and prove proper operation of bilge suctions in the two aforementioned spaces to the satisfaction of the attending CG Marine Inspector. 46CFR115.820(b), 46CFR119.520

Electrical box IWO the lazarette has been secured with plastic zip ties. Provide proper installation of the electrical box IWO the lazarette to the satisfaction of the attending CG Marine Inspector.46CFR120.220(a)

The lifefloats installed on the vessel have numerous items that require attention such as faded retro reflective tape, webbing for grablines worn/torn, missing paddles, paddles w/o names, repairs with the foam,mark with vessel. 46CFR117.175(f)

The electrical panel IWO of the galley space are not marked for the spaces/equipment being utilized. Prove proper labeling of circuit breakers in the galley electrical panel to the satisfaction of the attending CG Marine Inspector. 46CFR120.220(d)

The rescue markings are worn off. Prove proper markings of the rescue boat to the satisfaction of the attending CG Marine Inspector. 46CFR122.604

Two (2) light fixtures IWO the bow thruster space does not have suitable guards. Provide proper installation of lens guard to the lights in the bow thruster space to the satisfaction of the attending CG Marine Inspector. 46CFR120.410(a)

The safety relief valve was found inoperable. Prove proper operation of the safety relief valve to the satisfaction of the attending CG Marine Inspector. 46CFR115.812, 46CFR61.10-5(i)

Oil level in the containment has reached the drainage hole for the containment area. Prove proper disposal/cleanup of the containment space to the satisfaction of the attending CG Marine Inspector. 46CFR115.830(a)

Vessel does not have a magnetic compass installed; vessel had the required compass installed when it was delivered to Sector Miami's AOR in NOV 1998. Prove proper installation of marine type magnetic compass with proper illumination to the satisfaction of the attending CG Marine Inspector. 46CFR121.402

Hatch exposed to the weather must be watertight. The hatch knife edge (cover & deck) IWO the lazarette is worn. Provide gasket for the hatch ensuring watertight integrity is met to the satisfaction of the attending CG Marine Inspector. 464CFR116.1160

Provide a specification sheet (cut sheet) for the new hydraulic rams and associated parts in way of the steering gear Upon Coast Guard approval of the material and installation on board, demonstrate the effectiveness of the steering system during underway sea trials prior to carrying passengers.

Remove all damaged wiring for the lighting system and the rescue boat winch and replace with UL approved marine grade, copper strand wiring to the satisfaction of an attending Coast Guard Marine Inspector. Remove and replace all oil soaked and/or damged lagging in way of the exhaust piping in the lazarette to the satisfaction of an attending Coast Guaard Marine Inspector. In addition, ensure all exposed sections/area on the dry exhaust pipes that penetrate the space are suitably protected with sufficient lagging material.

Remove all released/spilled hydraulic oil that accumulated in the forward bilge of the lazarette as a result of the seal failure in the steering gear system to the satisfaction of an attending Coast Guard Marine Inspector. In addition, ensure the recovered product is properly disposed of.

Crop and Renew wasted section of the sea water supply piping located in the forward lazarette to the satisfaction of an attending Coast Guard Marine Inspector.

Submit a repair proposal for the holed section of the overhead in the lazarette which penetrates into the CO2 storage space on the deck above.

Make proper repairs and demonstrate watertight integrity of two (02) hatches on the aft deck in way of the lazarette. At a minimum, the gasket material needs to be replaced and the dogging devices renewed to ensure the hatches provide a watertight seal.

Provide servicing for a total of five (05) portable fire extinguishers that were discharged while the crew attempted to extinguish the fire in the lazarette.

Owner, Operator and Master are reminded that any repairs or alterations to the hull, machinery or equipment on an inspected passenger vessel must be made only with prior approval of the Coast Guard.
