The vessel's current SMS in not sufficient to adequately maintain the vessel in a seaworhty condition. Since Sep 2017 the vessel has been issued over 80 CG-835 requirements, over 25 of which have been prior to departure, prior to movement, or prior to conducting cargo operation deficiencies. Conduct an ISM audit to include a thorough review of vessel maintenance records and history by a recognized organization to the satisfaction of the attending Marine Inspector
