Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. MI observed port and starboard throttle commands on new propulsion control system correspond to the incorrect engines. Vessel is restricted from departing Tiburon Ferry Terminal until satisfactory repairs are witnessed by a Marine Inspector. 46 CFR 182.200(a)

The construction and arrangement of a vessel must allow the safe operation of the vessel IAW the terms of its COI giving consideration to provisions for a seaworthy hull. In the forepeak, at the transverse frame in the point of the bow, there were two fractures in the weld. 46 CFR 177.100

Machinery, equipment, materials, and arrangements ...inspected IAW standards acceptable to the cognizant OCMI as good marine practice. There was small a fuel leak in the center compartment. 46 CFR 176.800(a)

Electrical equipment on a vessel must be installed and maintained to provide services necessary for safety under normal and emergency condition. Several lights in the Engine room were extinguished. 46 CFR 183.200 (a) 46 CFR 183.200 (a)

A penetration in a watertight bulkhead must be as high as practicable, and made watertight. Several bulkhead cable penetration throughout the vessel were no watertight. 46 CFR 179.320(c)

Each nozzle must...be capable of being changed between a solid stream and a spray pattern. Nozzle was seized and had to be removed for the hose to spray water. 46 CFR 181.320 (d)

A vessel must carry six hand red flares and six orange smoke flares or 12 red hand flares. Four red flares were expired. 46 CFR 169.553 (c)(2)

Each life jacket must be marked with Type I retroreflective material. Retroreflective tape on one life jacket was degraded. 46 CFR 185.604(b)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: U.S. Coast Pilot or similar publication. U.S. Coast Pilot was not present on board. 46 CFR 184.420(a)(2)

Each EAP education program must include … hot-line telephone number for crewmember assistance. EAP hotline was not posted. 46 CFR 16.401(a)

Each entrance to a space storing carbon dioxide cylinders, a space protected by carbon dioxide systems, or any space into which carbon dioxide might migrate must be conspicuously marked as follows: (1) Spaces storing carbon dioxide—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. VENTILATE THE AREA BEFORE ENTERING. A HIGH CONCENTRATION CAN OCCUR IN THIS AREA AND CAN CAUSE SUFFOCATION.” (2) Spaces protected by carbon dioxide—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. WHEN ALARM OPERATES OR WINTERGREEN SCENT IS DETECTED, DO NOT ENTER UNTIL VENTILATED. LOCK OUT SYSTEM WHEN SERVICING.” The reference to wintergreen scent may be omitted for carbon dioxide systems not required to have odorizing units and not equipped with such units. Marine Inspector (MI) observed, CO2 protected engine room, and void containing CO2 bottles were not marked with appropriate signs. 46 CFR 185.612 (g)

Each ring life buoy on a vessel must: (1) Be approved in accordance with § 160.050 in subchapter Q. MI observed deteriorated and broken grab line on approved life buoy on aft main deck. 46 CFR 180.70(b)(1)

The design of all exhaust systems must ensure minimum risk of injury to personnel. Marine Inspector (MI) observed deteriorated flexible hosing for generator exhaust in lazarette on starboard side. 46 CFR 182.430

Each portable fire extinguisher on a vessel must be of an approved type. MI observed fire extinguisher in galley pantry space was not in an approved bracket. 46 CFR 181.500(a)

S. The fire pump shall be selfpriming and of such size as to discharge an effective stream from a hose connected to the highest outlet. During a test of the fire pump, MI observed poor discharge volume from the main deck fire hoses. 46 CFR 181.15-5 (Old T).

Ground detection must be provided that: (1) For an alternating current system, meets § 111.05-27 in subchapter J of this chapter; and (2) For a direct current system, meets § 111.05-29 in subchapter J of this chapter. MI observed dual voltage generator did not have ground detection installed. Install required ground detection prior to next credit drydock. 46 CFR 183.324(c)

Vessels operated beyond the 20 nautical mile limitation specified in paragraph (a)(1) of this section, but not more than 100 nautical miles from the nearest land, must be equipped with a MF-DSC frequency transmitter. MI observed that installed MF-DSC radio was not receiving vessel position or vessel MMSI. Limited to 20nm. 46 CFR 184.502 47 CFR 80.903

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician. MI observed MF-DSC radio on the vessel was not broadcasting all required information. 47 CFR 80.903

A periodic examination made in accordance with the requirements of this subpart shall include the under-water hull and shall be of sufficient scope to determine that these items are in a satisfactory condition for the service intended. Marine Inspector observed widespread pitting on integral fuel tank hull plating and longitudinal frame members below waterline spanning from port chine to starboard chine. Submit repair proposal to satisfaction of Marine Inspector. 46 CFR 176.15-5 (OLD T)

Suitable covers, guards, or rails shall be installed in way of all exposed and hazardous places such as gears, machinery, etc. Replace missing machinery guard on stbd engine. 46 CFR 177.35-15(a) "OLD T"

Where segregation can be obtained by means of a watertight and/or vapor tight engine box, this will be accepted as a substitute for the required bulkheads. Gasket on engine room hatch is missing, replace and repair. 46 CFR 182.35-1(b) "OLD T"

All vessels shall be provided with a satisfactory arrangement for draining any watertight compartment. Aluminum bilge piping in center void servicing forepeak bilge suction is showing signs of wastage. Replacement in kind to satisfaction of marine inspector. 46 CFR 182.25-1 OLD T.

A visual and audible alarm must be provided at operating station to indicate a high water level. High water level alarms for lazarette, center, and forepeak inoperable. Repaired in presence of attending MI.

Each lifefloat must be individually attached to the painter. Aft lifefloats not connected to weak link. Corrected on the spot.

An adult life jacket must be provided for each person carried on board a vessel. Vessel currently carries 120 adult life jackets. Until vessel obtains 3 additional adult life jackets, total persons allowed on board will be decreased to 120 (3 crew and 117 passengers).

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Vessel carries two types of life jackets, but has donning instructions for only one type. Vessel must have donning instructions for all types.

The equipment required for lifefloats and buoyant apparatus must meet the following specifications: (4)Light. The light must be a floating waterlight approved under approval series 161.010 or other standard specified by the Commandant. One of the six life float lights did not work.

Except when operating on lakes, bays, and sounds, or rivers routes in calm weather, all hatches and openings in the hull must be kept tightly closed except when being used. The vessel's hatch for the #2 port void is unable to be closed and does not meet the watertight requirements for the vessel's route. Repair or replace hatch so that it can be closed and meets the required watertight conditions.

A visual and audible alarm must be provided at the operating station to indicate a high water level in the following space: a machinery space bilge. The vessel's float switch in the machinery space was unable to produce a visible and audible alarm upon actuation. Repair or replace the float switch so that an audible and visual alarm sounds when there is a high water level.

The float-free link required by §180.130(a)(1) must be of proper strength for the size of the life float or buoyant apparatus as indicated on its identification tag. The float-free link on board is not strong enough for the life float installation.

Each ring life buoy with a floating waterlight must have a lanyard securing the waterlight around the body of the ring life buoy. Lanyard was not secured around the body of the ring life buoy.

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Vessel's starboard navigation light was burnt out.

The individual bilge suction lines shall be led to a central control point or manifold. Each line shall be provided with a stop valve at the control point or manifold and in addition shall be provided with a check valve at some accessible point in the bilge line. The check valve provided was seized and prevented discharge of water from the forepeak bilge. 46 CFR 182.25-5(c)

03 vents on the forward portion of the superstructure, flush with the main deck, must be permanently fixed to prevent water flooding into the space.
