Received report that the vessel lost propulsion while underway. The following is required: a report from the Recognized Organization (RO) detailing the cause of the loss of propulsion, the corrective actions taken, and attesting to the full functionality of all associated equipment. 74 SOLAS (14) I/11

Resolution: Crew replaced faulty solenoid that provides pilot air to the main starting air valves. Class Surveyor inspected repair and tested engine in ahead and astern directions. Survey attached in documents.

74 SOLAS(97) Ch.II-2/15.4 The aft most crane room has significant leaks; the deck is covered in oil which has pooled in corners of the space and lagging is brown and black with moist oil from deck to overhead spanning over ten feet on forward bulkhead. This space contains the electrical equipment for crane.

74 SOLAS(97) Ch. II-2/20.1 In all ships general arrangement plans shall be permanently exhibited for the guidance of the ships officers. Plans shall be kept up to date. There have been multiple alterations to the fire control plan on boards using paper and tape, with no mark of approval attesting to official verification, have no approval stamp. Class boundaries are indistinguishable on all plans.

Based upon the general arrangement plan of the ship, there is a suspected compromised weld on the forward most port side ballast tank. This leak was not anchor wash and continued unabated for 5.5 hours.
