Vessels second fire pump, the bilge and ballast pump packing gland was tightened which seized motor from turning shaft. PSCO found three fire hoses that were tested and found unserviceable in that they leaked excessively, and one semi-portable foam fire extinguisher with an unserviceable nozzle assembly. The vessel has no spare fire hoses. Class is required to attend the vessel and survey E/R firefighting equipment. 17 a, c SOLAS 74 (as amended) Chapter II-2, Reg 14.2.2

Resolution: NKK attended. Found 5 additional hoses to be bad (totaling 8 bad hoses). 10 hoses brought on board and placed in locations of bad hoses. The two extra hoses were stored as spares.

Numerous leaks/pools of fuel were found in E/R from vital systems to include, but not ltd to, fuel oil purifying system piping as evidenced by oil soaked lagging on hot oil lines &the pooling of oil under all gens. PSCOs observed the substandard practice of using over 40 buckets for catching waste/leaking oil from machinery that were being stored haphazardly thruout the E/R, creating a safety haz.

Resolution: As per NKK, oil was cleaned up in engine room and leaks were repaired. All oil catching/stowage buckets were removed from the E/R.
