A hatch exposed to the weather must be water tight... Aft hatch going to lazarette had a torn gasket, replace to MI satisfaction. 46 CFR 179.360(a)

Each vessel must have the following... As used in this section, “currently corrected” means corrected with changes contained in all Notices to Mariners published by the National Geospatial-Intelligence Agency... reasonably available to the vessel. Vessel's Local Notice to Mariners was outdated. Replace to MI satisfaction. 33 CFR 164.33 (a) & (c)

The employer shall provide an Employee Assistance Program (EAP) for all crew members... Each EAP must include education and training on drug use for crew members and the employer's supervisory personnel as provided below: Each EAP education program must include at least the following elements: display and distribute of informational material; display and distribution of community service hot-line telephone number number for crew member assistance, and display distribution of the employers policy regarding drug and alcohol use in the workplace. MI observed no EAP information publicly available, display to MI satisfaction. 46 CFR 16.401(a)

For fixed-gas fire extinguishing systems, the inspections and tests required by Table 176.810(b) of this section, in addition to the tests required by 46 CFR 147.60 and 147.65. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Missing Fire-boy annual servicing report. 46 CFR 176.810

Vessels operated solely within 20 nautical miles of land must be equipped with a VHF–DSC radiotelephone installation meeting the requirements of §80.1101(c)(2). VHF radio missing no GPS and MMSI. 46 CFR 184.502 47 CFR 80.905 (a)(1)

Remote fuel shutoff stations must be marked in clearly legible letters at least 25 millimeters (1 inch) high indicating purpose of the valve and direction of operation. Fuel shut off valve not labeled. 46 CFR 185.608

Each lighting branch circuit must be protected against over current either by fuses or circuit breakers rated at not more than 30 amperes. No over current protection for main cabin light switch. 46 CFR 183.380 (g)

Over current devices capable of carrying the starting current of the motor must be installed to protect motors, motor conductors. No over current protection on anchor windless. 46 CFR 183.380 (h)

Battery installation must be located in a locker, room or enclosed box solely dedicated to the storage of batteries. Battery missing box covers. 46 CFR 183.354 (a)

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (2 1/2 gallon) buckets. No fire buckets on board. 46 CFR 181.610

Each life float must be fitted with a lifeline, pendants, two paddles, a painter, and a light. Life float missing light. 46 CFR 180.175(d)

Each paddle must be of at least 1,220 millimeters (4 feet) in length, lashed to the life float to which it belongs and buoyant. Paddles not latched to life float. 46 CFR 180.175 (f)(2)

The name of a vessel must be marked or painted in clearly legible letters and numbers:On each life float and buoyant apparatus. Life float missing vessel name. 46 CFR 185.604 (a)(2)

Each ring life buoy with a floating waterlight must have a lanyard of at least 910 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the waterlight around the body of the ring life buoy. Water light not tied off and secured to the ring buoy properly. 46 CFR 180.70 (d)(2)

A bilge suction must be fitted with a suitable strainer having an open area not less than three times the area of the bilge pipe. Missing bilge pick up strainer. 46 CFR 182.510 (b)

Cable and wire for power and lighting circuits must: Meet Section 310–13 of NFPA 70. Lazarette high water alarm wire broken. 46 CFR 183.340 (d)(1)

Each pressure type wire connector and lug must meet UL 486A. Housing nuts on found used on wiring in the lazarette. 46 CFR 183.340 (I)

When operating a vessel on a body of water where the discharge of treated or untreated sewage is prohibited by the Environmental Protection Agency under 40 CFR 140.3 or 140.4, the operator must secure each Type I or Type II device in a manner which prevents discharge of treated or untreated sewage. MSD valve was found open. 33 CFR 159.7 (b)

Each EPIRB must be tested monthly. EPIRB is not being tested monthly. Master of vessel shall ensure that each EPIRB must be tested and logged on a monthly basis. 46 CFR 185.728(a)

A vessel must comply with the applicable requirements for any radio and EPIRB installations, including the requirements for a station license and installation certificates to be issued by the FCC, as set forth in 47 CFR part 80. Vessel was missing FCC Telephony Certificate. 46 CFR 184.502

Vapor barriers must be provided in spaces where flammable and combustible liquids or vapors are present, such as machinery spaces. Machinery and fuel tank spaces lack vapor tightness. Vessel shall make all bulkheads vapor tight. 46 CFR 177.405(e)

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, and vessel from hazards. Machinery room had an exposed wire leading into a J box located on the aft bulkhead of the space which creates an unsafe condition. 46 CFR 183.200(b)

All cable and wire must be installed with metal supports spaced not more than24 inches apart. Vessel has multiple wires suspended without support. 46 CFR 183.340(b)(4)

Annual inspection. Your vessel must undergo an annual inspection within 3 months before or after each anniversary date. Vessel is overdue for 2022 annual inspection.

Except as otherwise provided in this section, rails or equivalent protection shall be installed near the periphery of all weather decks accessible to passengers and crews. 03 Deck rail stanchions mounts and several stanchion base mounts were found with cracks. Renew/replace rail and base mounts. 46 CFR 177.35-1 (a)

Wooden stanchion mounting support port side forward had split and was no longer secure. Submit repair proposal to renew wood stanchion mounting support. 46 CFR 177.35-1(a)

An adult life jacket must be provided for each person carried on board a vessel. 04 Kapok life jackets were not in good or serviceable condition. Life jackets need to be replaced to meet COI requirements. Vessel is limited to operating with 18 Adults and 02 Children. 46 CFR 180.71(a)

Each link certified by the manufacturer to meet the requirements of this subpart must have a corrosion resistant, waterproof tag attached to it. Both Buoyant Apparatus links are missing tags, MIs are unable to verify breaking strength. Attach missing tags to weak links. 46 CFR 183.420

Each navigation light must meet the technical standards of the applicable Navigation Rules. Unable to verify if red and green running lights are visible from 2 miles away IAW applicable Navigation Rules, due to the visible condition of the lens. Both port and stbd running light lenses need to be replaced. 46 CFR 160.073-20(a)

Each paddle must be of at least 4 feet in length, lashed to the life float to which it belongs and buoyant. 02 buoyant apparatus paddles are shorter than the required 4 ft. Both paddles need to be replaced. 46 CFR 180.175(f)(2)

Connections must be made to battery terminals with permanent type connectors. Battery connector on the negative terminal post was found loose. Crew tighten connector nut, satisfactory. 46 CFR 183.350(d)

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. The hydrostatic release unit is not marked with installation or expiration date. Crew member marked hydrostatic release unit with expiration date. 46 CFR 185.740(b)

Annual inspection. Your vessel must undergo an annual inspection within 3 months before or after each anniversary date. Vessel is overdue for 2021 ANNUAL INSPECTION.

SHUTOFF VALVES, INSTALLED SO AS TO CLOSE AGAINST THE FUEL FLOW, MUST BE FITTED IN THE FUEL SUPPLY LINES, ONE AT THE TANK CONNECTION AND ONE AT THE ENGINE END OF THE FUEL LINE TO STOP FUEL FLOW WHEN SERVICING ACCESSORIES. THE SHUTOFF VALVE AT THE TANK MUST BE MANUALLY OPERABLE FROM OUTSIDE THE COMPARTMENT IN WHICH THE VALVE IS LOCATED, PREFERABLY FROM AN ACCESSIBLE POSITION ON THE WEATHER DECK. WHEN THE OPERATOR CLOSED THE VALVE MANUALLY FROM THE DECK, MI OBSERVED THE VALVE DID NOT CLOSE TO STOP FUEL FROM TANK.

AT EACH SUBSEQUENTIAL INSPECTION MI MAY REQUIRE THAT A MAN OVERBOARD DRILL BE HELD UNDER SIMULATED EMERGENCY CONDITIONS. VESSEL WAS NOT ABLE TO GET UNDERWAY DUE TO DEFICIENCY DISCOVERED DURING INSPECTION.

VESSEL THAT HAS ON BOARD FCC TYPE CATERGORY 1 406 MHz EPIRB MUST INSTALLED TO AUTOMATICALLY FLOAT FREE AND ACTIVATE. RESTIRCTED IN OPERATION OF 3 NM.

DISCHARGE ENDS OF VENT PIPE SHALL BE HIGH ABOVE THE WATERLINE AND REMOTE FROM ANY HULL OPENINGS AND SHALL TERMINATE IN U-BENDS AS HIGH ABOVE DECK AS POSSIBLE. MI OBSERVED SHARP BENDS ON PORT AND STARBOARD DISCHARGE VENT PIPES. 46 CFR 182.20-35(3)(c) OLD T

DISCHARGE ENDS OF VENT PIPE SHALL BE FITTED WITH REMOVABLE FLAME SCREENS AND PERMIT CLEANING OR RENEWAL OF FLAME SCREENS OR ARRESTOR ELEMENTS. MI OBSERVED ON STARBOARD FUEL TANK VENT WAS PARTIALLY BLOCKED BY DIRT AND PAINT. 46 CFR 182.20-35(3)(d) OLD T

THE NAME OF VESSEL MUST BE MARKED IN CLEARLY LEGIBLE LETTERS AND NUMBERS ON EACH LIFE FLOAT AND BE MARKED WITH TYPE II RETRO-REFLECTIVE MATERIAL.

EACH RING LIFE BOUY MUST BE APPROVED IN ACCORDANCE WITH 160.050 SUBCHAPTER Q, RING BOUY MSHALL HAVE TYPE II RETRO-REFLECTIVE TAPE AND BE MARKED. DID NOT HAVE VESSEL NAME OR RETRO-REFLECTIVE TAPE ON IT.

VESSEL MUST CARRY UP TO DATE CHARTS AND PUBLICATIONS. MI OBSERVED CHARTS WERE NOT UP TO DATE AND LIGHT LIST WAS OUT OF DATE.

LIFE FLOAT SHALL BE STOWED IN A WAY THAT PERMITS MANUAL RELEASE FROM ITS SECURING ARRANGEMENTS. MI OBSERVED THE LINE ATTACHED TO THE FLOAT FREE WEAK LINK WAS THREADED THROUGH LINE HOLDER WOULD NOT FIT THROUGH ASSEMBLY TO RELEASE LIFE FLOAT.

An adult life jacket must be provided for reach person carried on board a vessel. 02 Kapok lifejackets were condemned. The vessel is restricted to the carriage of 16 passengers until 02 Adult Type I pfd's are placed in service onboard. 46 CFR 180.71(a)

Each vessel that operates on the high seas must have on board a FCC Type accepted category 1, 406MHz EPIRB installed to automatically float free and activate. Hydrostatic release for EPIRB expired. Must install valid hydrostatic release for float free arrangement. Vessel restricted to voyages no greater than 3nm from shore.

Each life float must be fitted with a lifeline, pendants, two paddles, a painter, and a light. Stern life float two wooden oars were rotted, split, and not in serviceable condition. Oars must be replaced.

46CFR 171.124(b) - Each hatch cover must have a securing device. Securing cable for lazarette hatch was severed, requiring replacement.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date U.S. Coast Pilot or similar publication. Vessel Coast Pilot out of date, requiring updated version.

The operator of each self-propelled vessel of 12meters or more in length shall carry on board and maintain for ready reference a copy of the Inland Navigational Rules. Vessel carried outdated version of the navigational rules publication. Required to replace with current copy.

Each ring life buoy must be marked with Type II retroreflective material approved IAW 46CFR 164.018. Ring life buoy on starboard side missing all required retrorelective material.

Replace battery in EPRIB. Vessel must not operate more than 3NM from land. 46CFR185.728.
